1 Woodcreek Sober Grad Night is sending the Class of 2019 to Disneyland as a safe and fun way to celebrate graduation!! This trip is not school or district sponsored. The Roseville Joint Union High School District is not responsible or liable for any organization or participation of students on this trip. Schedule of Events for Sober Grad Nite 2019: June 3, :30pm Check in begins for Grad Nite (see rules attached to registration packet) 11:00pm PARTY begins!!! Graduates will have a light meal provided and awesome raffle prizes will be drawn 11:30pm Load charter buses June 4, :00am Arrive at Disneyland Park graduates get a Park Hopper Pass to go between Disneyland and California Adventure 10:00pm All graduates are in California Adventure for an exclusive all night PARTY! June 5, :00am PARTY ends and students load buses. 12:30pm Buses return to Woodcreek. Come to Legends and Heroes, 5880 Woodcreek Oaks Blvd, between 6:00pm and 8:00pm on one of the dates listed below to turn in your completed registration packet and deposit/payment. 4 STUDENT BEHAVIOR and CONSEQUENCE AGREEMENT: **REQUIRED FORM 1 OF 5** (Student must initial each line) I understand that if I choose not to attend Grad Nite I will NOT BE ISSUED A REFUND unless my seat can be filled. I understand I need to check in at WHS main parking lot by 10:30pm Monday, June 3, 2019 I will not consume or bring alcohol or any other drugs or tobacco to Grad Nite. I understand there is NO food or drink allowed at check-in nor on the bus other than what is provided by Grad Nite Chaperones (this includes WATER BOTTLES, GUM AND MINTS!). I will not bring any non-essential medications of any kind (including Tylenol or aspirin) to Grad Nite. (Parent or Guardian must drop off any medications needed in original packaging to their student s chaperone. BRING ONLY THE AMOUNT NEEDED ON TRIP!) NO purses, hand bags, etc. are allowed. ALL personal effects will be brought in a clear bag and will be thoroughly searched. I will not leave the designated area/bus after check in. All belongings WILL BE EMPTIED OUT TO BE EXAMINED BOTH AT WOODCREEK AND D ISNEYLAND. Grad Nite chaperones and workers are not responsible for lost or stolen valuables. I will abide by dress code established by Disney & I will wear my WHS Grad Nite T-Shirt (un-altered) for the duration of the trip. I agree and understand that I will not leave Disneyland for any reason. I understand that I must return to my original bus after Grad Nite for the trip back to Woodcreek and I am responsible for being ON TIME (There is a small grace period, and then the buses will leave). I follow any and all rules and guidelines set forth by my bus driver and chaperone. I agree to enhanced security searches at WHS and Disneyland. This includes but is not limited to a body pat down performed by an adult of the same sex, a shoe removal check and a thorough search of all personal items brought I understand that if I am not allowed to participate in the WHS Graduation Ceremony I will NOT be allowed to attend Sober Grad Nite I understand that the parent or guardian named on my registration packet will be contacted to pick me up, if deemed necessary by a WHS chaperone or Disneyland personnel, from Woodcreek High School, Disneyland in Anaheim, CA or anywhere in between those two locations I understand that if I do not follow ALL of these guidelines I will NOT BE ABLE TO ATTEND WHS SOBER GRAD NITE and I will NOT BE ISSUED A REFUND Student Name (please print) Student s Cell Phone Student Signature

5 PARENT/GUARDIAN AGREEMENT **REQUIRED FORM 2 of 5 ** I understand WHS Sober Grad Nite 2019 is NOT sponsored or endorsed by Roseville Joint Union High School District or Woodcreek High School. I waive all claims which I might have against Woodcreek Parent Boosters Sober Grad Nite 2019 if my graduate does not comply with the terms of the student agreement. I give my permission for the above named graduate to participate in the Woodcreek Parent Boosters Sober Grad Nite 2019 celebration. We have discussed the above conditions. I understand that I will be expected to pick up my child if he or she is suspected of being under the influence of alcohol or other drugs, displays inappropriate behaviors, is late in checking in and boarding the return trip bus or for any other inappropriate conduct. I (we) hereby release and discharge the Woodcreek Parent Boosters and all Woodcreek Parent Booster officers, employees and agents from all liability, claims or demands for any damage, loss or injury to the student, the student's property, or parent's property in connection with participation in these activities. And I (we) do hereby certify that to the best of my (our) knowledge and belief that said participant is in good health. In case of illness or accident, permission is granted for emergency treatment to be administered. It is further understood and agreed that the undersigned will assume full responsibility for any such action, including payment of costs. I (we) hereby advise that the above named minor has had the following allergies, medicine or food reactions or unusual physical condition, which should be made known to a treating physician or which could limit participation. Waiver of Claims. I understand that California Education Code Section 35330(d) provides that all persons participating in an excursion shall be deemed to have waived all claims for injury, illness or death occurring during or by reason of an excursion against Roseville Joint Union High School District, Woodcreek