Casa Roble Ram Booster Club High-energy music, dancing, rides, non-stop fireworks, and afterdark excitement!! Current ticket price: $ *Deadline to pay in full is Feb. 28th, CASH OR CHECK MADE PAYABLE TO CASA ROBLE GRAD NIGHT. Bus leaves CRHS on 06/01/2018 at 10pm and returns from Disneyland 1PM approximately. *Prior to boarding there will be a bag search and security. Please do NOT bring anything you know you re not supposed to, you will be refused entry and your money will NOT be refunded. Questions? 2 GRAD NITE 2018 STUDENT AGREEMENT **REQUIRED FORM 1 OF 3** (student must initial each line) STUDENT AGREEMENT: I will be at the bus location promptly by 10:00 PM to begin the check in process. I will not consume or bring alcohol or any other drugs or tobacco to Grad Nite. I will not bring medications of any kind (including Tylenol or aspirin) to Grad Nite. (a written note from parent or guardian must accompany any "prescription" medications) I will not leave the gym after check in until I am released to board the bus. No re-admits allowed. All containers, purses, hand bags etc... WILL BE EXAMINED BOTH AT CASA ROBLE AND DISNEYLAND. Grad Nite chaperones and workers are not responsible for lost or stolen valuables. I agree and understand that I will not leave Disneyland for any reason. I understand that I must return to my original bus after Grad Nite for the trip back to Casa Roble and I am responsible for being ON TIME (There is a small grace period, and then the buses will leave). I understand that I will be expected to pick up my child if he or she is suspected of being under the influence of alcohol or other drugs, displays inappropriate behaviors, is late in checking in and boarding the return trip bus or for any other inappropriate conduct. 3 GRAD NITE 2018 PARENT/GUARDIAN APPROVAL AND STUDENT WAIVER FORM **REQUIRED FORM 2 of 3 ** Student Name (please print) has my permission to participate in: CASA ROBLE High School's Grad Nite 2018 Disneyland Park, Anaheim, CA From 10:00 PM on Friday, June 1, 2018 To 1:00 PM on Sunday, June 3, 2018 (Times Subject to Change) The undersigned parent(s) or guardian(s) assumes all risks in connection with the student s participation in any and all Casa Roble High School Ramsmen, Inc. sponsored activities. I (we) hereby release and discharge the Casa Roble High School Ramsmen, Inc. and all Booster officers, employees and agents from all liability, claims or demands for any damage, loss or injury to the student, the student's property, or parent's property in connection with participation in these activities unless caused by negligence of the Booster. And I (we) do hereby certify that to the best of my (our) knowledge and belief the said participant is in good health. In case of illness or accident, permission is granted for emergency treatment to be administered. It is further understood and agreed that the undersigned will assume full responsibility for any such action, including payment of costs. I (we) hereby advise that the above named minor has had the following allergies, medicine or food reactions or unusual physical condition, which should be made known to a treating physician or which could limit participation. Waiver of Claims. I understand that California Education Code Section 35330(d) provides that all persons participating in an excursion shall be deemed to have waived all claims against for injury San Juan Unified School District, Casa Roble High School, Casa Roble Ramsmen, Inc., nor the Grad-Nite Committee or their contractors, agents, employees or volunteers illness or death occurring during or by reason of an excursion. Release, Waiver and Discharge. I hereby release, waive and discharge San Juan Unified School District, Casa Roble High School, Casa Roble Ramsmen, Inc., nor the Grad-Nite Committee or their contractors, agents, employees or volunteers, its Board of Directors, officer, employees, and agents from all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death or loss or damage to personal property, occurring during or by reason of participation in Grad Nite, including, but not limited to travel to and from Grad Nite. I fully recognize and understand that if Participant is injured, dies or his/her property is damaged, Participant is waiving his/her rights (as well as the rights of his/her heirs, executors, administrators or assigns) to make a claim or file a lawsuit against San Juan Unified School District, Casa Roble High School, Casa Roble Ramsmen, Inc., nor the Grad-Nite Committee or their contractors, agents, employees or volunteers even if San Juan Unified School District, Casa Roble High School, Casa Roble Ramsmen, Inc., nor the Grad-Nite Committee or their contractors, agents, employees or volunteers negligently or by some other act or omission causes the injury, death or damage. Acknowledgement of Risks. I acknowledge and understand that the activities involved in this Excursion may be dangerous and include risks which are inherent and cannot be reasonably avoided without changing the nature of the activity. I acknowledge that participation in the activities can cause personal injury, including emotional trauma and even death.

