4 Step 1: Learn The first big step on the path to choosing your coverage is to get organized so that you can effectively learn about the best options for you and your Medicare eligible dependents. If you choose to enroll, you will each need to enroll in an individual medical and/or prescription drug plan(s). You ll need to: Gather your personal information Make a list of expected expenses Check your budget Write down what you want in a plan Get smart about Medicare Tip: Check off the items on your to-do list as you complete each step Gather Your Personal Information List your current phone number, address and . You will need this information when selecting a plan. Gather your Medicare ID number and effective dates for Medicare Part A and Part B (these can be found on your Medicare ID card). Also, write down your preferred physicians and/or hospitals, phone numbers and addresses. Use the tables on pages 5 and 6 to enter this information. 4 (rev. 11/2017)

7 Make a List of Expected Expenses Write down the types of health care services including prescription drugs you think you ll need. Refer to your Explanation of Benefits statements, your provider billing statements, receipts and any other medical records you have kept. While you re making your list: Think of your current situation: What medical care have you had? What type of prescription drugs (name, dose, quantity) do you take? Will you continue to need the same medical care and prescription drugs? Think of your future: What are your health risks? Do you expect certain new medical conditions or care needs (e.g., surgery)? 9 Check Your Budget As you think about your budget for medical and prescription drug coverage, you will want to take into account: Medical plans with higher premiums can provide comprehensive coverage with fewer out-of-pocket expenses. These plans typically do not offer prescription coverage, so you may want to enroll in a Medicare Part D prescription drug plan as well. Medical plans with lower premiums allow you to pay for coverage as you use it in the form of copays/coinsurance. These plans often include prescription drug coverage. (rev. 11/2017)
10 Determine What s Important in a Plan Think about the features of your medical and prescription drug coverage that are important to you and your Medicare eligible dependents. Use the list below to rank the plan features from most important to least important. Add features that may not be listed and include them in your ranking. 11 Get Smart about Medicare Now is the time to learn how Medicare works, before you need to choose an individual plan. Understanding what is, and isn t covered, will help you and your Medicare eligible dependents decide which medical and/or prescription drug plan(s) best meet your needs. These resources can help you learn about Medicare: The Medicare Insurance Guide posted online at Local insurance company meetings Independent insurance brokers or carriers Friends, family members and local organizations Questions? 12 Step 2: Evaluate Once you ve learned the basics of Medicare and the types of plans available (see the Medicare Insurance Guide posted online at and thought about what s most important to you and your Medicare eligible dependents, it s time to investigate specific plans. Start Exploring Plans in Your Area Take Advantage of Aon Retiree Health Exchange 3M has partnered with Aon Retiree Health Exchange, experts in Medicare and health plans, to offer one-on-one consultation over the phone with you. Benefit advisors are available to answer your questions at no cost to you. You can also see and compare plans online at retiree.aon.com. Not all Medicare plans are offered by Aon Retiree Health Exchange and plans offered by Aon Retiree Health Exchange may not include auto reimbursement. Other Resources to Help You Get Started There are a number of resources to help you find the right plan for you and/or your dependent(s). You don t have to do all the research by yourself, here s a list to help you get started: Contact your local insurance agent or insurance broker Contact an insurance carrier directly Visit o Click Sign Up / Change Plans. Click Find health & drug plans. Enter your ZIP code and click Find Plans. You will be asked about your company coverage and prescription drug needs and your preferred pharmacy. Refer to the lists you made in this workbook to answer questions. o A summary of your search results will appear. You may refine your results based on what you are looking for in a plan. o Not sure what something is? Click the question mark near it for details. o Click the box next to the plans you wish to compare and then click Continue to plan results. Refine further, and then click Compare Plans. Refer to Determine What s Important in a Plan on page (rev. 11/2017)
13 Ask Questions: Don t be Shy As you consider your needs and begin to evaluate a variety of plans, continue to ask questions. Involve others in your discussions, such as a family member or friend. Attend local meetings in your community, and continue to learn about the differences in plans. Narrow Down Your Options Once you have an idea of the various plans that are available in your area, you might want to start to narrow your options. Certain plans may address your needs, and other plans may address the needs of your Medicare eligible dependents. Consider these questions for you and for your Medicare eligible dependents: Do these plans meet my budget? Do these plans address what s important in a plan? Do these plans cover expected expenses? If you re uncertain about any of the plans, make a list of the questions you need answered. 13 (rev. 11/2017)
14 Step 3: Choose You ve done the homework and researched your options. Now it s time to pick your plan(s). Pick Your Plan Decide which plan best meets your needs, with the features you value, within your budget. You ll receive a personalized appointment letter from Aon Retiree Health Exchange. The letter includes an appointment time for a one-on-one phone call with a benefit advisor who can walk through your plan choices, answer your questions and help you pick your plan(s). When you receive your letter, you must: Call to confirm, Call to reschedule, or If you re enrolling on your own, call to cancel your appointment Enroll Once you pick your plan(s), it s time to enroll for you and your Medicare eligible dependents. If you are working with Aon Retiree Exchange, you will complete the enrollment process with your Benefits Advisor either over the phone with a telephonic signature or by completing a paper application. If you are enrolling on your own with an independent broker or carrier, you will work with them directly to complete your enrollment. Attention Medicare Advantage and/or Medicare Prescription Drug Plan (Part D) Participants! Confirm Your Enrollment. If you and/or your dependent enroll in a Medicare Advantage and/or Part D Prescription Drug plan(s), you will each receive a verification call to confirm your enrollment, and Medicare requires that each of you must reply to confirm your coverage. If you don t confirm your enrollment, you will not have coverage.