High School, Woodcreek Parent Boosters, the Sober Grad Nite Committee or their contractors, agents, employees or volunteers. Release, Waiver and Discharge. I hereby release, waive and discharge Roseville Joint Union High School District, Woodcreek High School, Woodcreek Parent Boosters, the Sober Grad Night Committee or their contractors, agents, employees or volunteers, its Board of Directors, officer, employees, and agents from all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death or loss or damage to personal property, occurring during or by reason of participation in Sober Grad Nite, including, but not limited to travel to and from Sober Grad Nite. I fully recognize and understand that if Participant is injured, dies or his/her property is damaged, Participant is waiving his/her rights (as well as the rights of his/her heirs, executors, administrators or assigns) to make a claim or file a lawsuit against Roseville Joint Union High School District, Woodcreek High School, Woodcreek Parent Boosters, the Sober Grad Nite Committee or their contractors, agents, employees or volunteers even if Roseville Joint Union High School District, Woodcreek High School, Woodcreek High School, the Sober Grad Nite Committee or their contractors, agents, employees or volunteers negligently or by some other act or omission causes the injury, death or damage. Acknowledgement of Risks. I acknowledge and understand that the activities involved in this excursion may be dangerous and include risks which are inherent and cannot be reasonably avoided without changing the nature of the activity. In the event that I, or another parent/guardian cannot be reached in an emergency, I give The WHS Sober Grad Nite Committee or their contractors, agents, employees or volunteers, and staff the authority to obtain immediate medical attention for minor Participant. I do hereby consent to any and all x-ray, examination, anesthetic, medical, surgical, or dental diagnosis or treatment and hospital care deemed necessary in the best judgment of the attending physician, surgeon or dentist and performed by or under the supervision of the medical staff of the hospital or facility furnishing medical or dental services. INSURANCE PROVIDER: MEDICAL #/POLICY: I understand and acknowledge that neither, Roseville Joint Union High School District, Woodcreek High School, Woodcreek Parent Boosters, nor the WHS Sober Grad Nite Committee or their contractors, agents, employees or volunteers insure Participant s or any other party s behavior, actions or participation in the Grad Nite excursion and that none of the above assume any liability whatsoever for personal injuries or property damages to Participant or to third persons arising out of the transportation to or from Disneyland or participation in the Grad Nite excursion. Participant and Parent/Legal Guardian expressly waives any claim of personal injury or property damage arising out of the transportation provided to or from Disneyland or participation in the Grad Nite excursion. 8 DISNEY REQUIRED FORM **REQUIRED FORM 5 OF 5 (as per Disney)** PARK HOURS Participants will arrive at a pre-designated time and may park hop during this time. However, Participants must be inside Disneyland California Adventure by 10pm on Tuesday, June 4th. At this time, the gates will be locked and no Participants will be allowed in or out of the park. DRESS CODE All apparel must be in good condition (excessively ripped or torn jeans, shirts, etc. are prohibited) and extremely revealing clothing is not allowed. PROHIBITED ITEMS Pets (Service animals are permitted and must be wearing an identifying assistive companion vest; pets may be kept at the on-site kennel) Wagons Skateboards Scooters (motorized and non-motorized) Inline skates Shoes with built-in wheels Bicycles Motorcycles Tricycles Unicycles Pogo Sticks 2-wheeled vehicles, including Segway Human Transporters Strollers larger than 36 x 52 Suitcases, backpacks, or similar bags with wheels Suitcases, backpacks or similar bags larger than 18 wide x 25 high x37 deep (the size of large rentable lockers at the park) Any trailer-like object that is pushed or towed by an ECV, wheelchair or stroller *Note: Guests are not permitted to pull items behind them. Any item that requires a Guest to pull it behind him or her, including a stroller, is not permitted into the theme parks. Alcoholic Beverages Any illegal substances Folding chairs, with the exception of cane-chair and seat-walker mobility aids Glass containers with the exception of small perfume bottles Costumes on guests older than age 9, except during special events that permit children up to age 12 to wear costumes Water pistols, toy guns and replica weapons with the exception of those sold in the Disneyland Resort Sporting goods or equipment (e.g., baseball bats, helmets, hockey sticks, golf clubs, bows and arrows, camping equipment, chairs, stools, tables and Frisbees) Weapons of any kind (including guns, knives, bully clubs, brass knuckles, nunchucks, stars and other martial arts equipment) Self-defense equipment (pepper spray, mace, stun guns) Restraining devices (e.g., handcuffs, zip ties) or any suspicious items (e.g., box cutters, razor blades, duct tape, wire) Items that may be disruptive (e.g., laser pointers, slingshots, stink bombs, air horns) Cremated remains (e.g., urns, vases, boxes) Miscellaneous other items (tools, fire extinguishers, musical instruments, megaphones, pots, pans) Professional cameras and recording equipment.
