GENERAL The regulations and rates contained herein are applicable to local exchange telephone services furnished within the District of Columbia (Washington Zone of the Washington Metropolitan Exchange Area [WMEA]). B. REGULATIONS 1. of Terms Household A household defines those who dwell in a single housing unit or address at which Local Exchange Service may be offered. 2. Touch-Tone Calling Service and Pay Telephone Lines are provided as specified in Sections 3 and 4D respectively, of this tariff. 3. Local Service Area The local service area of the Washington Zone includes all telephones bearing the designation of any central office of the WMEA and the Ashton (Md.), Braddock (Va.), Dulles (Continental Tel. Co. of Va.), Engleside (Va.), Gaithersburg (Md.), Herndon (Va.), Laurel (Md.) and Lorton (Continental Tel. Co. of Va.) exchanges. The WMEA embraces the District of Columbia and certain adjacent areas in Maryland and Virginia. The WMEA comprises zones designated as follows: Alexandria-Arlington (Va.), Berwyn (Md.), Bethesda (Md.), Bowie-Glenn Dale (Md.), Capitol Heights (Md.), Clinton (Md.), Fairfax-Vienna (Va.), Falls Church-McLean (Va.), Hyattsville (Md.), Kensington (Md.), Layhill (Md.), Marlboro (Md.), Oxon Hill (Md.), Rockville (Md.), Silver Spring (Md.) and Washington, D.C. 4. Residence First and Additional Lines Only one Residence First Line is available per household. All other residence lines in the same household, regardless of the number of subscribing customers or Local Exchange Services, will be Residence Additional Lines. 5. Lifeline Service As of February 9, 2016, Lifeline Service/Economy II Flat Rate Service and Link Up America will no longer be available for resale. Economy II Flat Rate Service is the Telephone Company's Lifeline Service designed to provide qualified customers access to local exchange telephone service. Such qualified customers are charged a reduced rate for local exchange telephone service. Customers who qualify for this Lifeline Economy II Flat Rate Service (hereafter "Economy II Service") and meet other requirements set forth below are entitled to a Locally Funded Service Upgrade, as specified in C.2. ( 6) following. 6. Mixing Classes of Service on the same Premises Residence customers may have different classes of service provided to the same customer or household on the same premises. RATES A Local Service Provider Freeze is available to residence and business end user customers as a means of protection from changes to their local service provider being made without their consent. This freeze prevents a change in the end user s local service provider unless such a change is requested by the end user. A request to activate or remove a local service freeze may be transmitted by an end user to the Telephone Company either orally or in writing. No charges apply for such requests. 1. Application of Rates The rates shown herein entitle the customer to local messages to all telephones of the local service area as specified in B. preceding. 2. Message units* are $.06 each. (3) This service is offered as a combined flat/message rate service which permits flat rate calling to telephones of the Washington Zone and message rate calling to the remainder of the local service area, specified in B.3. preceding, with each message unit* being $.060. * Not applicable for calls to Audiotex Service, except as specified following for local messages from PTL pay telephones. Additional message units are $.060 each. # Message units are $0.143 each (I) ** Effective June 16, 2011, Message B Service is grandfathered. ~ Effective June 16, 2011, Message B Service is withdrawn and no longer available to new customers. (4)* ~ This service will terminate following the Telephone Company's (C) implementation of a billing system which can maintain multiple billing balances in order to selectively disconnect services. This service will be available to any eligible customer for up to a 24 month period. This is a single line service available only to residence customers whose telephone service has been or is about to be disconnected for nonpayment. This service includes Long Distance Message Restriction and restricts Credit Card billing and collect calls to customers of this service, where feasible. Customers of this service may not subscribe to or utilize any other regulated services offered by the Telephone Company, except Touch-Tone Calling Service, Nonlisted or Nonpublished Service and Call Trace. Customers disconnected from Message "B" Service cannot return to this service. Message B Service is not available to Economy II customers. * Effective June 16, 2011, Message B Service is grandfathered. ~ Effective June 16, 2011, Message B Service is withdrawn and no longer available to new customers. Only one Economy II Service line is available per household. Any household subscribing to Economy II Service will not be allowed to have any other exchange service. (d) Households in which one or more individuals are receiving: Medicaid; or Supplemental Nutrition Assistance Program (SNAP); or Supplemental Security Income payments under Title XVI of the Social Security Act; or Federal Public Housing Assistance (Section 8); or Veterans Pension Benefit; or Veterans Survivors Pension Benefit; or Issued: November 30, 2016 Effective: December 1, 2016

7 S TARIFF 3rd Revised Page 6 Cancels 2nd Revised Page 6 (5) Economy II Service is offered subject to the following conditions: (Cont'd) (e) (f) (g) In addition to meeting the criteria in (d) preceding, customers with Economy II Service must be certified for participation by the District of Columbia Lifeline Program Third Party Administrator (DCLPTPA) Office. Customers must file an application for Economy II Service with the DCLPTPA. Customers with Economy II Service lines will be entitled to a reduction in the Interstate Customer Access Line Charge specified in The Verizon Telephone Companies Tariff F.C.C. No. 1, for the District of Columbia. Lifeline Toll Restriction is a toll blocking arrangement that permits customers with Economy II Service lines to complete local calls; calls to the operator; calls to E911 and other N11 Service Codes; calls to Toll Free Service telephone numbers; and local Directory Assistance Calls. Lifeline Toll Restriction will be offered at no charge, and will be added to a customer s account solely at the customer s discretion. The circumstances in which a deposit will be required for a customer prior to establishment of Economy II Service will be governed by the Consumer Bill of Rights, except that under no circumstances will a deposit be required for a Economy II customer, if the customer voluntarily elects to receive Lifeline Toll Restriction. If the Telephone Company collects and holds a customer deposit and the customer later voluntarily elects to receive Lifeline Toll Restriction, the deposit will be refunded. Lifeline Toll restriction prevents the origination of all interstate long distance calls; 700/900 calls; and calls to Audiotex Service Telephone numbers. This arrangement also blocks 2 the use of Connect ReQuest; and interstate long distance Directory Assistance calls. (h) (i) Economy II Service will not be interrupted or disconnected for failure to pay for long distance charges. The Telephone Company may require an advance payment of one month s local charges from Economy II customers to restore local service after disconnection for non-payment of local charges. 1 Verizon Busy Line Verification and Busy Line Interrupt Operator Services (BLV/I) are withdrawn as of April 7, Effective August 8, 2016, Collect and Bill-to-Third Numbers calls will be discontinued and will not be accepted as a billing arrangement for calls. 8 S TARIFF Original Page 7 (6) Locally Funded Service Upgrade (a) A Locally Funded Service Upgrade is available to Economy II customers who are 65 years of age or older. (b) The Locally Funded Service Upgrade reduces the Economy II rate to $1.00 per month. Issued: April 18, 2001 Effective: April 18, 2001
