3 Hay Insurance Instructions, Guidelines & Forms Hay Insurance offers a new Moisture Deficiency Endorsement for dryland hay, based upon rainfall at specified weather stations. See the Moisture Deficiency Endorsement section for details. If you want this endorsement you must notify the office or complete the enclosed Hay Change Request form. Hay was *auto-renewed and billed, based on last year s options. The billed coverage is binding, unless AFSC is notified otherwise, prior to the last day of February. (*Some exceptions apply, read your package carefully) Insurance Proposals Individualized proposals are provided to ensure you understand the coverage options and costs for the current year. Review it carefully to ensure you have elected the coverage that best suits your farm/ranch. Statement of Coverage and Premium (billing) Review and verify that the hay type(s), insured acres, and land locations you have been billed for, are correct. Ensure that the price option and coverage level indicated are what you want. Early-Payment Discount 2% 2% discount is applied to premium payments received by AFSC on or before June 25 th. Cheques post-dated to June 25 th are acceptable If your billing is correct and you have no carry-over, then you have nothing more to do until you are finished haying. You are then required to file Harvested Production Reports (HPR S). Claims are triggered based on information provided on HPR s If you have a loss AFSC adjusters may be sent to your farm to measure /account for your production. Statements and a cheque (if applicable) will be mailed to you. If changes are required: Before the last day of February, select one of the following options to make changes: Phone your AFSC district office, or Complete and return the enclosed Hay Change Request form and return it by: o Fax, o Mail, or o deliver in person to your district office- Save time and make an appointment For New Applicants: Contact an AFSC insurance office, or phone the call center at AFSC and ask for a quote. To buy insurance, stop into an AFSC office - Avoid waiting and call for an appointment. Information Required to Insure Remember to bring the following information: Legal descriptions for acres seeded to hay Number of acres/field and type of hay Business numbers ((BIN) for GST) Social insurance numbers (SIN) Name, phone, address of shareholders etc. Hay Insurance Change Request Form Enclosed in your package Crop Type(s): Indicate the type of hay you want to insure. All rented and owned land sown to hay types elected, must be reported and insured. Dryland hay types: grass-(less than or equal to 50% legumes) legume-(greater than 50% legumes) alfalfa (greater than 50% alfalfa) Irrigated hay type: - alfalfa Price Option: Review the proposal and select one (1) price option from the list provided. If you want to purchase the Moisture Deficiency Endorsement, select Weather Stations: Refer to the Weather Station Map and Weather Station Normals, and then select one (1), two (2) or three (3) weather stations nearest your land. (Weather stations you select are subject to approval by AFSC Staff) - When multiple stations are selected data is averaged in calculations. Coverage Options for MDE: Review the program information and your proposals, then select one (1) season option from the list provided. Consider these questions to help you decide the best options. 4 HAY INSURANCE - INSTRUCTIONS, GUIDELINES & FORMS Is Spring Soil Moisture important to you, or do you want to exclude it? Which coverage allocation or weighting do you want? Acres, Land Locations and Crop Types: Record the legal locations (the Quarter - Section, Township, Range, and meridian Report the total number of acres (rented and owned) of the hay type you want to insure. Carry-Over Hay In accordance with the Contract of Insurance and to protect your insurance coverage, AFSC must be able to identify your current year s hay production. Carry-over is hay inventory which you may have purchased, harvested in previous years or uninsured production. You will receive a Report of Hay in Storage Prior to Harvest form enclosed in your package. Please complete and return BEFORE you begin harvesting, (and not later than July 15). when AFSC is unable to identify the current year s production, you may not be eligible to claim a loss. Harvested Production Reports (HPR) Enclosed in your package Within 15 days of completion of harvesting your hay, (within 5 days if you anticipate a loss,) and not later than October 15 complete and file an HPR. The information contained is used to individualize your coverage and may trigger a payment. A $50.00 late filing fee is required for HPR s filed after October 15 Failure to file an HPR results in your hay production being recorded as zero which results in a loss of coverage in the future. Contracts If you harvested hay from acres that were not insured, (rejected fields, acquired/purchased hay), record number of bales, the location, number of acres, and whether that production is stored separately from insured production. If you harvest less acres than you insured record the difference in the appropriate column for that land location and explain the reason. District office staff will complete pre-harvest information. If you harvested more than one bale size or type from a single field, use another line to re-list the field information and record the weight and number of each bale size/type separately. Scale tickets or receipts are required when Hay is sold or exchanged for services/rent In the lower part of section B, indicate the date you completed your first/second cuts. Part C. Harvested Production Summary Part B Report Field documentation. Part C Report Production Stored/Fed/Sold Note: Production from uninsured acres should not be recorded in this section. Part D & E Penalties and Declaration Read, date and sign. If under irrigation: Provide a log showing the dates and approximate amounts of rainfall, and water applied to your hay crops. Part A. Carryover If you have carryover Hay, and you anticipate you may have a claim, notify the office prior to harvest. Adjusters must be able to identify new production from carryover, or it will all be counted as the current year s production. Contact an AFSC Insurance office or the Call AFSC Part B. Field Documentation Answer the four questions and if some acres were cut for silage, pastured or plowed down, check the YES box and indicate whether AFSC was notified. Complete this document, first and then use information to complete your HPR. 8 EXPORT TIMOTHY HAY - INSTRUCTIONS, GUIDELINES & FORMS Step 2 Starting with Lot #1 - record the legal location of the first field, the number of acres and bales harvested, then estimate the grade, average bale weight, and percent of moisture. Clearly mark where each lot begins and ends, so that AFSC adjusters can identify the lots. If you baled at different times, if grades vary, and/or when bales are stored separately, use a separate line in the Step 2 section, and record the bales as lot #2 completing rest of the line. Enter the location where the bales are stored in Step 3. Continue until all of your fields/bales are accounted for, then sign and date the form. Use the information to complete your HPR and attach them to your Harvested Production Reports when you return them to the district office for processing. Deadline October 15 Harvested Production Reports Enclosed in your package Within 15 days of completion of harvesting your export timothy hay, (within 5 days if you anticipate a loss) and not later than October 15, you are required to complete and file an HPR. It is important to file as the information contained is used to individualize your coverage and may trigger a payment. Indicate total number of lots and bales harvested; and if the yield/grade is less than expected, provide an explanation for each field. Record the following for each shed or tarped stack: land location lot number Instructions for Completing HPR s Verify that preprinted information is correct: If something is not correct, or you cannot find your HPR s, contact the office and have them replaced. If you know the current year s Animal Unit Months (AUM s), record it in the area provided, otherwise use the previous year s AUM s. If you do not know your AUM s, record the number of head allotted, type(s) of animal, their average weight, and number of pasturing days. Community pasture acres will be determined based on the AFSC risk area carrying capacity and the producer s allocation.
