Registration No.: M Complimentary Insurance Citi Debt Consolidation Plan The following is a description of the group insurance policy (Master Policy Number P ) held by Citibank Singapore Ltd ( Citibank ), the Policyholder having a registered place of business at 5 Changi Business Park Crescent, Singapore for the benefit of Account Holders of Citi Debt Consolidation Plan ( Account Holders ). The policy of insurance is issued by AXA Insurance Singapore Pte Ltd. (Company Registration Number, D) ( AXA ), 8 Shenton Way #27-01, AXA Tower, Singapore is also the underwriter of the policy. This policy of insurance is the obligation of AXA. This insurance is not a bank deposit or obligations of or guaranteed by Citibank, Citibank, N.A., Citigroup Inc. or any of its affiliates or subsidiaries. The benefits under this group insurance: is free of charge for Account Holders. can be changed from time to time or discontinued altogether by notice to the Account Holders following a decision by Citibank and/or AXA. are available in accordance with the terms of our policy with AXA, extract of which is reproduced below. Account Holders and/or their authorized representative(s) will have the right to make claims on their own behalf with AXA as Insured Persons of this policy within the relevant terms and conditions as set out herein. SUMMARY OF BENEFITS Benefits Section 1 Section 2 Section 3 Accidental Death Total Permanent Disablement Loss of sight of both eyes; or Physical severance of two limbs; or Complete permanent paralysis Involuntary Employment Disruption Maximum Limit Payable The outstanding Indebtedness up to S$160,000. The Minimum Payment Due for every consecutive month up to a maximum of six (6) months. Page 1 of 6
2 Subject to the following terms and conditions, AXA will pay the benefits provided for under this Policy to Citibank for the purpose of repaying the outstanding amount owing to Citibank by the Account Holders. A. DEFINITION OF TERMS For the purpose of this Policy, the following definitions shall apply: Terms Accident Account Holder, Insured Person Citibank, Policyholder Citibank Account, Account Commencement Date Insured Event Indebtedness Injury Minimum Payment Due Meaning A sudden, unforeseen and fortuitous event which occurs during the policy period which solely results in Injury and which is not attributable to any illness or naturally occurring medical conditions. The person being the principal account holder of the Citibank Account which outstanding amount is covered under this Insurance. Citibank Singapore Limited Refers to a Citibank Ready Credit account held or maintained in connection with an Account Holder s Citi Debt Consolidation Plan. The date the Account Holder becomes the principal account holder of the Citibank Account. The events happening to the Account Holder, pursuant to which the benefit under this Policy is payable, namely: (a) Death or Total Permanent Disablement; and (b) Involuntary Employment Disruption. The total amount due and payable by the Account Holder in respect of the Account as at the date of Insured Event. Bodily injury resulting solely from an Accident occurring on or after the Commencement Date. The minimum payment due and payable by the Account Holder to Citibank as set out in the statement of the Account issued immediately after the occurrence of the Insured Event giving rise to a claim. 3 Pre-Existing Conditions Total Permanent Disablement The existence of any injuries, illnesses, disease or symptoms of the conditions whether known or unknown to the Account Holder that existed prior to the take up of the Account. Injury which totally prevents the Account Holder from attending to any business or occupation or if the Account Holder has no business or occupation, from attending to his/her usual duties. Total Temporary Disablement A state of incapacity which lasts for at least thirty (30) consecutive days that is directly caused by Injury and as a result of which the Account Holder is certified by the attending doctor to be temporarily unfit to perform every duty pertaining to the Account Holder s business or occupation. AXA AXA Insurance Singapore Pte Ltd B. SCOPE OF COVERAGE IMPORTANT NOTICE : This is an ACCIDENT policy and benefits will only be payable upon an Accident occurring. Subject to the terms and conditions provided below, AXA shall pay the benefit(s) to Citibank under this Policy as provided: SECTION 1 ACCIDENTAL DEATH BENEFIT In the event that the Insured Person sustains Injury resulting directly in death within three hundred and sixty five (365) days of the Accident, and upon due proof of the death, AXA shall pay to Citibank the amount of the Insured Person s Indebtedness as at date of death up to a maximum sum of S$160,000. SECTION 2 TOTAL PERMANENT DISABLEMENT BENEFIT In the event that the Insured Person sustains Injury resulting directly in Total Permanent Disablement within three hundred and sixty five (365) days of the Accident, and upon due proof of the Total Permanent Disablement, AXA shall pay to Citibank the amount of the Insured Person s Indebtedness as at date of Total Permanent Disablement up to a maximum sum of S$160,000. For the avoidance of doubt, Total Permanent Disablement shall refer specifically to the occurrence of any one of the following: Permanent Loss of sight of both eyes; Physical severance of two limbs; or Complete and permanent paralysis. Special conditions applicable to Sections 1 and 2: 1. AXA shall pay for only one benefit under either Section 1 or 2 but not both. Page 3 of 6
