Each year, you may choose the proram that works best for you. As an FEHB member, you pay nothin for tobacco-cessation prorams and prescription and over-the-counter tobacco-cessation drus approved by the FDA. Talk to your physician about your treatment options. The rewards proram runs from January 1, 2015, throuh December 31, 2015, and is open to all Kaiser Permanente FEHB subscribers and their enrolled spouses who are 18 and older. You can take the Total Health Assessment as often as you d like and as many healthy lifestyle prorams as you d like, but you can only earn rewards for them once durin the reward period. You must complete the activities durin the calendar year. Rewards will be issued 4 to 6 weeks after you complete your activity. 3

Coverae for certain outpatient follow-up care and outpatient continuin care outside of Kaiser Permanente service areas throuh the FEHB travel benefit. You do not suspend your FEHB coverae to enroll in Senior Advantae for Federal Members. There is no increase to your FEHB or Kaiser Permanente premium when you enroll in Kaiser Permanente Senior Advantae for Federal Members.
