1 MEMBERSHIP WARRANTY TERMS, CONDITIONS AND DEFINITIONS OF THE WARRANTY In return for you paying the premium and continuing to meet all the conditions of this warranty, if VHI accepts the premium it will provide cover under the Membership Warranty agreement as described in this document. You must apply for the scheme that you require and the relevant premium must be paid for the scheme option chosen as per your Membership Service Agreement Definition of a Warranty: A Warranty sometimes called a Service Agreement, a Service Contract, or a Maintenance Agreement, is a prolonged Warranty offered to consumers. The Warranty may be offered by the Warranty Administrator, the retailer or the manufacturer. Warranties cost extra and are a percentage of the item's retail price. Words appearing in this contract in bold print have specific meanings which are explained in the GLOSSARY under SECTION SECTION 1 ELIGIBILITY To be eligible for cover under this warranty you must meet the following conditions at the start date: - You must be the first named member (Principal member) on the VHI Membership Agreement; - You must be in good health; - You must work, live and have the right to reside permanently in South Africa; - You must agree to keep to the terms and conditions of the warranty. 2. SECTION 2 LIFE COVER, PERMANENT TOTAL DISABILITY AND CRITICAL ILLNESS COVER WHAT WE WILL PAY: If you should die or become permanently totally disabled or suffer from a critical illness during the period of cover, we will pay the benefit on your behalf across to the Membership Provider. This benefit will equal all current and future Service Fees payable in accordance with your VHI Membership Agreement at the date the covered event occurred: - Less any rebate to which you would have been entitled had you settled the VHI Membership Agreement on the day the covered event occurred; - Less any amount owing in excess of the maximum benefit payable under the warranty. WHAT WE DO NOT PAY FOR: LIFE COVER We will not pay any benefit on your behalf to the Membership Provider if your death arises directly or indirectly from any of the following: - a self-inflicted injury or suicide within 2 years of the start date;
2 - any condition you had at the start date; - any condition for which you have received treatment or advice during the 12 months immediately before the start date; - a condition related to AIDS or HIV unless contracted during medical treatment which is not as a result of drug abuse or a sexually transmitted disease; - war, riot, radioactive contamination, nuclear accidents and similar risks; - your participation in a criminal act; - your participation in a hazardous pursuit; - driving under the influence or above the legal limit of alcohol intake or drug abuse; - refusing medical treatment as recommended by your own medical practitioner. PERMANENT TOTAL DISABILITY and CRITICAL ILLNESS COVER We will not pay any benefit on your behalf to the Membership Provider if your permanent total disability or your critical illness arises directly or indirectly from any of the following: - any condition for which the life cover will not be paid; - any condition which arises within 30 days of the start date of the Membership Agreement; - any condition for which you had received treatment or advice for during the 12 months immediately before the start date of the Membership Agreement; - pregnancy, childbirth, the ending of pregnancy or any related complications; - a medical procedure undertaken at your request which is in the opinion of a doctor not necessary to maintain the quality of your life; - unreasonable failure to obtain or follow medical advice; - a permanent total disability or critical illness arising while you are working outside of South Africa. 3. SECTION 3 DISABILITY COVER WHAT WE WILL PAY: If during a period of insurance you become disabled for more than 30 consecutive days we will pay a benefit on your behalf across to the Membership Provider equal to your monthly Service Fee. The disability benefit is payable on condition: - have been in work when your disability occurred; - be under the continuous care of a doctor for this disability; - be prevented from working only as a result of this disability; - give us any evidence we may ask for in order to prove your claim is valid and continues to be so; When paying your claim we will consider the first day of disability to be the day a doctor certifies you are disabled and unable to attend work.
