2 ASSOCIATION OF MINE MANAGERS OF SOUTH AFRICA Safety Recognition Scheme 1. Aim: To recognize participating mines and mine managers for achieving high standards of safety in the South African Hard Rock Mining Industry 2. Awards: 1. Million Fatality Free Shifts 2. Thousand Fatality Free Production Shifts 3. Safety Performance Improvement: LTIFR 4. Individual Awards The Million Fatality Free Shift Certificate is aimed at mines employing 1500 persons or more underground, whilst the Thousand Fatality Free Production Shifts Certificate is aimed at surface operations or mines not exceeding 1500 persons underground at any time. Depending on these criteria, the choice rests with each mine, with the proviso that a mine is eligible to participate in one or the other category, but not both. Examples: persons X 275 shifts p.a. = FFS per year, or 2.4 years to achieve FFS shifts p.a. X 2 shifts per day = 550 FFPS per year, or 1, 8 years to achieve 1000 FFPS. 3. Eligibility: In order to qualify for recognition any participating mine must comply with the following: Registered as a mine with the DMR if based in South Africa; Enrolled and accepted by AMMSA Council for mines outside South Africa; Manager shall be a member of AMMSA; Manager to inform AMMSA secretariat of qualifying achievement; Mine must register and submit monthly information as required. 4. Definitions: LTI = Lost Time Injury: Any injury that prevents a person from returning to his normal duty on the following calendar day. This will include any Serious Injuries.

3 LTIFR = Lost Time Injury Frequency Rate: Number of Lost Time Injuries per hours worked. Hours Worked: Includes all hours worked by all employees (including contractors) during the period (including overtime). Shift: Shall be one working shift per employee worked at a mine, including contractors, within the specified participating 24-hour period. Production shift: shall mean a full production shift during which the participating mines was operational. When more than one shift per day is worked, each separate production shift during the 24-hour period shall be recorded. Date of fatality: shall be the date on which the death occurs and not the date of the accident from which the fatality resulted. Mine: A mine consisting of 2 or more shafts can opt to enroll as 2 or more participating mines, e.g. each shaft or area under a 3(1) (a) appointee can participate on their own. The 4(1) appointee can also qualify for the whole mine with the proviso that his area of responsibility is not exactly the same as a single 3(1) (a) appointee. Fatality: shall mean the death of an employee (including a contractor s employee), which is the result of a mine accident that occurred not more than 12 months prior to the death. 5. Million Fatality Free Shifts Certificate: AMMSA presents an award known as the Million Fatality Free Shift Certificate to the industry for recognition of successful participants. A mine, or a portion thereof, which is in continuous operation and is enrolled with AMMSA will be eligible for this certificate. As soon as a mine records a million consecutive fatality-free shifts, it will qualify for recognition of the Million Fatality Free Shifts Certificate. A participant that has become eligible for presentation of the Million Fatality Free Shifts Certificate shall, by arrangement with AMMSA be presented with a certificate at the following AMMSA General meeting. The calculation of fatality free shifts after a fatal accident shall commence from the shift immediately following that during which the fatality occurs. 6. Thousand Fatality Free Production Shifts Certificate AMMSA presents an award known as the Thousand Fatality Free Production Shifts Certificate to the industry for recognition of successful participants. A mine, or a portion thereof, which is operational and is registered with the DMR, will be eligible for this certificate. As soon as a mine records one thousand consecutive fatality-free production shifts, it will qualify for the Thousand Fatality Free Production Shifts Certificate.

4 A participant that has become eligible for presentation of the Million Fatality Free Production Shifts Certificate shall, by arrangement with AMMSA be presented with a certificate at the following AMMSA General meeting. The calculation of fatality free production shifts after a fatal accident shall commence from the shift immediately following that during which the fatality occurs. 7. Hard Rock Mining Industry Safety Performance Improvement Award. AMMSA presents an award known as the Safety Performance Improvement Award to the Hard Rock Mining Industry for recognition of successful participants. A mine, or a portion thereof, which is in continuous operation and is registered with the DMR, will be eligible for this award. The basis of improvement will be the comparison of each mine s own safety performance in terms of LTIFR for a determined period (1 July to 30 June), expressed as a percentage improvement over the previous years performance e.g. LTIFR LTIFR % Improvement: ( ) x 100% = 30.2% In order to diligently interrogate the safety performance of individual mines for the purposes of identifying Safe Performers, it was agreed to implement a Recognition Scheme at the Annual Hard Rock Safety Conference. This Recognition Scheme will track the safety improvement of all participating mines during the year and indentify mines in various categories that have demonstrated a significant improvement in their own safety performance. Categories for the Hard Rock Mining Industry Recognition Award will be: Platinum Gold Base Metal and Diamond Contracting Companies 8. Individual Awards: Any individual, or group of individuals, such as a mine rescue team, mine safety committee, which has contributed to the overall health and safety program of a mine in a manner which has led to a significant improvement in conditions at that mine, may be nominated, by the manager of that mine or the employee representatives for an Individual Award.

5 This nomination will then be assessed by AMMSA Council to determine whether or not such a nomination is warranted. Any specific acts of bravery will also be considered for this award. When any award is recommended it must be made clear that it is in respect of a specific action which was of service to others and not an act which was in the interest of the person who performed it, e.g. saving his own life. Any such nomination must be adequately motivated to allow AMMSA Council to make an informed decision on any such recommendation. 9. Certificates and Awards Certificates for the Million Fatality Free Production Shifts and Thousand Fatality Free Production Shifts will be coordinated by AMMSA secretariat. Certificates will be handed over to the manager of the qualifying mine at the quarterly AMMSA General Meeting. Awards for the qualifying mines in the Hard Rock Safety Improvement Scheme will be presented to the manager at the annual Hard Rock Safety Conference. Prizes for the qualifying mines in the Individual awards will be presented to the manager and the individual at the annual Hard Rock Safety Conference. Certificates for 2 Million Fatality Free Shifts or more, and 2 Thousand Fatality Free Production Shifts or more, will be presented at the annual AMMSA Banquet. Should there be exceptional cases, or disputes, these should be raised with AMMSA Council, who will make a final ruling, based on information presented.
