3 SCHEDULE 0.0 INTRODUCTION AND INSTRUCTIONS 1. Tenderer shall complete this Appendix by giving and submitting all details, information and supporting documents, as required under the various Schedules of this document. Additional sheets may be used as required and attached to the appropriate Schedules as applicable. Responses such as will be advised are unacceptable. Failure to submit all information required under the Schedules or failure to comply fully with these instructions in any way may render the Tender null and void. 2. The Schedules submitted under this Appendix constitute Tenderer s technical proposal and shall form the basis for the technical evaluation of the Tender. 4 SCHEDULE 1.0 Understanding) TENDER EXECUTIVE SUMMARY (Project Scope and The Tenderer shall provide here, in no more than 10 standard A4 sheets, an executive summary outlining the key elements of its bid including, but not limited to: Understanding of Scope o The Tenant shall outline his understanding of the required Scope and Services of the Project based on the Project Brief and the Scope of Services in this RFP. o How Tenderer intends to deliver the Scope of Services. o Demonstration of its understanding of the requirements of the Project and its obligations under the Lease Agreement. Proposed Project Strategy o The Tenant shall detail his strategies to successfully deliver the project. The strategies should be directly linked to the Client's objectives and the Scope of services to be undertaken by the Tenant o The Tenant should clearly demonstrate the depth and extent of consideration they have given to the realisation of the project objectives and requirements. Any objectives or requirements that are needed or require further expansion or clarification must be raised as a query prior to the deadline for submission of proposals. Detailed Methodology and Presentation o For each element of the proposed Services, the Tenant shall set out the type of services to be provided, in his proposed methodology inclusive of of Fuel delivery and other expected deliverables. The emphasis should be on the delivery of the Fuel Project with attention to quality and HSE execution o How Tenant intends to manage and deliver the various phases of their operations and services to the boating community o Major risks involved in the Project and arrangements to resolve them. 8 SCHEDULE 4.0 TENDERER ORGANIZATION STRUCTURE 1. In the event Tenderer is a consortium, an organization identifying the consortium lead company, all companies participating in the consortium and the relationships between those companies should be submitted. 2. In the event Tenderer is a member of a group of companies, a corporate organization chart showing the parent and all other companies forming the group and identifying Tenderer s relationship to other companies, must be submitted. 3. 9 SCHEDULE 5.0 PROPOSED STRUCTURE & ORGANIZATION Tenderer shall attach an organization chart proposal, based on its experience with other Marine projects and its knowledge of requirements, for performing the marine related SERVICES. The Organization Structure should show Tenderer s inter-relationships and reporting lines among all disciplines of its project team with responsibility matrix, safety functions, as well as its proposed reporting lines to LREDC. Tenderer s organization chart must show, in particular, Tenderer s. KEY PERSONNEL identified and whose CVs are included in Schedule 6.0, should be identified on the organization chart. 10 SCHEDULE 6.0 KEY TENANT PERSONNEL Tenderer shall provide here all details, as required, on the individuals it proposes to fill the positions of KEY CONSULTANT PERSONNEL as listed below, and any other positions as may be required by LREDC and/or considered by Tenant to be Key Positions: On each member of KEY TENANT PERSONNEL, Tenderer shall as a minimum provide the following details, together with a detailed CV. Proposed position. Name and age of candidate. Educational, technical and professional qualifications with dates and certificate numbers. Details of current and past employment including number of years working in the discipline, dates, duration, and names of previous employers and brief descriptions of duties and responsibilities. A confirmation by Tenderer that the candidate will be available for LREDC interview and for the SERVICES. 12 SCHEDULE 8.0 RELATED EXPERIENCE TENANT shall submit a statement of its total corporate experience of services for major and similar to the SERVICES. The following details are required as a minimum. The statement should cover all works completed over the last five (5) years. 13 SCHEDULE 9.0 FINANCIAL INFORMATION AND INSURANCE Tenderer shall provide here a description of its financial condition and capability. Tenderer shall also attach a copy of each of the last three (3) year s Annual Report including Directors, Company Secretary s, and Auditor s reports, including its financial trading profit and loss and funds statements and balance sheets. These statements must also be provided from parent, associated or related company, where the Bidder is to be supported by such company; and all members of a Joint Venture or a Consortium. Tenderer shall provide details of its current insurance arrangements appropriate to the SERVICES including current workmen compensation, public liability and professional indemnity insurance. 14 SCHEDULE 10.0: QA PROCEDURES The Tenants s Quality Assurance and Quality Control System dedicated for this Services is to be provided in this Schedule. This document will be reviewed by LREDC and any comments made shall be incorporated by the Tenant. Final document must be issued three (3) weeks after the Lease Commencement Date of Agreement The quality system shall be formalized in a Quality Manual which documents general policies, procedures and practices of the organization covering all quality related elements, addressing as a minimum requirements of LREDC. Details given are to be for all WORK undertaken as a sole CONTRACTOR, joint venture partner or SUBCONTRACTOR.