9 S TARIFF 6th Revised Page 8 Cancels 5th Revised Page 8 e Not (7) An optional 24-month term agreement is available at a monthly rate of $ , 2 per line, pursuant to the following conditions: (I) A. Termination Liability 1. Customers may choose one of the following Termination Liability Plans: PLAN 1 - Early Termination Liability for 24-month Term Agreements in effect prior to July 1, 2006 applies as follows: If a customer terminates services during the first 18 months of the 24 month commitment, the customer will pay early termination charges. Early termination charges will be calculated by taking the difference between the month-to-month rate and the plan rate and multiplying it by the number of months in service. The customer must contact the Company prior to the end of the plan period in order to renew the plan for additional periods of time. If the customer does not renew the plan before the end of the plan period, the rate will revert to the month-to-month rate. The customer will not pay termination liability charges under the following circumstances: a. If the customer terminates the service during the first 18 months of the 24 month commitment in order to subscribe to another Verizon service of greater value. b. If the customer terminates the service during the first 18 months of the 24 month commitment in order to subscribe to another Verizon service and commits to a term plan of a duration that is equal to or greater than the time remaining under the 24 month commitment. c. If the customer terminates service at the end of the 18th month of the 24-month commitment. d. In the event the customer terminates service within the first 30 days, the customer will be liable for the applicable monthly rate incurred by the customer and non-recurring charges; however, the Termination Liability will be waived. If the customer terminates service after 30 days but prior to completion of the term commitment period, the customer shall be liable for an early termination charge and non-recurring charges. 1 Effective September 21, 2013, the 24 month term rate of $14.59, as provided in this section, is no longer available to new customers. Existing 24 month term rate customers may continue their service at the rate of $14.59 until their 24 month term rate contract expires or their service is disconnected, whichever occurs first. The 24 month term rate of $16.59 will apply to new customers. (I) 2 Termination liability applies to lines purchased under a term plan, as described in Section C.2.7.A. Lines subscribed to under Termination Liability Plan 3 may not be combined with other voice services or features under a term commitment. Other voice services and features may only be combined with lines on Termination Liability Plan 3 when these other voice services and/or features are purchased on a month-to-month basis. (Cont d): PLAN 2 - Early Termination Liability for 24-month Term Agreements in effect for new or renewing customers beginning July 1, 2006 applies as follows: In the event the service is terminated by the customer prior to completion of the current term commitment period, the customer shall be liable for an early termination charge, except as noted in Section C.2.A.2. below. The amount of the early termination charge will be 25% of the monthly recurring charge(s) (MRC) for the remainder of the term. For example: 25% x MRC x # of Lines x Remainder of Term = Termination Charge. a. End of Term Options. Prior to the end of the term commitment period, the customer may select one of the following options, to be effective at the end of the term: Renew their term commitment, Arrange for termination of the service Arrange for change of service In the event the customer does not select one of the above options, the customer will be converted to month-to-month service. PLAN 3 - In the event the service is terminated by the customer prior to completion of the current term commitment period, the customer shall be liable for an early termination charge, except as noted in Section C.2.A.2. below. The amount of the early termination charge will be calculated at 35% of the monthly tariffed recurring charge(s) (excluding any promotional discounts) for the remainder of the term on a per line basis. Customers with Plan 3 will automatically renew at the end of each existing term to a new 24 Month term with 30 days before the end of a term to 60 days after the term automatically renews to opt out with no early termination charge. The expiration date for all terms will be coterminus based on the date of the first term established. Customer termination of service within first 30 days will result in customer liability for applicable monthly rate incurred by the customer and non-recurring charges; termination liability will be waived. Early termination charges will not be assessed under the following circumstances: (T) Customer moves existing service either to a new location within the same address and/or same building (inside move) or to a new location (outside move) and maintains that service for the remainder of the term; Customer attempts to move the existing service to a new location within Company's service area, but the service is unavailable; Customer renegotiates a new term commitment plan for the same service before the current term commitment expires and the value of the new term commitment is equal to or greater than the remaining value of the current term commitment; or, Customer changes to another service or upgrades service to a higher speed or capacity under a term commitment, provided the value of the new term commitment is equal to or greater than the remaining value of the current term commitment; The Company provides the new service via tariff or on a Customer Specific Pricing (CSP) agreement; and the order to discontinue the existing service and the order for the new or upgraded service are received by the Company at the same time. (8) (9) Effective July 15, 2012, PBX Trunk, additional and Economy I services are no longer available for purchase. Existing customers of record may retain these services at existing locations in accordance with the terms and conditions described herein. Issued: October 17, 2014 Effective: October 19, 2014
