Simply Blue A new, affordable plan that starts to pay right way Maybe you re at the point in your life when you are on your own and not covered by a health plan. A pre-existing condition is any injury, illness, or condition for which the covered person had medical treatment, symptoms, or any manifestations before the effective date of coverage. Benefits are based on the allowed amount. If you see a doctor who isn t in the Blue Cross network, you re responsible for any charges over the allowed amount. Any health condition that may have occurred during a previous Insta-Care contract will be treated as a pre-existing condition under subsequent Insta-Care contracts. 3

5 How to apply for one of our plans 1 Choose the plan that s right for you. 2 Complete and sign the application in the pocket at the back of this folder. 3 Follow the instructions in the Pay-O-Matic brochure in the back pocket of this folder if you decide on the automatic withdrawal method of monthly payment. 4 Send the original application, your payment and any Pay-O-Matic information to the address on the application. The application and payment must be received by Blue Cross within 15 days of the date of your signature. Minnesota law prohibits Blue Cross from accepting an employer s check for individual plans. Please pay by personal check or money order. 5 Be sure to allow time for Blue Cross to process your application. Once it s approved, Blue Cross will confirm the effective date of your coverage.