3 Thereafter will pay your monthly Service Fee for each further 30 continuous days you remain disabled, or until the end date occurs, whichever is first. When you are no longer disabled, you must return to work for at least 6 continuous months before you can make a claim for a related disability, or for one complete month before you can make a claim for an unrelated disability. WHAT WE DO NOT PAY FOR: We will not pay your disability benefit across to the Membership Provider if your disability arises directly or indirectly from any of the following: - any condition listed in Section 3 above What We Do Not Pay For - a condition of mental or nervous origin including stress, anxiety or depression unless the condition is being treated under the direct, and regular care of a consultant psychiatric specialist; - a back or spinal condition unless it is shown to exist by a MRI or CT scan conducted or supervised by an appropriate specialist. 4. SECTION 4 - CLAIMS - All claims must be notified as soon as possible and no later than 30 days after the covered event by contacting the scheme administrator (Ricforb Warranty Administrators) - details in the Statutory Disclosure attached hereto. - A claim form will be sent to you or your beneficiary in the event of your death. You or your beneficiary in the event of your death must return it to the administrator within 30 days of receipt. Please ensure that all sections of the claim form are fully completed and any relevant documents are enclosed and sent the administrator. - Should you need any help in completing your claim form please contact (Ricforb Warranty Administrators) - details in the Statutory Disclosure attached hereto. SETTLING A CLAIM - We will need proof of your death, permanent total disability, critical illness, disability, retrenchment or hospitalization and the circumstances leading to your claim. - A death claim must be notified in writing and a certified copy of your death certificate and a certified copy of your ID Book must accompany the claim. - If you are claiming for permanent total disability, critical illness, or disability, we will require a doctor s certificate confirming your condition. We may ask you to go for a medical examination with a doctor appointed by us. We will pay the cost of the examination. - During the period of a claim, we will need proof that you are still disabled; you must pay any costs of getting this proof. - You must take all reasonable steps to keep the period of disability, hospitalization as short as possible. - We may ask for other evidence in support of your claim. - Where you (the warranty holder) dispute the Warranty Issuer rejection of your claim you have 90 (ninety) days after the date of receipt of such notice to make representations to the Warranty Issuer in respect of the rejection of your claim and you have a further
4 6 (six) months to institute legal action by way of the service of Summons against the Warranty Issuer failing which you will forfeit your claim and no liability can arise in terms of such claim. 5. SECTION 5 - WHEN THE WARRANTY COVER ENDS Your cover under this warranty will end and no further benefit will be payable, as soon as one of the following occurs: - The date your Service Fee payments are due to end; - The date your Service Fee payments actually end; - A benefit is paid in respect of death, permanent total disability or critical illness; - You fail to pay the premium; - We advise you that your warranty cover has ended. 6. SECTION 6 - GENERAL CONDITIONS - If you do not keep to the terms and conditions of this warranty, you will not be entitled to any benefit under this warranty. - If you gave false or misleading information when you applied for cover under this warranty, and this information affected the decision to cover you, your cover under this warranty will end, we will not pay any benefit and we will refund the percentage of the premium that we think is reasonable. - If you give false or misleading information when you make a claim you will not receive any benefit under this warranty and your cover under this warranty will end. - If any benefit is paid as a result of your false claim, you will have to repay any benefits you have received and we will take legal action against you. - The contract between you and us is made up of this warranty, any endorsement, and any written statement of your medical conditions and any other information provided by you. - The rights under this warranty cannot be transferred to anyone else and this warranty cannot be used to protect any person other than you, unless the membership is sold in an arm s length transaction - When your cover under this warranty ends it will not have a cash value. - We have the right to change or cancel your warranty cover under this warranty. You will be told at least 30 days before the change or cancellation takes effect. - For the purposes of disclosure of private underwriting and claims information you consent and acknowledge that the sharing of claims information and underwriting information (including credit information) by warranty issuer is essential to enable the underwriting of warranties and assess risks fairly, and to reduce the incidence of fraudulent claims, and accordingly waive any rights of privacy of warranty information in respect of any claim made. - You also acknowledge that the information provided by you may be verified against other legitimate sources or databases. You also waive any rights of privacy and consent to the disclosure of any information relevant to any warranty of claim concerning you. - Should you wish to cancel your cover under this warranty, please do so in writing with one calendar month notice to the administrator. - The laws of South Africa govern this policy.

