3 COVERAGES MOBILE-HOMEOWNERS The Mobile-Homeowners policy provides property and liability coverages, using the forms as follows: PROPERTY COVERAGES SECTION NS-1 - Basic Form + NS-7800 Mobile-Homeowners Endorsement. Covers the mobile home, private structures, personal property and additional living costs against loss by: Fire or Lightning Windstorm or Hail Explosion Riot or Civil Commotion Theft Vehicles or Aircraft Sudden and Accidental Damage from Smoke Vandalism or Malicious Mischief Glass Breakage Sinkhole Collapse Volcanic Eruption NS-2 - Broad Form + NS-7800 Mobile-Homeowners Endorsement. Covers the mobile home, private structures, personal property and additional living costs against loss by: All the perils listed for NS-1, plus: Falling Objects Weight of ice, snow or sleet Collapse of a building or part of a building Sudden and accidental tearing apart, cracking, burning or bulging of a heat or air conditioning system or water heater Accidental discharge or overflow of liquids or steam Freezing of a plumbing, heating or air conditioning system or domestic appliance Sudden and accidental damage from artificially generated electrical current NS-3 - Special Form + NS-7800 Mobile-Homeowners Endorsement. Covers the mobile home, private structures and any resulting additional living costs against risks of direct physical loss unless the loss is excluded. Personal property and any resulting additional living costs are covered for the same perils as provided in NS-2. NS-4 - Contents Broad Form + NS-7800 Mobile-Homeowners Endorsement. Covers personal property, including the insured s interest in building alterations and additions, and additional living costs, against loss by the same perils as provided in NS-2. ALL FORMS: Loss Settlement a. Mobile Homes are covered according to Actual Cash Value terms unless Replacement Cost Coverage is purchased. b. Personal Property is covered according to Actual Cash Value terms unless Replacement Value - NS-55 is purchased. All Forms: LIABILITY COVERAGE SECTION 1. Personal Liability - Covers payment on behalf of the insured of all sums which he/she shall become legally obligated to pay as damages because of bodily injury or property damage caused by an occurrence arising out of his/her premises or personal activities. 2. Medical Payments - Covers medical expenses incurred by persons, other than the insured, who sustain bodily injury caused by an accident arising out of the insured s premises or personal activities. 4 STANDARD LIMITS Following are the standard limits under the Mobile-Homeowners Policy: Coverage NS-1, NS-2 and NS-3 NS-4 A. Residence (Mobile Home) $15,000 NS-1 and NS-2 $20,000 NS-3 B. Private Structures 10% of limit on Mobile Home C. Personal Property 50% of limit $8,000 on Mobile Home D. Additional Living Costs 20% of limit 20% of limit on Mobile Home on personal property L. Personal Liability $100,000 Each Occurrence for ALL FORMS * M. Medical Payments to Others $1,000 Each Person For ALL FORMS * * Optional limits are available. See Liability Coverage section. ELIGIBILITY A Mobile-Homeowners Policy NS-1, NS-2 and NS-3 may be issued to an owner-occupant of a mobile home which is used exclusively for private residential purposes and contains not more than 2 families and with not more than 2 boarders or roomers per family. A Mobile-Homeowners Policy NS-4 may be issued only to the Tenant (non-owner) of a mobile home, provided the residence premises occupied by the insured is used exclusively for residential purposes and is not occupied by more than 1 additional family or more than 2 boarders or roomers. NOTE: Tenant coverage is not available for students unless the student is older, married, employed or we have supporting coverage for the parents. MANDATORY COVERAGES It is mandatory that insurance be written for all coverages provided under both the Property Coverage and the Liability Coverage Sections of the Mobile-Homeowners Policy. The Liability Coverage Section of the policy requires coverage for the following exposures and the additional premium developed from the Premium Charts must be charged when such exposures exist: 1. ALL additional residence premises where the Named Insured or spouse maintain a residence other than business or farm property; 2. ALL domestic employees of the Named Insured or spouse not covered or not required to be covered by Workers Compensation Insurance (charge required for domestic employees in excess of 2); and 3. Incidental office, professional, private school or studio use by the insured on residential premises of the insured. UNDERWRITING REQUIREMENTS 1. A completed Mobile-Homeowners application must be submitted along with a recent photo and Social Security number. Credit Reports are one of the factors used to determine acceptability of a risk. 2. Mobile home must be insured for 100% of actual cash value (except 90% or more of Replacement Cost if optional Replacement Cost coverage is written). 3. Any Mobile home with more than 1 lienholder is not acceptable. 4. Must be written for the owner-occupant only (except for Tenants NS-4). 5. At least 1 of the following must apply to the mobile home. a. Has factory approved tie-downs that are anchored. b. Is a double-wide mobile home. c. Is on a basement. 6. Only adults and family units are acceptable. 7. Mobile home must be on a foundation or blocks and connected to utilities. Oklahoma New Business North Star Mutual Ins. Co. NSMHO - 2 Renewals