In the event that I, or another parent/guardian cannot be reached in an emergency, I give The Grad-Nite Committee or their contractors, agents, employees or volunteers staff the authority to obtain immediate medical attention for minor Participant. I do hereby consent to whatever x-ray, examination, anesthetic, medical, surgical, or dental diagnosis or treatment and hospital care are considered necessary in the best judgment of the attending physician, surgeon or dentist and performed by or under the supervision of the medical staff of the hospital or facility furnishing medical or dental services. INSURANCE PROVIDER: MEDICAL #/POLICY: I understand and acknowledge that neither, San Juan Unified School District, Casa Roble High School, Casa Roble Ramsmen, Inc., nor the Grad-Nite Committee or their contractors, agents, employees or volunteers insure Participant s or any other party s behavior, actions or participation in the Grad-Nite excursion and that none of the above assume any liability whatsoever for personal injuries or property damages to Participant or to third persons arising out of the transportation to or from Disneyland or participation in the Grad-Nite excursion. Participant expressly waives any claim of personal injury or property damage arising out of the transportation provided to or from Disneyland or participation in the Grad-Nite excursion. 5 ATTACHMENT "A" PARK HOURS Participants will arrive at a pre-designated time and may park hop during this time. However, Participants must be inside Disneyland by 10pm. At this time, the gates will be locked and no Participants will be allowed in or out of the park without chaperone consent. *Participants must stay in the park until 2am when Grad Nite ends. Participants must be at bus no later than 2:30am. DRESS CODE All apparel must be in good condition (excessively ripped or torn jeans, shirts, etc. are prohibited) and extremely revealing clothing is not allowed. PROHIBITED ITEMS Pets (Service animals are permitted and must be wearing an identifying assistive companion vest; pets may be kept at the on-site kennel) Wagons Skateboards Scooters (motorized and non-motorized) Inline skates Shoes with built-in wheels Bicycles Motorcycles Tricycles Unicycles Pogo sticks 2-wheeled vehicles, including Segway Human Transporters Strollers larger than 36 x 52 Suitcases, backpacks or similar bags with wheels Suitcases, backpacks or similar bags larger than 18 wide x 25 high x 37 deep (the size of large rentable lockers available at the park) Any trailer-like object that is pushed or towed by an ECV, wheelchair or stroller o Note: Guests are not permitted to pull items behind them. Any item that requires a Guest to pull it behind him or her, including a stroller, is not permitted into the theme parks. Coolers larger than 6-pack sized are not permitted into the theme parks. o Note: Guest can store coolers up to 18 wide x 25 high x 37 deep in rentable large lockers located outside the Disneyland Park Main Entrance. Lockers are not refrigerated and avail ability is limited. Guests who need to refrigerate medication may do so at First Aid. Exceptions may be made for special dietary or religious needs. Alcoholic beverages Any illegal substances Folding chairs, with the exception of cane-chair and seat-walker mobility aids Glass containers, with the exception of baby food containers, medicine or small perfume bottles (under 4 oz) Costumes on Guests older than age 9, except during special events that permit children up to age 12 to wear costumes Water pistols, toy guns and replica weapons, with the exception of those sold in the Disneyland Resort Sporting goods or equipment (e.g., baseball bats, helmets, hockey sticks, golf clubs, bows and arrows, camping equipment, chairs, stools, tables and Frisbees) Weapons of any kind (including guns, knives, billy clubs, brass knuckles, nunchucks, stars and other martial arts equipment) Self-defense equipment (pepper spray, mace, stun guns) Restraining devices (e.g., handcuffs, zip ties) or any suspicious items (e.g., box cutters, razor blades, duct tape, wire) Items that may be disruptive (e.g. laser pointers, slingshots, stink bombs, air horns) Cremated remains (e.g., urns, vases, boxes) Miscellaneous other items (tools, fire extinguishers, musical instruments, megaphones, pots and pans) Professional cameras and recording equipment. Reporters must make prior arrangements with Disneyland Resort Media Relations. Folding tripod stands that can fit inside a standard backpack are permitted.
