As members of Constructionline we are also committed to providing great value service to our customers. When carrying out Portable Appliance Testing on our Client s premises, JR PAT TESTING LTD and its personnel shall at all times apply due diligence and fully comply with Health & Safety Regulations as detailed and updated by the Health & Safety At Work Council UK throughout the duration of each assignment. We shall always seek to identify: WHAT ARE THE HAZARDS? WHO MIGHT BE HARMED AND HOW? WHAT ARE MY ARE MY REQUIRED ACTIONS? WHAT ARE MY RECOMMENDATIONS? DOCUMENT ACTIONS BY WHOM, WHEN BY AND COMPLETED BY. EXAMPLE: At the time of testing, all the equipment listed in the Test Report that obtained a PASS result, did so by meeting the standards set out in the IEE Code of Practice for the Inspection & Testing of Electrical Equipment. 2 PAT TEST Site Procedure: A Senior PAT test engineer will make himself known to the client s representative upon arrival on site, and will explain the schedule and purpose of work to be carried out. This person will then be asked to accompany our engineer around the site to highlight out any sensitive areas and confirm any arrangements/appropriate times that have been made for shutting down any equipment. This person will be advised of any concerns and items that fail. Standards for Testing Inspection & testing will be carried out in accordance with IEE s Code of Practice for In-service Inspection & Testing of Electrical Equipment. This includes the following types of electrical equipment as detailed in the IEE s Code of Practice: portable appliances; moveable or transportable equipment; hand held appliances; stationary equipment or appliances; fixed equipment or appliances; appliances or equipment for building in; IT equipment and extension leads; and multiway adaptors. Equipment Identification For each appliance, a decision shall be made as to which tests are appropriate depending on the type of equipment, e.g., Class 1, Class 2, etc. Inspection A visual inspection of the appliance will be carried out prior to any testing. The inspection includes the following: Plug this is examined to ensure it is in physically good condition, is free from cracks, or damage and signs of overheating, and that the pins are insulated. The plug top may be removed to establish the outer sheath of the flexible cable is securely gripped by the cable clamp, and the terminals sufficiently tightened, and that it is correctly wired. The fuse may then be examined to ensure it is of the correct rating for the type of appliance. Flex this is inspected ensuring its length is suitable and safe for the equipment and the way it is used; it is in good condition and free from splits, fraying or damage. Appliance the appliance, its casing or cabinet is inspected to ensure it is free from damage, cracks, etc., that could allow access to live parts; that it switches on & off properly; is in good working order and operates safely. Any minor faults that are identified during the visual inspection, for example: damaged 13amp plugs, incorrectly rated fuses, loose connections, will be rectified prior to the testing procedure.
3 Testing Subject to a satisfactory visual inspection, the following tests will be carried out were relevant: Earth Continuity Test applies to Class 1 appliances only Tests on computer equipment are soft tests of 200mA on earth continuity Insulation Resistance Test Operational Test - No operational tests are carried out on computer CPUs Earth Leakage Test - Only carried out if operational test applies Polarity Test only applicable to power leads and extension cables Labelling Each piece of equipment will be labelled with a unique id number; this will be used to identify it against the test results report. For appliances that pass the Inspection and Test, a Passed Electrical Safety Test label will be attached stating the date tested and re-test due date, along with the initials of the engineer conducting the testing. Failed appliances will be labelled with a Failed Do Not Use label, then removed from service and brought to the attention of the Client representative on site. Documentation On completion of testing, a report will be issued listing all appliances by: Identification number, description, location at the time of testing, any repairs carried out and the result of all tests performed. Appliances that fail testing will be listed separately and highlighted in red. This report will be produced on paper format in a bounded file. In addition to the report, a certificate shall be provided for display on the premises to advise that all portable electrical appliances have been tested for electrical safety. This is then both ed and hard copy posted to the client with the appropriate invoice. Communication Throughout the PAT test procedure all staff retain direct lines of communication with Director John Ross to enable resolution to any specific site related issues or matters relating to personal health and wellbeing.

7 JR PAT Testing Portable Appliance Testing Risk Assessment Date of Document August 2017 Date of next review: August 2018 Injury from use of Hand Tools and Testing Equipment Low Testing Equipment: All equipment inspected before each job and regularly serviced. Test Engineers instructed in correct methods of use. Correct use of tools such as screwdrivers ensure risk kept to a minimum. Slips, trips and falls Low Slips: Safety footwear to be worn by Test Engineers and no liquids used in test procedure. Trips: Test engineer to inspect test area for uneven surfaces and to ensure obstacles removed. No trailing leads to be left in areas where tripping can occur. Falls: Majority of test work carried out at ground level. Engineers instructed not to work at heights over 2m unless safety equipment in place and training given. Noise Low Hearing protection equipment provide for work undertaken where a noise hazard is identified Lone Working Low Test engineers provided with mobile communications and in contact with Supervisor throughout the day. Harmful Substances Low COSHH assessments carried out as required and controls implemented. On site COSSH advisory information sought if required on a site-specifc basis. Manual Handling Low No heavy equipment used and items tested are not expected to be heavy or awkward.
8 Fire Low Test work does not involve any significant fire hazards. Test engineer to request details of site fire action plan prior to commencing work. Personal Protective Equipment PPE equipment available to Test Engineers dependent on environment in which testing procedure takes place: Workwear Hard Hat Safety Glasses Fleece outer wear Gloves Safety boots/shoes Face masks High Viz tabard/vest All personnel either employed by or sub contracted to JR PAT Testing will be made aware of the details in this document and of any and all amendments that occur.