SECTION 7 GLOSSARY Accident Membership Agreement Blindness Cancellation Cancer Coronary Artery Disease Requiring Bypass Surgery Critical Illness Disability/Disabled Doctor Due Date End Date Membership Provider Hazardous Pursuit Heart Attack Hospitalization Covered Event Covered Person Kidney Failure A specific, sudden, unpredictable, unusual and unintended external action with no apparent and deliberate cause but with marked effects. The Vacation Hub International Membership Service Agreement with which you have arranged cover under this Warranty. The total and permanent loss of all sight in both eyes. This Warranty may be cancelled at any time by the member giving one calendar months notice in writing to the administrator. Malignant tumours characterized by the uncontrolled growth and spread of malignant cells and invasion of tissue. This includes leukaemia, and Hodgkin s disease, but excludes Stage1 Hodgkin s Disease, non-invasive cancers in situ and skin cancer other than malignant melanoma. A condition for which you undergo open-heart surgery on the advice of a Consultant Cardiologist to correct narrowing or blockage of one or more coronary arteries with by-pass grafts. No cover will be provided in respect of non-surgical techniques such as balloon angioplasty or laser relief. One or more of the following conditions: heart attack, blindness, coma, HIV, loss of limb, loss of speech, major burns, paraplegia, replacement of heart valve, surgery for a disease of the aorta, cancer, stroke, major organ transplant, coronary artery disease requiring bypass surgery, and kidney failure. Critical illness must be certified by a duly qualified and registered medical doctor with the appropriate specialist knowledge. Being unable to work at any occupation, which you are reasonably able to do, given your experience, education or training because of an injury illness. A medical practitioner registered with the South African Medical and Dental Council. The doctor who confirms your condition when you are making a claim cannot be you, a relative or a close friend. The date on which your monthly service fee premiums are payable in accordance with your Membership Agreement The last day of your warranty cover as defined in Section 5 When the Cover Ends. The Company providing membership under your Membership Agreement, namely Ricforb Finance Company (Pty) Ltd t/a Vacation Hub International Flying, other than as a fare paying passenger, hang gliding, ballooning, land and water based motor sports, winter sports, sub aqua diving, mountaineering, and any other activity which would reasonably be expected to enhance the chance of a claim under the warranty. The death of a portion of the heart muscle as a result of Inadequate blood supply as evidenced by an episode of chest pains, new electrocardiographic changes and elevation of cardiac enzyme levels. Being admitted to hospital and registered as an in-patient because of an accident or illness. You must be selfemployed and not in full-time employment to be eligible for this cover. The accidental death of the Covered Person; Permanent Disability of the Covered Person caused by Accident; Temporary Disablement of the Covered Person caused by Accident The warranty holder / person / signatory identified in the Membership Service Agreement. End stage renal failure, resulting in chronic irreversible failure of both kidneys, as a result of which regular renal dialysis or a renal transplant is required.
6 Loss of Limb Loss of Speech Major Burns Major Organ Transplant Monthly Service Fee Paraplegia Period of Warranty Permanent Total Disability Warranty Premium Replacement of Heart Valve Start Date Stroke Surgery for a Disease of the Aorta Temporary Disability/disabled VHI You/Your Loss by physical separation of upper arm, forearm, hand, thigh, lower leg or foot as a result of an accident. The Covered Person must be able to prove that the loss occurred as a result of external causes. Total and irrevocable loss of the ability to speak which must be established for a continuous period of 12 months. In the event of loss of speech, we must be notified within 3 months of the onset of loss of speech. Third degree burns covering at least 20% of the body surface. The receipt of a heart, liver, lung, kidney, pancreas or bone marrow by means of a transplant. The sum you must pay to the finance provider each month in respect of the Membership Agreement. The total and irreversible loss of use of both legs, or both arms The period over which the Service Fees are payable in accordance with your Membership Agreement The warranty holder being totally and permanently unable to work in any occupation which you are reasonably able to do, given your experience, education or training, because of an injury or illness. Your permanent total disability must be certified by a duly qualified and registered medical doctor. Vacation Hub International Membership Warranty Your monthly premium is payable in advance and must be paid on the due date. If the premium is not received on the due date, the warranty will automatically be cancelled with effect from midnight on the last day of the month in which a premium was received and no cover will be in place and the warranty issuer shall not be obligated to accept any premium tendered after this date but may do so upon such terms as it at its sole discretion may determine. The replacement of one or more heart valves with artificial valves. This includes the replacement of aorta, mitral, tricuspid of pulmonary valves with artificial valves due to stenosis or incompetence or a combination of these conditions. The date your Membership Agreement with VHI starts. A cerebrovascular incident lasting more than 24 hours and resulting in permanent neurological damage. Evidence of permanent neurological deficit must be produced. The actual undergoing of surgery for a disease of the aorta needing excision and surgical replacement of the diseased aorta and not its branches. Traumatic injury of the aorta in excluded. Being unable to work at any occupation, which you are reasonably able to do, given your experience, education or training because of an injury illness, for a period exceeding 1(one) month but not exceeding 12 (twelve) months.