15 Watch for Welcome Materials Once you are enrolled, you should receive your plan information and ID card from your plan(s). If you don t receive your ID card before your coverage becomes effective, contact your plan provider. What happens if you don t enroll? If you don t enroll in an individual plan: You will not have: Prescription Drug Coverage Supplemental Medical Coverage You will have: Medicare Parts A and B 3M Retiree HRA (if eligible) A gap in prescription coverage and/or supplemental Medical Coverage Keep in mind, you may incur penalties for late enrollment. 15 (rev. 11/2017)
16 Step 4: Use Start using your new plan. Take time to understand your new plan. Use Your New Insurance Cards Present your new cards to your doctors and pharmacists. Pay Your Premiums You will need to pay your insurance carrier directly for your coverage (not through deductions from your 3M pension). You have three options: Have your premium automatically withdrawn from a checking or savings account (recommended). Pay by check. Social Security deduction (available with certain plans). Request Reimbursement Once you have paid an eligible health care expense, you can be reimbursed from the 3M Retiree HRA, if eligible. You can request to be reimbursed in two ways submit a paper claim form or file your claim online. Depending on the individual medical and/or prescription drug plan(s) you choose, you may be eligible for auto reimbursement of your medical premiums. 1. Auto Reimbursement Through Aon Retiree Health Exchange If you enroll in a plan through Aon Retiree Health Exchange, you may be eligible for auto reimbursement of your medical and/or prescription drug premiums (many plans offered by Aon Retiree Health Exchange offer this feature). With auto reimbursement, you pay the premium, the insurance carrier reports the amount you paid to Aon Retiree Health Exchange, and then you automatically receive reimbursement from the 3M Retiree HRA without filing a claim form. If auto reimbursement is important to you, ask Aon Retiree Health Exchange if the plans you re considering include it. 16 (rev. 11/2017)
17 2. Standard Reimbursement If you don t enroll in a plan through Aon Retiree Health Exchange, or you pick a plan that doesn t allow auto reimbursement for your medical and/or prescription drug premiums, you will need to file a claim to be reimbursed. You can file a paper claim form or you can file a claim online. You pay the premium to the insurance carrier and then file a claim to be reimbursed for your expenses. In order to be reimbursed, you will need to submit a copy of a statement from your insurance carrier or other documentation showing the premium amounts paid and the start and end dates. You must also submit documentation showing proof of payment, for example, a cancelled check or a paid receipt. After your claim form has been received and processed, you will receive reimbursement from the 3M Retiree HRA via check or direct deposit. 18 Power of Attorney To speak to 3M FIRST Line Center or Aon Retiree Health Exchange If you have assigned Power of Attorney (POA) to someone who will be signing enrollment forms on your behalf, it s important he or she be on file at the 3M FIRST Line Center and that your POA s address be registered as the preferred address on file to ensure they receive all communications from the 3M FIRST Line Center and Aon Retiree Health Exchange. If you have not yet sent in your Power of Attorney document to 3M FIRST Line Center, please do so as soon as possible. The address is: 3M FIRST Line Center 4 Overlook Point PO Box 1459 Lincolnshire, IL Once the Power of Attorney is approved, the person you assigned as your POA will receive a user ID and password for your accounts. If you have already sent in the Power of Attorney form to 3M FIRST Line Center: If your POA is calling to make a change to the address information, they will need their password to access the account so changes can be made. If you or your approved POA do not have your passwords, a Customer Service Associate at the 3M FIRST Line Center will be able to help you obtain a new one. Call the 3M FIRST Line Center at (888) (toll free) or access the Your Benefits Rewards website at to confirm your current address. To Enroll in Coverage The procedure outlined above allows the person assigned as POA to speak to 3M FIRST Line Center or Aon Retiree Health Exchange on another s behalf. If the POA is actually signing an insurance application for someone else, a copy of the POA document will likely need to be attached to the application upon submission. Your Aon Retiree Health Exchange benefits advisor or your insurance agent/broker will assist you with that process and advise as to what documentation is required. 18 (rev. 11/2017)
