1 LiveWell Columbia Project Community Assessment Snapshot Jackie Belton is a successful patient in the diabetes prevention program at Palmetto Health and enjoys her healthy lifestyle of nutrition and exercise.
2 Let s create a healthier community. Palmetto Health is involved in the LiveWell Columbia Project to improve the health and the well-being of our community. Together, with our partners, we are developing new ways to address diabetes, obesity and related health issues. From July through October 2013, Palmetto Health administered assessments to the ZIP code neighborhoods and received 1,542 responses. These residents were from different backgrounds, family types, and age groups. Palmetto Health is working with community members to develop a creative and collaborative plan with unique elements. Together, we can. It is essential to learn as much about a community s readiness and capacity to change before committing valuable energy and resources. The information shared in this assessment will help set the direction for action and help Palmetto Health and our partners know where and how to start in the community. We will use this information to meet the following objectives: 1. Understand options and approaches for addressing community challenges. 2. Increase awareness and knowledge of local and national resources to help create healthier communities. 3. Introduce sustainability as an outcome to be addressed throughout our community change initiatives. Within the neighborhoods, we plan to meet these objectives through the development of a collaborative and creative plan.
3 Of those completing the assessment, the majority were female and African-American, with about 63 percent age 55 or older. The average household size was more than two people and, for those with children in the home, the average household size was four people, with two children per home. About onethird of respondents are married with 23 percent single, 26 percent divorced or separated and 19 percent widowed. Nearly 50 percent of respondents report income at $25,000 or less, and 18 percent are currently unemployed. Thirty percent of respondents work full-time and 44 percent are retired. There were 24 percent of respondents with a high school diploma or GED, and more than 40 percent of those surveyed have received an associate s degree or higher. LiveWell Columbia Survey Assessment 63% AGE 55 AND OLDER Type 2 diabetes, overweight and obesity, high blood pressure and alcohol use were seen as the most important health problems affecting this community. A healthy diet and regular exercise is important for reducing three of these four health conditions. However, of those responding, only half said that they are healthy eaters. Only a third of those responding indicated they have been physically active for an extended period of time. When asked why it is hard to exercise in their community, respondents said there aren t enough safe places to walk and sidewalks are needed. In addition to the traditional barriers to eating healthy, such as cost and too much fast food, respondents also expressed a need for more farmers markets and community gardens in the area. Increasing access to affordable health care, safe neighborhoods and a better economy with high job security also were seen as ways to make the community healthier. 32% MARRIED ADULTS 26% 23% 19% 1,542 DIVORCED OR SEPARATED SINGLE WIDOWED TOTAL RESPONSES The majority of responses were from African-Americans and females, with more than half over the age of 55. Although 39 percent rate the community as somewhat healthy, high blood pressure (63 percent), diabetes (49 percent) and overweight/obesity (45 percent) were rated as the three most prevalent health conditions in the community. 4

Residents ranked safe neighborhoods, access to affordable health care, and good jobs/healthy economy as the three most important characteristics of a healthy community. 5

This community has a greater percentage of African-Americans compared to Richland County and the state (United States Census Bureau and American Fact Finder, 2012). The area also has a higher percentage of females and unemployment among those of workforce age (Palmetto Health Triple Aim Project, 2012). Wealth is lower and the per capita household income in 2010 was below South Carolina s average of $25,065 (United States Census Bureau and American Fact Finder, 2012). The percentages of people living in and/or below poverty also are higher than the percentage living in poverty for Richland County, which was 14.5 percent in 2010 (U.S. Census Bureau, 2012). COPAD
