2 Summary of FY2015 Part A Annual Utilization The Ryan White Part A Annual Utilization Report provides an overview of aggregate demographic and service utilization data for all people living with HIV (PLWH) that received a Boston Public Health Commission (BPHC) Part A funded service during Fiscal Year 2015 (March 1, 2015 February 29, 2016). This report includes breakdowns for all PLWH who received Part A funded services as well as additional breakdowns by specific service categories. Part A funds were used to serve 5,523 PLWH in the EMA in FY2015, including 1,420 clients who did not receive services during FY2014. Fiscal Year Number of clients served , , ,891 Total records in e2boston 9,128 This year s report marks the second year that data was generated via e2boston, an online based data reporting system. All funded agencies are required to use this system to report data to BPHC. In addition to giving agencies real-time access to their own data and the ability to update client records electronically, it has greatly enhanced BPHC s capacity to conduct comprehensive reviews of data accuracy and completeness. To date, a total of 9,128 client records exist in the system. Some new additions to the report include the following: Client Health Insurance information for each service category has been added, as this data was not available last year. In the Overall Client Demographic Summary, two new columns indicate the number of clients and corresponding percentage from the previous year s data for each demographic variable. The report breaks out Rental/Utility Assistance and Housing Search and Advocacy. In last year s report, data for these two types of housing services were combined. However, due to different program models, breaking out by type of program will provide a better delineation of demographic and volume of clients accessing rental/utilities assistance versus those receiving housing search and advocacy. Part A client demographics in are very similar to those in FY14. The one area of difference is in the distribution of income among Part A clients. In FY14, 63% of clients were making less than 100% of the Federal Poverty Level (FPL), compared to 53% in. As with all systems, however, some additional adjustments are currently being made to improve how data are generated for the Annual Utilization Report. The FY 2015 report has been able to replicate all previous demographic and service utilization data fields found in last year s report, but supplemental reports and updates will be made after the initial publication of the Annual Utilization Report. This is the first year that Outcomes information is available in e2boston, and client outcomes data will be made available in a subsequent summary report. In addition to 2

3 summary reports, BPHC staff are working on comparative analyses of client level data that will also be made available later this summer. Several important caveats that may impact interpretation of current results include the following: Currently, Risk Exposure add up to more than 100% because e2boston currently allows PLWH to select more than one exposure so it is possible for a client to contribute more than one answer regarding exposure categories. For this reason, only the total number of unique clients is identified for Risk Category tables. This is also the only table that features a percent total greater than 100%. Information for Part A MAI-funded Medical Case Management and Psychosocial Support Peer Support services were previously reported as separate categories. Although these two service categories are tracked as a separating funding stream within e2boston, BPHC is currently reviewing the proper breakout of Part A non-mai versus Part A MAI funded programs. Updates with this MAI-specific data will be released as a supplemental report later this year. The FY14 comparison column More than One Race table in the All Client Demographics sheet does not add up to 100% because we introduced new combination race categories that were not included in last year s report. Categories were only included for comparison if they were exactly the same. This section is a special data run and was only conducted for the total client summary. The most recent amounts of Total Billing were used in the Service Utilization Summary sections, but these amounts are yet to be finalized and as such may change. BPHC will update the report as necessary. 3

