1 Medical Waiver and Release/Authorization for Treatment: I hereby give my consent to Summerfuel Sports personnel to provide, through a medical staff of its choice, any and all medical attention and emergency medical services as warranted in the course of my son s participation in Summerfuel Sports. As parent/guardian, I am solely responsibility for any and all expenses incurred as a result of such treatment. I am fully aware of and appreciate the risks, including the risk of catastrophic injury, as well as other injuries and losses associated with participation in this outdoor summer program. I further agree on behalf of myself, my heirs, and personal representatives, that selected Summerfuel Sports programs, along with the directors, medical staff and counselors of this organization, or the administration or employees of the Summerfuel Sports staff shall not be liable for any injury or damage occurring as a result of my son s participation in this program. Camp Refund Policy No refunds will be issued for FCB Escola. However, players who cannot attend the program after registering can use their program fee towards future (up to 2019) Summerfuel/FCB Escola programs. Dates, fees, itineraries and courses may be changed without notice should circumstances require it. Academic Study Associates/ Summerfuel / Barcelona Soccer Schools USA reserves the right to cancel any program and to decline to accept any person as a member of one of its programs. If Summerfuel cancels a program, the full program fee will be refunded. Page 1 of 5

2 Rules, Regulations and Responsibilities: As an FCB Escola camp participant, you will be afforded fantastic opportunities to learn, grow, discover and build new friendships. With these opportunities and freedoms however, come responsibilities. Throughout the program, you will be expected to conduct yourself as a mature, law-abiding, cooperative and responsible individual. We insist on an atmosphere of respect, trust and consideration in your dealings with fellow students and your program leaders. Behavior inconsistent with such will not be tolerated and will result in disciplinary action being taken up to and including expulsion from the program. Participation: Campers must attend all training sessions and onsite workshops. The Head Coach and / or Summerfuel Senior Staff will address violations of this rule; repeated violation will result in dismissal from the Program. Respect for Soccer Schools Participants & Staff; Respect for Host Institution Personnel, and Property: Participants are expected to respect the individual rights of all members of the Summerfuel community and treat each with courtesy. Participants must show respect for Summerfuel and host institution and FCB Escola equipment and facilities. Participants will be held financially responsible for damage to Summerfuel or host institution facilities and/or equipment. Resources: Copies of background check, health care and discipline policies, and grievance procedures are available upon request. Safety Guidelines Roaming Area: During the duration of the camp, students must stay in the soccer field and dining areas of campus. Obeying the Law All participants are expected to obey the law. Any participant caught breaking the law or suspected of doing so will be disciplined accordingly. If you are sent home for any reason, you will receive NO REFUND whatsoever of the unused portion of the program. Alcohol/Drugs: Summerfuel & FCB Escola has a zero tolerance policy for alcohol or illegal drug use. Use or possession of alcohol or any illegal drugs in any quantity, (e.g., marijuana, hashish, cocaine or any unauthorized prescription drug), or possession of any drug paraphernalia is strictly forbidden and will result in immediate dismissal from the program. Suspicion of Alcohol/Drug Use Participants who are suspected of possession or use of alcohol and /or drugs will be placed on probation and their parents will be notified. If there is another instance of strong suspicion or evidence that this participant has broken any Summerfuel rule, they will be subject to immediate dismissal. Page 2 of 5

3 Medical Care/Expenses Participants are responsible for providing Summerfuel with an accurate medical history & current health insurance prior to the program start date. Participants failing to disclose medical or psychiatric conditions may be subject to dismissal. All associated medical expenses are the responsibility of the participant receiving treatment and his parent or guardian. In the case that your child is hospitalized and the hospital asks for payment upon release of your child, then parents will be obligated to pay the hospital directly by credit card or money transfer. 4 Parent / Legal Guardian Agreement: I have read the Summerfuel/Barcelona Soccer Schools website and hereby give my permission for my child/ward to participate in this program during the summer of With the application and this Agreement, I confirm my awareness and acknowledge the risks of injury which may be associated with travel, particularly in foreign countries. My child/ward is enthusiastic and prepared, and I believe he is capable of handling both the emotional and physical aspects of the program as well as any risks involved. Furthermore, I agree that, should my child/ward s conduct, at the sole discretion of Summerfuel, be deemed to be in violation of Summerfuel rules or otherwise detrimental to the maintenance of standards or to the successful operation of Summerfuel s program, Summerfuel, in its sole discretion, may dismiss him/her from the program. I further agree that Summerfuel shall have no further responsibility for my child/ward upon his dismissal from the program, and I understand that such dismissal may occur at a location far from the child/ward s home. I affirm that Summerfuel shall have the exclusive authority to determine the manner and means of transporting my child/ward home without supervision, should Summerfuel be unable to get in contact with the students listed contacts in a timely manner, and that all additional expenses (including but not limited to the entire costs of the transportation) shall be borne completely by me, and that Summerfuel shall have no obligation to provide any refund of the tuition fee with respect to any dismissed student. Notwithstanding the foregoing, in the event Summerfuel elects to send my child/ward home with a supervising Summerfuel representative, all expenses of such Summerfuel representative (including but not limited to the entire costs of the transportation) shall be borne completely by me. Prior to the commencement of the program, I shall sign a credit card authorization to be held by Summerfuel authorizing the payment from my credit card of any such expenses of my child/ward and the Summerfuel representative in the event of the dismissal of my child/ward. I have read the payment schedule and refund schedule on this application and agree to the terms cited. I understand and acknowledge that no refunds are available for any cause. It is understood that Summerfuel may make use of students photographs and testimony in publicity materials, including the brochure, poster and website, without payment of any consideration, and I hereby grant Summerfuel permission for such use. Furthermore, in applying for an Summerfuel program, I hereby understand and accept the following terms and conditions, the violation of which may result in my child/ward s dismissal, without further notice: The possession or use of drugs or alcohol is strictly prohibited. All participants must adhere to all rules of safety and conduct at all times, including those promulgated by Summerfuel, the educational institution and the jurisdiction where the educational institution is located. Any violation of the rules, terms or conditions, as well as behavior incompatible with the programs, could result in dismissal of a student at parent/guardian expense as stated above. I authorize Summerfuel, at its sole discretion, to place my child/ward at my own expense and without any further consent or advance notice in a hospital for medical services and treatment or, if no hospital is readily available, to place my child/ward in the care of a licensed medical doctor for treatment. By your completion and submittal of registration forms, you agree and acknowledge that you are over 18 years of age and that you have read and agree to the terms and conditions, including the Privacy Policy, of Soccer Camps International as provided, and as updated from time to time, on website and which includes that SOCCER CAMPS INTERNATIONAL HAS NO AUTHORITY OR CONTROL OVER THE OPERATION AND/OR RULES OF THE CAMP.
