This past yea, we conducted an extensive maket analysis of ou benefits. Ou analysis found that ou health plan continues to be vey competitive among employes in the maketplace. In addition, we continue to contol costs fo ou employees by maintaining ou pemium cost salay ties. Open enollment fo you 2018 benefits will take place Oct. 16 though Oct. 31. As you pepae to make impotant decisions about you benefits options fo 2018, hee ae a few highlights. We ae offeing a single plan, featuing low deductibles, fixed copays fo most docto visits, and out-of-pocket limits that potect you fom catastophic expenses. We have selected Delta Dental as ou new dental benefits povide. To help keep pesciption dug costs low fo you and you family, we have eviewed and evised ou list of pefeed band-name dugs that the pesciption plan pays fo. New goup voluntay benefits will be offeed by Aflac and Unum. We continue to suppot ways fo you to impove you health and possibly ean discounts on you health insuance pemiums such as: The Healthy at Hopkins online wellness potal which featues useful infomation and esouces fo you health and wellbeing and offes a quately incentive pogam. The employee Health & Wellness Cente at The Johns Hopkins Hospital which offes employees a convenient, fee option fo medical appointments duing wok hous and accommodates aftenoon and evening hous at the Employee Afte-Hous Clinic. 2 NEW ENROLLMENT SYSTEM EVERYONE MUST ENROLL FOR 2018! Open enollment is you chance each yea to select o make changes to you medical and dental coveage, fund a flexible spending account, and enoll in ou many voluntay benefits. Fo 2018 benefits, we ve launched a new enollment potal that will make choosing benefits easie than eve, but you must enoll. If you e keeping you benefits fom last yea, making changes, o enolling fo the fist time, you must sign on to the new potal and enoll to have benefits in 2018! 2018 BENEFITS UPDATES JHHS HEALTH PLAN ADMINISTERED BY EHP Fo 2018, we e offeing a single plan, featuing low deductibles, fixed copays fo most docto visits, and an out-of-pocket limit that potects you fom catastophic expenses. Thee ae thee options to eceive cae. Hopkins Pefeed: Sevices povided by Johns Hopkins povides and at ou facilities will be paid by the health plan at 90%. Employees will pay 10% of the cost afte all applicable deductibles and copays have been applied. EHP In-Netwok: Sevices povided by povides and locations in the EHP netwok will be coveed at 80%. Employees will pay 20% afte all applicable deductibles and copays have been applied. Out-of-Netwok: Sevices povided by povides and locations that ae outside of the Hopkins and EHP netwoks will be coveed at 70% of the easonable and customay ate afte all applicable deductibles and copays have been applied. We will continue to povide 100% coveage fo peventative cae sevices endeed by Hopkins Pefeed, and EHP in-netwok povides. This includes diagnostic sevices fo peventative exams, peventative mammogams and peventative colonoscopy. To help keep pesciption dug costs low fo you and you family, we have updated ou list of pefeed band-name dugs that the pesciption plan pays fo. As esult, some band name dugs may no longe be coveed unde the plan, but will have lowe cost options and that ae equally as effective. The new list will help ensue that pesciptions dugs emain affodable fo you and you family. You will pay the least when you choose geneic altenatives, and when you ode a 90-day supply of maintenance medications. A NEW DENTAL PROVIDER RICHER PLAN. WIDER NETWORK. Dental cae is an impotant pat of you health plan. In 2018 ou dental plan will be offeed though Delta Dental. Delta Dental will povide both compehensive and high option plans with the addition of coveage fo dental implants at majo estoative benefit levels and one additional cleaning pe yea fo pegnant women.

3 NEW VOLUNTARY BENEFITS We ae eplacing ou existing individual Aflac plans and offeing new goup plans though Aflac and Unum. This change should povide a cost savings in the pemium ates. AFLAC We ae offeing Citical illness and Hospital Indemnity. Employees cuently enolled in an individual plan may continue to be able to continue with thei existing coveage and payoll deductions. Howeve, the goup Aflac plans may be a bette altenative by poviding geate benefits at a lowe cost. If you have an individual Aflac plan, Aflac will be mailing infomation to you home to help you make a decision. DEPENDENT TUITION Effective Januay 1, 2018, employees must have fou yeas of continuous sevice befoe being eligible fo the dependent child tuition benefit. All employees with a hie date of Decembe 31, 2017 o ealie ae gandfatheed in with a two yea continuous sevice equiement. UNUM A new whole-life goup insuance plan and Unum s goup accident plan will be offeed though Unum. This will be a new plan in addition to the Basic and Supplemental life benefits cuently offeed. Infomation on enolling in these plans will be available on the Benefits enollment website.

4 2018 BI-WEEKLY MEDICAL & DENTAL PREMIUM RATES SALARY TIERS The biweekly pemium each staff membe pays fo medical cae fo themselves and coveed dependents is detemined by salay level. These salay levels ae gouped into fou ties, with those employees eaning least, paying lowe pemiums. Ou goal is to ensue that the medical plan emains affodable to all employees SALARY TIERS Tie 1 $0-$34,999 Tie 2 $35,000-$49,999 Tie 3 $50,000+ MEDICAL PLAN BI-WEEKLY PREMIUMS Johns Hopkins pays fo most of the cost fo you medical and dental coveage and all of the cost of you shot-tem disability and basic life insuance benefits. As we have peviously communicated, thee will be a small incease in plan costs in You bi-weekly contibutions fo medical pemiums fo 2018 ae listed hee and ae also available online in the Employee Benefit Guide. 5 TOBACCO-FREE CREDIT As pat of ou effots to pomote employee health, the Johns Hopkins Health System will again ewad employees who don t use tobacco poducts with a cedit of $11.03 each pay peiod. Only employees enolled in ou medical coveage ae eligible to eceive this cedit. To eceive the cedit fo 2018, employees must paticipate in voluntay tobacco sceening test. Onsite testing will be offeed duing open enollment at locations thoughout the health system. If you ae unable to attend onsite testing you will have the option to go to a LabCop facility fo testing. Testing only applies to employees; coveed dependents will not be tested. We will continue to offe esouces fo those who wish to quit. You may paticipate in a Healthy at Hopkins tobacco cessation class o coaching at no cost to you. Upon eceiving you completion cetificate you can then begin eceiving the tobacco-fee cedit. Woking closely with ou membes and thei health cae povides, case manages: Assess physical, psychosocial, spiitual, and financial needs Educate membes on ways to manage thei health Assist with efeals to specialty povides Coodinate cae with othe depatments and community agencies Povide ongoing communication to tack and eview pogess Plan membes with less complicated conditions such as less sevee asthma o diabetes, but who ae at isk of developing othe conditions o complications, can benefit fom ongoing monitoing and help to impove thei self-management skills. The Cae Management team encouages these membes to pogess towad goals and peiodically povides infomation about maintaining a healthy lifestyle. Case Management also povides Health coaching to adopt lifestyle changes that will help you lead a fulfilling and healthful life, we encouage you to sign up fo ou Health Coaching Pogam. The Health Coach Pogam gives you the oppotunity to wok one-on-one with a pesonal health coach and ceate an individualized action plan.
