8 CONTACT TEACHERS RETIREMENT SYSTEM (TRS) Upon making your decision to retire this year, these are the steps to follow: Contact the Teachers Retirement System at Inform them you will be retiring as of the last day of the school year (May 30) or if you have other contract obligations, use the appropriate date.
9 TRS PAPERWORK TRS will send you the following forms to complete: Retirement Application Pre-populated based on your conversation with TRS. Will list the pension annuity you elected, dependent status, and elected federal tax exemptions. Complete and return to TRS.
10 TRS PAPERWORK Continued Depository Agreement for TRS Benefit Payments This form is used to direct deposit your pension payment. If depositing into a joint account, the other account holder must sign the form. Your bank will need to complete Section 2. Attach a voided check or deposit slip. Complete and return to TRS.
11 TRS PAPERWORK Continued Teachers Retirement Insurance Program (TRIP) Form This form is used to make your medical insurance selection. Insurance will be effective September 1, If you are eligible to have the District pay your premiums, send this form directly to Human Resources. DO NOT MAIL TO TRS Review insurance information at (This is the only opportunity you will have to enroll in TRIP and have the District pay all or part of the premium.)
12 STEP TWO ELIGIBILITY FOR DISTRICT BENEFITS
13 ARE YOU ELIGIBILE FOR DISTRICT BENEFITS? Article 33-Section B of the contract states: A staff member may apply for early retirement providing he/she: 1. Is eligible under the Illinois Teachers Retirement Act, and 2. Is at least 55 years of age or will be 55 years of age within six (6) months of the end of the academic year in which he/she retires, and 3. Has been credited with 10 or more years of service in the Rockford School District. 3
14 STEP THREE ACTION REQUIRED
15 ACTIONS NEEDED TO RETIRE Submit your letter of retirement no later than April 1, 2017, to the Human Resources Generalist for your school or location. Example: I am going to retire as of May 30, 2017 (or the last day of your required contract). Patricia Rohde, Payroll Director, at patricia.rohde@rps205.com to inform her how you want your sick days distributed.

19 NEXT STEPS TOWARD RETIREMENT Once approved by Rockford Board of Education, Human Resources will send you the following: Certified Retiree Insurance Enrollment Form Authorization for Direct Debit for Insurance Premiums 5 Authorization for Disclosure 6 You must also complete the TRIP enrollment form. These forms must be returned to Human Resources by May 26. 4

21 TRS PLANS UNDER AGE 65 TRS Offers 2 Types of Plans: Managed Care HMO and OAP Medical Indemnity Managed Care Plans For Winnebago County: HMO IL or Blue Advantage HMO (BlueCross BlueShield) SwedishAmerican Health Alliance HMO Rockford Memorial Hospital HealthLink OAP OSF and Rockford Memorial Hospital (Tier 1) Coventry HMO or Coventry OAP (Tier 1) - SwedishAmerican and Rockford Memorial Hospitals 7
22 MANAGED CARE HMO HMO IL, Blue Advantage HMO, Health Alliance HMO, Coventry Healthcare HMO If you select one of the HMO s you must: Select a Primary Care Physician (PCP). Provide the physician s National Provider Identifier (NPI) on the TRIP form. You can obtain this from the website or the physician. Obtain referrals to be seen by other providers. Each HMO has its own Pharmacy Benefit Manager. Check the plan website to see if your doctor is in the network you choose.
23 MANAGED CARE Open Access Plan HealthLink OAP, Coventry Healthcare OAP If you select one of the OAPs: Will have three (3) tiers of providers. Benefit level is determined by the tier in which the provider is contracted. Offers flexibility in selecting healthcare providers. No deductible for Tier I, deductibles apply for Tiers 2 & 3. Each OAP has its own Pharmacy Benefit Manager. Check the plan website to see if your doctor is in the network you choose.
