Students joining FST Sixth Form from other institutions will be required to provide evidence of free school meal entitlement. Priority 3 Group (Assessment of household income being less than 24000): Please enclose proof, if parents or guardians are in receipt of one of the following: Income Support/Universal Credits Job Seekers Allowance (JSA) Housing or Council Tax Benefit Working Tax Credit Pension Guarantee Credit NHS Tax Credit Exemption Card Employment and Support Allowance Support under part VI of the immigration and Asylum Act 1999 If you are not in receipt of the above, please enclose proof of household income being beneath the threshold to qualify for priority 3 funding, including the following: Earnings Partner s Earnings Benefits Unearned Income Interest from savings and grants/scholarships from other sources Proof should be in the form of a copy of your benefits book, NHS Tax Credit Exemption Card, bank statements, a letter from the Department of Work and Pensions, Pay Slips, P60 s and, if you are self-employed, authorised evidence of income. Priority 3 applications will be assessed on a case by case basis. DATA PROTECTION DECLARATION I give my consent to Cornwallis Academy to record and process the information recorded on this form. 4 I declare that the information I have given is correct. Signature & Full Name FUTURE SCHOOLS TRUST SIXTH FORM Bursary Fund Guidance What is the Bursary Fund? The fund is made available from the government through its funding body - the Education Funding Agency (EFA) for year olds - to provide assistance to students whose access to, or completion of, education is inhibited by financial constraints or barriers. The funding is intended to help students to meet the cost of travel, books and equipment or other expenses. Who is eligible to apply for Bursary Funding? Students following government funded full time or part-time courses, who are aged at least 16 and under 19 years of age on 31 st August 2018, who satisfy one or more of the following criteria:- Students who are in care, are leaving care, are in direct receipt of Income Support/Universal Credits or are disabled and in receipt of both Employment Support Allowance and Disability Living Allowance/Personal Independence Payments Students who are economically or socially disadvantaged and at risk of not taking up or continuing in their education. This group includes students with physical or other disabilities, medical conditions and/or learning difficulties. Students who have been in care, on probation, are young parents or are otherwise considered at risk. Asylum seekers Refugees (who will have access to Bursary Funding if they are in receipt of supportive evidence from DWP). Lone parents Who is not eligible for Bursary Funding? Students under 16 years of age or over 19 years of age on 31/08/18. Students who do not meet the residency qualification.

5 How does the Academy assess applications and allocate Bursary funding? The Bursary Fund is a limited fund and the Academy will prioritise allocation. There will be 3 priority groups. Priority 1 Group Students aged between 16 and 18 on 31 st August 2018 and who fall into one of the following categories: Are currently in care (as defined by social services) Are leaving or have recently left care Are in receipt of Income Support/Universal Credits in their own name. Are disabled and in receipt of both Employment Support Allowance and Disability Living Allowance/Personal Independence Payments Eligible students will be guaranteed a cash bursary of 1,200 per year Payments will be made into the student s bank account on a termly basis (i.e. 6 times per year) Priority 2 Group Students eligible for free school meals may apply for a small cash bursary. In determining the amount awarded, the academy may take into account the number of applications received and the funding available. Payments will be made into the student s bank account on a termly basis (i.e. 6 times per year) Priority 3 Group A small amount of money will be retained by the academy to contribute towards the cost of books, equipment, travel trips, exam re-sits etc. This fund will be spent on students at the academy s discretion, and subject to availability after Priority 2 bursaries have been awarded. In some cases, awards may not be made until after 31 December Proof of household income may be required in certain circumstances. Timing of payments will be at the academy s discretion but it aims to match those for Priority 1 and 2 which is at the end of each academic term.

6 If my application is successful, will I continue to receive payments until the end of the academic year? Provided that you continue to meet the minimum attendance rate of 90%, payments will continue for the full six terms. If you leave the course part way through the year, payments will cease. Priority 3 payments will be made at the discretion of the Sixth Form management team and the finance department.