5 UNDERWRITING REQUIREMENTS (Continued) 8. Mobile homes over 25 years old are not acceptable for new business. 9. Risks with factory installed woodburning fireplaces are acceptable, however, we will not accept any other type of solid fuel equipment. 10. The number of mobile homes insured in 1 mobile home park is limited to 12. Contact Home Office for possible exceptions. 11. Unsupported seasonal mobile homes are ineligible. 12. Vacant mobile homes or isolated risks not accessible by emergency vehicles are not acceptable. 13. Park Model Trailers are not eligible for coverage on a Mobile-Homeowners policy. They are not occupied as a primary residence and are not anchored in the same way as a mobile home. With supporting coverage, Park Model trailers may be insured in the Inland Marine program. 14. Farm Liability is required when: a. there are more than 40 acres; b. the insured has farming operations; or c. there are horses or other livestock. 15. Following is a list of Mobile-Homeowners risks not to be bound and generally will not be written: a. Property which is in generally below average physical condition. b. Risks which have sustained 2 OR MORE losses during the past 3 year period. c. Risks cancelled or nonrenewed by other carriers. d. Risks where insureds have a history of financial instability within the last 5 years. e. Hobby farms or any risk currently and/or formerly occupied as FARM PROPERTY on which any current and/or former farm buildings still exist. f. Mobile homes with more than two related private structures or with a related private structure larger than 1,200 square feet. g. Risks with wild animals. h. Risks with Dobermans, Rottweilers, Presa Canarios (also known as Canary Island Cattledog), Pit Bulls, (including, but not limited to, American Pit Bull Terriers, American Staffordshire Terriers, or Staffordshire Bull Terriers), Chows or dogs with aggressive tendencies. i. Do not submit risks with trampolines or swimming pools, unless prior approval has been received from a North Star underwriter. POLICY TERM Policies are written on a 1 year basis. The policy will be automatically renewed each year with an updated declarations page. The policy will be subject to the premiums, rules and forms in effect at the time of renewal. Policyholders will initially receive copies of all policy forms. Upon renewal they will receive only an updated declarations and forms that are new or revised. 6 FIRE PROTECTION DEFINITIONS The following 3 fire protection classes will apply: 1. Protected: Mobile home is located within 1,000 feet of a fire hydrant and within the city limits of a Class 8 or better town. 2. Partially Protected: Mobile home is located within 5 road miles of a responding fire department. (Fire Department must be Class 8 or better.) 3. Unprotected: All others. (Submit basis only.) 7 CALCULATION OF PREMIUM Step 1 - Determine the Base Premium based on the amount of Coverage A or Coverage C, as applicable. The Base Premium must be adjusted to reflect revised limits of Coverage C, if applicable. Step 2 - Apply applicable Location of Risk Modifier from page NSMHO-4 to the rate determined in Step 1. Step 3 For an ACV policy, if applicable, apply appropriate Inflation Guard coverage factor to Coverages A, B, C, and D. Step 4 - If rating a seasonal or secondary mobile home issued in conjunction with the primary mobile home, give duplicate liability credit and add the liability charge for an Additional Residence Premises Occupied by Insured. Step 5 - Modify the amount determined in previous steps to reflect the selection of a deductible option. (The Base Deductible for NS-1, NS-2 and NS-3 is $1,000. The Base Deductible for NS-4 is $500.) Step 6 - Modify the amount determined in Step 5 for all percentage discounts or charges. Discounts or charges should be multiplied by the Base Premium in the following order: Protective Device Discount, New Home Discount, Mature Mobile-Homeowners Discount, Companion Policy Discount, Age Surcharge, Factory Installed Fireplace Surcharge. Discounts or charges should not be added together. Step 7 - When Replacement Value on personal property is purchased, the premium is determined by applying the appropriate percentage charge to the premium determined in Step 6. Step 8 - When Replacement Cost on the Mobile Home is purchased, the premium is determined by applying the appropriate percentage charge to the premium determined in Step 6. Step 9 - Add to the amount determined in previous Step the additional premium for all other mandatory or optional Personal Property Coverages and all other mandatory or optional Liability Coverages. 