24 MEDICAL INDEMNITY (TRADITIONAL 80/20 PLAN) TEACHERS CHOICE HEALTH PLAN - CIGNA The current provider is CIGNA. Have a nationwide network of physicians, hospitals and ancillary providers. CVS Caremark is the Pharmacy Benefit Manager. OSF, SwedishAmerican and Rockford Memorial Hospitals are considered in-network. Will receive the Retiree Supplemental Health Plan from the District administrated by Allied Benefits. 8
25 TRIP PREMIUMS District will pay 100% of Managed Care premiums (HMO & OAP Plans). District will pay 50% of TCHP premiums (CIGNA). Payment of these premiums will last until the end of the school year in which the retiree reaches age 65. Example: Birthday Term
26 TRIP PREMIUMS DEPENDENTS You may cover your spouse and/or dependent children up to age 26. Dependent premiums will be deducted from the retiree s pension check. Benefits or payment of premiums for dependents are not available through the District.
27 TRIP INSURANCE PREMIUMS - EXAMPLES Current Managed Care premium is $ per month. Retiree cost is $0.00. Current TCHP premium is $ per month. Retiree cost is $ %. If you relocate out-of-state and Managed Care is not available (TRS will verify), retiree can enroll in TCHP and District will cover 100% of premium. Current premium - $ Retiree cost is $0.00.
28 TRIP INSURANCE - MAKE A CHOICE Complete the TRIP form. List dependents you will be covering. Attach marriage/civil-union certificates and birth certificates as required. Select TRS insurance plan. If electing a HMO plan, list your primary care physician and their National Provider Number (NPI). Sign, date, and return the form to Human Resources by May 26.
29 SAMPLE TRIP ENROLLMENT FORM
30 INSURANCE PREMIUMS DENTAL One time opportunity to continue in the District s dental plan upon your retirement: Monthly rate is $34.48 Retiree only If you decline coverage at this time or drop coverage later, you will not be able to reenroll.
31 PAYING YOUR INSURANCE Premiums are: PREMIUMS Paid only through direct debit from checking account. Withdrawn on or about the 5 th of each month (please notify the District immediately of any change to your banking information).
32 PREMIUM INSURANCE CHANGES Premiums will change due to: TRS updating premiums each July 1. You move and must enroll in a different plan. You enroll in a different plan during the TRS Benefit Choice Open Enrollment Period. District updates dental premiums.
33 OTHER INFORMATION Enrolling in the insurance with TRS and having the District pay the premiums is a one-time option. If you enroll in insurance under a spouse or another employer and subsequently lose that insurance, you may be eligible to enroll in TRS TRIP insurance, but the District will not pay your premiums even if you are still under age 65.
34 TRIP PLANS - OVER AGE 65 MEDICARE ADVANTAGE PLANS - TRAIL No benefits are available from the District for retirees over age 65. TRIP offers Medicare Advantage Plans through TRAIL (Total Retiree Advantage Illinois): Must be enrolled on Medicare Parts A & B. UnitedHealthcare Medicare Advantage PPO (nationwide plan). Coventry Advantra HMO Humana Benefit Plan HMO Humana Health Plan HMO Health Alliance MAPD HMO For more information regarding Medicare Advantage Plans please contact the State of IL, Bureau of Benefits, at or see:
35 CHANGES Notify TRS at and RPS205 of any changes: Address change Plan change Change in dependent information Cancellation of coverage Please use the RPS Insurance Change Form or the Retiree Address Change Form. Both are located on the District s website.
36 OTHER BENEFIT INFORMATION Flexible Spending Accounts will end as of All claims must be incurred by to be eligible for reimbursement. Don t sign up during open enrollment because you won t be an active employee when the new plan year starts. Life Insurance Group Life Insurance will last through the end of the school year in which you turn age 65. The current life insurance amount is $20,000 for REA and 2X the ending salary for Admin.
37 COBRA As required by Federal law, you will receive an offer of COBRA from our administrator Health Care Service Corporation. If you are eligible for retirement benefits from the District, do not enroll yourself in COBRA. Your dependents may enroll to continue the District medical and/or dental insurance.
38 403(b) RETIREMENT PLAN 403(b) contributions will cease with your final payroll check, August 11 (last check 2017). Information on distribution of your funds can be obtained by contacting TSA Consulting. No District employee or department is authorized to sign off on any distribution request. You may obtain the Transaction Routing Request form on the District s website or TSA Consulting Group s website (also included in your packet). Please contact TSA at for any questions on completing this form.