4 2. Any benefit payable under Accidental Death or Total Permanent Disablement shall be reduced by any benefit already paid under Temporary Employment Disruption, if any, in respect of such Injury. SECTION 3 - INVOLUNTARY EMPLOYMENT DISRUPTION BENEFIT This benefit is applicable only to the Insured Person who is employed/self-employed at the time of the Insured Event. In the event that the Insured Person sustains Injury resulting directly in Total Temporary Disablement, and upon due proof of the Insured Person s Total Temporary Disablement, AXA shall pay to Citibank the Minimum Payment Due for every consecutive month that the Insured Person suffers from Total Temporary Disablement up to a maximum of six (6) months. Special conditions applicable to Section 3: 1. The Insured Person must be gainfully employed or engaged in any business with an active Central Provident Fund (CPF) account at the time of the Insured Event, that is, the Insured Person has made CPF contributions in the 3 months prior to the Insured Event. 2. No benefit shall be payable for the first thirty (30) days of any period of the Insured Event. 3. The Insured Person must be suffering from Total Temporary Disablement corresponding to the period the monthly benefits are paid. If it is discovered that the Insured Person has resumed work during the period the monthly benefits are paid, AXA will recover any monies paid to Citibank in respect of such period the Insured Person had resumed work. 4. The Insured Person can only claim this benefit once in each calendar year. C. GENERAL CONDITIONS 1. Misrepresentation AXA will not be liable to pay any benefits under this Policy in respect of an Insured Person if such Insured Person: (a) Fails to observe and fulfil the terms and conditions set out herein. (b) Makes any claim that is fraudulent or exaggerated, or making any false statement in support of a claim. 2. Limitation on Payment of Benefits (a) The maximum payable per Account Holder shall be the Indebtedness up to a sum of S$160,000. (b) All benefits will be paid to Citibank to repay the Account Holder s Indebtedness and/or Minimum Payment Due as the case may be. Page 4 of 6

Cancellation of Cover The insurance cover for Account Holders provided under this policy shall terminate upon the happening of the first of any of the following: Cancellation of the Account Holder s Citibank Account. The Account Holder having attained the age of 65. Upon payment of a claim under Section 1 or Arbitration and Governance Any dispute in connection with this policy must be referred to the Financial Industry Disputes Resolution Centre Ltd (FIDREC). If mediation under FIDREC fails, the dispute has to be referred to arbitration in Singapore in accordance with the Arbitration Rules of the Singapore International Arbitration Centre ( SIAC Rules ) for the time being in force. 5. Sanction Clause Under no circumstances shall this Policy be deemed to provide cover and no liability be incurred to pay any claim or provide any benefit hereunder to the extent that the provision of such cover, payment of such claim or provision of such benefit would expose AXA to any sanction, prohibition, or restriction under United Nations resolutions or the trade or economic sanctions, laws or regulations of the European Union, United Kingdom, United States of America or Singapore. 6. Illegality Clause Under no circumstances shall this Policy be deemed to provide cover and no liability be incurred to pay or provide any benefit hereunder to the extent that the provision of such cover, payment of such claim or provision of such benefit would cause AXA to be in breach of, or expose AXA to any prohibition, or restriction under the laws or regulations of Singapore. D. CLAIMS PROCEDURES Upon happening of an event giving rise to a claim hereunder, the Account Holder or the Account Holder s legal representative shall follow the following procedures: 1. Give immediate written notice to AXA but no later than forty five (45) days from the date of the Insured Event. 2. Failure to furnish notice within the time provided shall not invalidate any claim if it is shown not to have been reasonably possible to furnish such notice and that such notice is furnished as soon as was reasonably possible. 3. The Account Holder or the Account Holder s legal representative shall complete the claim form as provided by AXA (which is made available at AXA website) and produce at no cost to AXA, the necessary proofs to substantiate the claim. 6 4. AXA will be entitled to appoint doctors to examine the Account Holder as may reasonably be required for the purpose of verifying any claim made. E. GENERAL EXCLUSIONS No benefits will be payable hereunder where an Insured Event occurs as a result of: 1. Any unlawful act or wilful exposure to danger. 2. Intentional self-injury, suicide or attempted suicide, while sane or insane. 3. Any Pre-Existing Condition. 4. Death or Injury resulting from childbirth or pregnancy related issues. 5. Under the influence or effects of alcohol or drugs unless properly prescribed by a licensed medical doctor and taken as prescribed. 6. Any Accident occurring on or in any aircraft other than an aircraft in which the Account Holder was travelling as a fare-paying passenger in a licensed passenger-carrying aircraft. 7. War (whether war be declared or not), rebellion, insurrection, resurrection, popular uprising, usurped power and terrorism. 8. Having previously received Total Permanent Disablement benefits for a claim made under the same Account. 9. Any Injury arising from the following occupations or whilst performing the following occupational activities:- Full time military, airforce, navy, police and civil defence personnel (other than serving full-time Singapore National Service or reservist training). Underground worker in tunnel or quarry, construction worker Any occupation dealing with explosives or hazardous substances 10. Nuclear radiation, nuclear fission, nuclear fusion and/or radioactive contamination. Policy Owners Protection Scheme This Policy is protected under the Policy Owners Protection Scheme which is administered by the Singapore Deposit Insurance Corporation (SDIC). Coverage for your Policy is automatic and no further action is required from you.
