The Travel Accident Insurance program is a complimentary feature of select Citi Cards where if you buy travel tickets using the card, you will automatically be covered by insurance. The travel ticket purchased can be either by air, land, or sea travel on board a common carrier. What are the Citi Card Types included in the Travel Accident Insurance and its coverage? What is the coverage of the Travel Accident Insurance? Cards that have Travel Accident Insurance are: Citi Platinum Visa PhP10 Million Citi PremierMiles Gold Card PhP5 Million Citi PremierMiles Platinum Card PhP10 Million Rustan s Citi Platinum Card PhP5 Million Cebu Pacific Citi Card PhP1 Million Travel Accident Insurance offers coverage on the following: a. Accidental Death In the unfortunate event of accidental loss of life within 180 days from the date of travel accident, you will be paid 100% of your coverage. b. Disablement In case of disablement, the benefit amount paid shall depend on the loss sustained. If more than one loss is sustained in the accident, AIG shall pay the largest amount based on the losses. Disappearance In case the body of the insured cardholder has not been found within one year from the date of disappearance, forced landing, stranding, wrecking or sinking of vehicle, ship or plane, it will be assumed that the cardholder has suffered loss of life and will be paid 100% of his/her coverage. Will there be a premium charge to my Citi Card account for this program? How long am I covered in this plan? No, this is a complimentary feature of the Citi Platinum, PremierMiles, Rustan s Platinum, Cebu Pacific or Corporate Card. You are covered by Travel Accident Insurance all year round. Please just ensure that your travel ticket is purchased using your Citi Platinum, PremierMiles, Rustan s Platinum, Cebu Pacific or Corporate Card and that your card is active and in good credit standing. In case of any change in terms i.e. decrease in coverage, Cards later on decide not to include the card type in the covered cards, shouldn t we be informing the client of this? We have advised the coverage is year round but we haven t indicated that in case of any change, or if Cards decided to cancel the complimentary coverage all throughout, the coverage will be terminated. Can I extend this coverage to my family or friends if I purchase their common carrier ticket/s using my Citi Card? Is the plan coverage only applicable to international travel? Will I receive a policy or proof of coverage each time my trip is booked using my Citi Card? This feature is offered to the Principal or Supplementary Cardholder who purchases the travel ticket using his/her Citi Platinum, PremierMiles, Rustan s Platinum, Cebu Pacific or Corporate Card. The Travel Accident Insurance is a group insurance policy where Citi is the policyholder for and behalf of its eligible cardholders.

5 Who is the Insurer of the Free Travel Accident Insurance? What is the claim procedure? AIG Philippines Insurance Inc. (formerly Chartis Philippines Insurance Inc.) is the insurer. The insured cardholder or his/her relatives, can file a claim by completing the Travel Insurance Claim Form. Once accomplished, please submit the claim form along with the required documents indicated in the form per type of claim to AIG Philippines Insurance. Fax number and address of AIG is also indicated in the claim form. For Loss of Life, kindly prepare the attending physician s report, Police investigation report or statement of witness(es), Birth certificate, Death certificate, Autopsy report and Marriage contract. For accidental disablement or dismemberment, please prepare the attending physician s report, police investigation report or statement of witness(es), Medical reports (e.g. history and diagnosis) and EENT specialist s report. Any Loss or Damage resulting from willful, malicious, or unlawful act or negligence of the insured person b. Act of war, foreign enemy, civil war, invasion, revolution, use of government or military power c. Disease of any kind

Any physical injury, sickness or disease as a direct or indirect result of use of nuclear materials, dispersal, release, or application of pathogenic or poisonous biological or chemical materials, or bacterial infections except pyogenic infection which shall occur through an accidental cut or wound. For the purpose of this exclusion, serious physical injury is defined as: - Physical injury that involves a substantial risk of death - Protracted and obvious physical disfigurement - Protracted loss of or impairment of the function of bodily member organ e. Loss, destruction, or damage to any property or any loss or expense directly or indirectly caused or attributed to ionizing radiation, contamination by radioactivity from any nuclear fuel, or from any nuclear waste. f. Any illegal or unlawful act by the Insured Person or confiscation, detention, destruction by customs or other authorities g. Any prohibition or regulation by any government h. Any breach of government regulation or any failure by the Insured Person to take precautionary measure to avoid a claim under the policy following the warning of any strike, civil commotion, or riot through general mass media i. The insured person not taking all reasonable effort to safeguard his/her property or to avoid injury or minimize any claim j. Hunting, riding, or driving in any kind of race, participating in any professional sport or in any sport where the insured person would or could earn or receive remuneration, donation, sponsorship, award, or certificate of any kind, and air travel other than as fare paying passenger in any licensed private or commercial aircraft or other mode of conveyance or transportation. k. Any pregnancy related, childbirth, and injury or sickness associated with pregnancy or childbirth or venereal disease l. Suicide, attempt to suicide, intentional self injury, provoked assault, or drug intoxication