10 REPLACEMENT COST - Mobile Home (NS-2 and NS-3 only) Coverage A (Mobile Home) can be changed from Actual Cash Value to Replacement Cost if the mobile home is less than 10 years old. Coverage must be written for 90% or more of full replacement cost. Additional premium: 10% of the Base Policy Premium. NOTE: The Inflation Guard Provision is mandatory. Property coverages will automatically be periodically updated based on changing construction cost factors. This replacement cost coverage will be removed from the mobile home when it gets to be over 20 years of age. SECONDARY LOCATION A secondary location owned and occupied by the insured (seasonal mobile home) and located in the same state as the primary location may be insured by attaching NS The same form and deductible must be used for both the primary and secondary locations. Use the Base Premium Charts for computing the base secondary location premium. To this must be added the liability charge for an ADDITIONAL RESIDENCE PREMISES OCCUPIED. When issued in conjunction with the primary location deduct $15.00 for duplicate Personal Liability coverage. NOTE: Unsupported seasonal mobile homes are ineligible. PERSONAL PROPERTY - INCREASE OR DECREASE INCREASE - The limit for Coverage C under NS-1, NS-2 or NS-3 may be increased at the following annual premium: $3.00 per $1,000 of increase. DECREASE - The limit of Coverage C under NS-1, NS-2 or NS-3 may be decreased to not less than 40% of the limit of Coverage A at the annual credit of: $1.00 per $1,000 of decrease. 11 DEDUCTIBLES NS-1, NS-2 and NS-3 Base Deductible: A flat $1,000 deductible applies to all property losses (except for the Inland Marine forms - See Inland Marine Section). Optional Deductible: The policy deductible may be changed by applying one of the following Modification Factors to: 1. Premium from Base Premium Charts; and 2. Premium for amended limits for Coverages B and C. NS-4 Base Deductible: A flat $500 deductible applies to all property losses (except for the Inland Marine forms - See Inland Marine Section). The applicable North Star Mutual policies to which this provision applies are: 1. Homeowners; 2. Farmowners; 3. Personal Automobile; 4. Commercial Automobile (applies only to vehicles indicated on the Commercial Automobile declarations as Farm Use ); 5. Mobile-Homeowners; and 6. Inland Marine. This provision will not apply: 1. to property rented to others for a fee when that property is insured on a separate policy than the insured s owner-occupied property; 2. if the total loss is less than the highest deductible; or 3. if it is in the insured s benefit that separate deductibles be applied. 12 INSURANCE SCORE A rating factor from the table below will be applied to the total policy premium based on the insurance score of a named insured. The insurance score, from LexisNexis, is developed from credit related information including types of accounts, balance, dates opened, and account activity, plus public records items such as judgments and liens and inquiries initiated by the insured. The insurance score will be applied to all new business and to existing policies at renewal. 2. A statement to indicate an insurance score has been used will be shown on the policy Declaration when an insurance score is used. 3. The insurance score used, including No Hits and No Score, will apply for a minimum of 12 months except in cases where corrections have been made for errors in the insured s credit history file with the national credit bureaus. A higher score obtained in this process will be applied back to the beginning of the current policy term. 4. The insurance score will be ordered every 3 years, unless an insured requests it at the time of a renewal. However, we do not need to obtain the updated insurance score more frequently than once in a 12-month period. 5. A No Hits or No Score will continue to apply to a policy except in cases where an insurance score is obtained from a national credit bureau. If an insurance score is returned, it will apply. 6. In order to comply with statute, a Neutral score from LexisNexis indicates that the insured s credit report has been influenced negatively by an extenuating life circumstance such as catastrophic injury or illness, temporary loss of employment or the death of an immediate family member. 7. On or after May 1, 2018, if the insurance score drops at the 3-year review, the amount of increase to the insurance score factor is limited to 0.05 per policy period. 13 NEW HOME DISCOUNT (Policy Forms NS-1, NS-2 and NS-3) A new mobile home may be eligible for a premium discount as shown in the table below. For the purpose of this rule, a mobile home is considered new if it has been completed and model year is within the last 7 calendar years. This discount would apply to the base premium. REPLACEMENT COST LOSS SETTLEMENT TERMS PERSONAL PROPERTY - NS-55 This endorsement is available for those insureds who wish to have the following covered property on a replacement value basis: 1. 