3 EQUIPMENT DAMAGE POLICY FREELANCE RESPONSIBILIY The Company believes in providing you with the relevant business tools and equipment, however frequent financial loss is suffered due to loss of, or damage to our equipment whilst it is being used under the terms of a Hire Agreement. As it is the Hirers responsibility to hold the relevant insurance for the equipment, in some instances we are able recoup the financial loss from the Hirer, however, more frequently, the Company incurs this loss due to not being informed of loss/damage quickly enough to take the appropriate action, unfortunately this financial loss does not fall under the SSE Company insurance policy. When you are contracted as Freelance Crew for SSE you are responsible for the equipment in your care, as such it is expected that you will take proper steps to prevent the loss, damage, destruction of equipment. EQUIPMENT LOSS / DAMAGE FORM It will be your responsibility to ensure that SSE is advised of any loss or damage immediately (or as soon as practicable, within 24 hours) using an online form found on the intranet. 4 Select the link [click here to Report Damage] The following screen will appear: The information that you will be required to collate is as follows: Your name You will need to confirm the statement I have read & agreed to the Damaged Equipment Policy to submit the online form. The details of the item damaged, including bar code and/or serial number and a description of the item. Select the Project Manager from the drop down list (this will ensure they see a copy of the report) Date & time the damage occurred Who is responsible for the damage / what company they are from & relevant contact details Where appropriate, confirmation that the Production Manager has been made aware Description of the damage, including cause Click on [Submit] This form will be automatically sent to the SSE Account Manager for review. SSE do accept that in the nature of the use of our equipment there will be some losses or damage, however, it must be an SSE Account Handler / Director decision whether costs will be waived or if a charge should be levied. IMPORTANT NOTES Note 1: You may be held responsible for any damage to equipment that is found, but has not been reported, or in instances where costs can not be recouped due to late reporting. 5 Note 2: You are responsible for ensuring that the equipment in your possession is kept safe and secure at all times. Any accidental damage, or wear and tear must be reported immediately and equipment will be removed from use if its continued use is considered a safety hazard or renders it illegal, until such equipment is returned to a serviceable condition. Should you continue to use such equipment following its withdrawal from use this may make you liable with any resultant legal proceedings. COMPANY RESPONSIBILTY On completion of an online Equipment Loss / Damage Report an will be generated notifying the relevant Project Manager and Directors. The contains all details of the report and should be reviewed immediately. All action and status changes are to be recorded on the intranet report. Reports have the following status: Open for reports that have been generated but not actioned Processing for reports which are being reviewed and actioned Closed for reports which have been fully investigated and closed down All expense whether written off or to be claimed requires the authorisation of a Director.