4 Service Category Definitions (in alphabetical order) AIDS Drug Assistance Program (ADAP/HDAP) (Drug Reimbursement) The service funded under this category is the provision of medically prescribed pharmaceuticals used in the prevention, management, and treatment of HIV disease. Services funded under this category are only available for HIV + individuals who have met the eligibility requirements for participation in the Plan and have been enrolled according to standard Plan procedure. Food Bank/Home-Delivered Meals This category includes the provision of actual food or meals. Services funded under this category are the provision of calorically and nutritionally appropriate food, which may include, but are not limited to: prepared meals; congregate meals; home-delivered food; food banks; nutritional supplements; and the provision of nutrition counseling under the supervision of a registered dietician. Housing Search and Advocacy These services include assessment, search, placement, and advocacy services provided by those who possess an extensive knowledge of local, State, and Federal housing programs and how they can be accessed. Housing Rental and Utilities Assistance Services funded under this category include the provision of short-term and/or emergency rental assistance, the provision of housing support in a group home or scattered site setting, and emergency housing-related expenses such as utilities. Medical Case Management Historically known as Case Management, services funded under this category provide clientcentered services that link clients with primary medical care and health-related support services in a manner that ensures timely, coordinated access to appropriate levels of care. Clientcentered services support a client s ability in maximizing his/her self-sufficiency and independence. Key activities include: information and referral; assessment of the client s needs and personal support systems; development of a comprehensive individualized service care plan; coordination of the services required to implement the plan; client monitoring to assess the efficacy of the plan; periodic reassessment; and adoption of the plan. It is expected that medical case management programs will establish and maintain effective linkages with primary care and other service providers and ensure that medical case management services are not being duplicated across providers. Services will also be provided within a harm reduction framework and will include basic risk assessment and risk reduction planning. MAI Medical Case Management 4

The purpose of MAI Medical Case Management is to provide services that are culturally and linguistically appropriate to African Americans, individuals of African descent, Latinos, Native Americans, Asian Americans, Native Hawaiians, Pacific Islanders, and individuals of Asian descent who are living with HIV/AIDS. Medical Nutrition Therapy This category includes the provision of actual food or meals. Eligibility for this service requires certification by a physician and reassessment by a registered dietitian. Services funded under this category are the provision of calorically and nutritionally appropriate prepared food, which may include, but are not limited to: prepared meals; congregate meals; home-delivered food; food banks; nutritional supplements; and the provision of nutrition counseling under the supervision of a registered dietician. Medical Transportation Services funded under this category include, but are not limited to taxi vouchers and public and private transport services that enable clients and their caregivers to access HIV primary medical care and health-related support services. Oral Health Care (Dental) Services funded under this category are recruitment of dentists and preventive diagnostic and therapeutic services rendered by dentists, dental hygienists and other dental practitioners licensed to provide health care in the State or jurisdiction. Psychosocial Support (Peer Support / Mental Health / Substance Abuse) Services funded under this category include Peer Support, Mental Health, and Substance Abuse. Peer Support services provide assistance to clients where the person(s) providing the service is a person infected with HIV and of the client s self-identified community. Such services include the provision of culturally competent psychosocial support and assistance in obtaining a range of services and entitlement that will meet the needs of the client and are provided by licensed, or non-licensed, paraprofessional individuals. Mental Health services are those 5

6 facilitated by mental health professionals, including psychiatrists, psychologists, and clinical social workers who are licensed by the appropriate state services to render such services. These services are non-traditional in that they are intended to serve clients in agencies and settings that are not certified to bill Medicaid or other insurers. Mental health services that are reimbursable by third parties are not eligible for funding under this service category. Substance Abuse services refer to counseling services aimed at addressing substance abuse problems (i.e. alcohol, and/or legal and illegal drugs) in an outpatient setting provided by qualified personnel. MAI Psychosocial Support Also known as Peer Support, services funded under this category provide assistance to clients where the person(s) providing the service is a person infected with HIV and of the client s selfidentified community. Such services include the provision of culturally competent psychosocial support and assistance in obtaining a range of services and entitlement that will meet the needs of the client and are provided by licensed, or non-licensed, paraprofessional individuals. The purpose of MAI Psychosocial Support is to provide services that are culturally and linguistically appropriate to African Americans, individuals of African descent, Latinos, Native Americans, Asian Americans, Native Hawaiians, Pacific Islanders, and individuals of Asian descent who are living with HIV/AIDS. Substance Abuse-Residential Services funded under this category may include: Treatment to address substance abuse problems (including alcohol and/or legal and illegal drugs) in a residential health service setting (short-term). 6