15 IDENTITY THEFT PROTECTION This protection consists of: 1. Identity Theft Recovery Service An advocacy service provided through our partnership with CyberScout. This fully case-managed expert service takes care of victims problems by leading them through the restoration process, including notifying the appropriate authorities, dealing with governmental agencies, credit collection agencies, credit bureaus, and credit grantors until the crisis is resolved. They even follow-up months later to ensure success. 2. Disaster Document Recovery CyberScout also assists disaster victims rebuild their original means of identification, as well as recreate other crucial personal and financial documents or help them with emergency authentication if they are impacted by disasters, such as hurricanes, tornados, earthquakes, including acts of terrorism, as well as home fires and floods - regardless of whether an identity theft occurred. Should an identity theft occur as a consequence of such a disaster, their advocate is there to protect and rebuild their identity. 3. Identity Fraud Expense Coverage Coverage of $5,000 is provided, at no additional cost, for expenses incurred by an insured as the direct result of identity fraud. Identity fraud occurs when an individual s personal identifying information is used without his or her permission to purchase goods or services, obtain credit, borrow money, or otherwise violate federal, state, or local law. A $100 deductible applies to Identity Fraud Expense coverage. Coverage is provided for expenses resulting from identity fraud that occurs at any time prior to the end of the policy period and is discovered by the insured during the policy period or during the one-year period that follows. In the event that the insured obtains replacement identity fraud expense coverage, the one-year extended discovery period terminates as of the date the replacement coverage takes effect. Covered expenses include: Costs for obtaining, reproducing, notarizing, or sending affidavits or other pertinent documents; Costs for telephone calls; Research fees; Lost earnings, not to exceed a maximum of $250 per day, per insured, or a total of $5,000 for lost earnings of all insureds; Loan application fees for reapplying for a loan; and Attorney fees to defend lawsuits, remove criminal or civil judgments, or challenge information provided in a consumer credit report. 16 RELATED PRIVATE STRUCTURES Occupied by Insured - HO 3048 OPTIONAL COVERAGES - PROPERTY Coverage B is intended to provide coverage for structures that are pertinent to the household and not used or designed for business purposes. Typically these are detached garages or small backyard storage sheds for lawn mowers, snow blowers, garden equipment, etc. When an additional amount of insurance is written on a specific private structure, the following premium shall apply separately to each such structure: $8.00 per $1,000 coverage (all forms) On Premises - Specifically Scheduled Structures - NS-248 When an acceptable private structure is over 1,200 square feet or there is more than 1 private structure on the premises then the private structure must be specifically scheduled for its full replacement cost. The limit that applies to Coverage B does not apply to specifically scheduled structures. With underwriting approval, a building is acceptable when the following are met: 1. Building is not used or designed for business purposes. 2. Building must be in good condition and qualify for replacement cost. 3. Building must be written at 100% of replacement cost. The following premium shall apply separately to each such structure: $8.00 per $1,000 coverage (all forms) Private Structure Rented to Others - NS-3540 Coverage can be provided for private structures on the described premises rented or held for rental to others, used for residential purposes and occupied by no more than 2 families. 20 LIVESTOCK LIABILITY EXPOSURE CHARGE (Farm Personal Liability Only) A livestock liability exposure charge is made whenever the insured owns livestock or has livestock in his/her care. This charge is made for the extra liability exposure that livestock create. In no way does this charge limit or add any coverage; it is strictly a rating criteria. Refer to Rate Pages - Farm Liability Coverage for rates. HORSES: Liability and Medical Payments may be extended to provide coverage for horses used for the insured s own pleasure or for use in the insured s own farming operation. Underwriting Rules for Liability Coverage on Horses 1. Horses must be for insured s own pleasure or for the insured s own farming operation. 2. We must be notified if horses are in any horse shows or parades. 3. The following situations are not acceptable: a. Insured permits extensive used by friends. b. Insured gives riding lessons. c. It is not mandatory to include both as long as the applicant fully understands the coverage he/she has purchased. 4. Snowmobiles, ATV s, personal watercraft, golf carts, boats and motors are not acceptable: a. If licensed for road use. (Does not apply to CF-1489) b. If used for business purposes. c. If used for racing, competition or similar contest purposes or any stunting activity. d. If rented to others. e. If the engine or gear ratio has been modified for speed or acceleration purposes. f.
