What you need to fulfill You must serve the company for 2 years without interruption with a clean disciplinary record. What you need to give us Details of the certified course content from university, college or institution including the duration, fees, results sheet and/or certificate Result sheet is mandatory for reimbursement The course must be associated to the work you do In the event an employee fails one or more subjects, the re-sit fee will not be accomodated via this scheme Unit Managers (UM) & above are eligible for reimbursement of MBA, Msc or any Masters Programme Page 05

8 50% of the course fee and 100% of the examination fee can be reimbursed within the annual entitlement Registration and any other payments made to the institute cannot be accomodated via this scheme You must apply for reimbursement within 3 months upon receiving results Subject wise reimbursment is not allowed, however stage or semester wise reimbursement is possible How to apply? You make the request via HCM with supporting documents attached Once approved, GHR will make the payments to you along with the salary (applications approved by the 5th of the respective month will be reimbursed through pay of the same month) Page 06

9 Benefit No. 03 PROFESSIONAL MEMBERSHIP Become accredited Why not gain a professional membership? Who can apply? Unit Manager and above levels from the date of joining Executive to Asst. Manager on completion of one year of service Membership must be relevant to the current job scope. Page 07

You make the request via HCM within 3 months upon making the payment for membership / renewal (attach soft/scanned copies of the supporting documents which are the confirmation letter from institute, original payment receipt & membership card if any) Verification of application & approval / rejection status informed to you On approval GHR would make payments to you along with your salary (applications approved up to 5th of the respective month, would be reimbursed through salary of the same month) Page 08

STAFF SIM You make the request via HCM Upon approval you visit the customer service outlet (except for franchise outlets) with the company ID, NIC & HCM approval Collect Staff SIM Follow steps 2 & 3 for the i250 SIM Upon resignation, the connection will be converted to an ex-staff package. Definition of immediate family Married employees: spouse, child over 18, mother, father, sister, brother, mother-in-law and father-in-law Unmarried employees: mother, father, sister and brother Page 10

13 Benefit No. 05 STAY IN TOUCH! Enjoy the benefits of CDMA Connections! Now you are entitled to two CDMA connections from the date you join. Two CDMA connections will be offered to you, on request. Choices are as follows: Zero monthly rental connection FOC (only pay for the unit) - No free outgoing calls Rs.100/- monthly rental connection FOC (only pay for the unit) - 60mins free per month on outgoing calls Page 11

You make the request via HCM Upon approval you visit the customer service outlet (except for franchise outlets) with the NIC, company ID and HCM approval Receive your CDMA connection The process for ex-employee At the time of resignation, the above connections will be converted to an ex-employee CDMA Scheme - Rs.200 package with no free outgoing calls. (Connection will be converted to an ex-staff package automatically) Page 12

15 Benefit No. 06 STAY CONNECTED! You can surf more and more A data connection will be provided at a subsidised monthly rental, from the date you join. The package cost may vary according to the prevailing pricing. The dongle needs to be purchased separately. How to get your connection? You make the request via HCM Upon approval you visit the customer service outlet (except for franchise outlets) with the NIC, company ID and HCM approval Receive your Data Connection On receipt of the monthly bill you make payments through cashier point Page 13

16 Benefit No. 07 ENTERTAINMENT UNLIMITED! Presenting you with a Global Television Experience! You become entitled to a free primary and secondary DTV connection at a subsidised rate for the same location, upon completing 6 months of service. You may obtain a staff DTV connection by paying Rs. 2,990/- per connection from the date you join Package cost may vary according to the prevailing pricing Page 14

17 How to get your connection? You make the request via HCM Upon approval you can visit the customer service outlet (except for franchise outlets) with approved documentation and NIC or Passport You must fill the DTV Customer Application and obtain receipt from the counter DTV Technical to make necessary installation at the requested location, in liaison with you On receipt of the monthly bill, you make the payment through cashier point The process for ex-employee Upon resignation, connection will be converted automatically to a ex-staff DTV connection as same as an external customer package. 18 Benefit No. 08 GO FOR YOUR FIRST VEHICLE We want to be a part of your life s special milestones We provide you with financial assistance to purchase a vehicle, once you complete 2 years of continuous service and have a clean disciplinary record. What you need to fulfill Have no outstanding loans for the past 06 months Have no more than 40% deductions of your basic salary What you will be entitled for Co-ordinator & Senior Executive - upto Rs. 100,000/- for purchase of motor vehicle Executive and below levels - upto Rs. 50,000/- for purchase of motor bicycle If you are on contract basis, you need to have a further 24 month contract Page 16

19 You need to have Original invoice from the vendor Two guarantors (internal) Guarantors should have completed 2 years of uninterrupted service with no outstanding loans to the company. Should not have signed as a guarantor to any other Salary or Vehicle Loan. If Guarantors are on contract basis, they need to have a 24 month further contract period. Interest of 1% per month will be charged for all 24 months How to get your vehicle loan? You make the request via HCM Upon successful verification, approval will be granted within 12 working days On approval, GHR will make the payment to you and recover through salary Notes: Recovery of loan at the time of resignation The Remuneration Unit recovers any outstanding loan amounts to be settled in full by the employee or If employee fails to settle at the time of resignation, to recover from guarantors or seek legal action to recover Page 17

20 Benefit No. 09 SALARY LOAN In the hour of need... You can obtain a salary loan upon completing 2 years of continuous service and have a clean disciplinary record. What you need to fulfill Have no more than 40% deductions of your pay (basic) Have no outstanding loans for the past 6 months If you are on contract basis, you need to have a further 6 or 12 months contract period based on installments You need to have Two guarantors (Internal) Guarantors should have completed 2 years of uninterrupted service with no outstanding loans to the company. Should not have signed as a guarantor to any other Salary or Vehicle Loan. If Guarantors are on contract basis, they need to have a 12 month further contract period Loan re-payment period - 06 months or 12 months If the loan recovery period is 06 months - NO interest will be charged If the loan recovery period is 12 months, 1% per month interest would be applicable from the 7th to the 12th month Page 18

21 How to get your salary loan? You make the request via HCM Upon successful verification, approval will be granted within 12 working days On approval, GHR will make the payment to you and will be recovered through your salary Notes: Recovery of loan at the time of resignation The Remuneration Unit will check and recover any outstanding loan amounts to be settled in full by the employee, at the time of resignation or If employee fails to settle the loan at the time of resignation The Remuneration Unit will recover from guarantors or seek legal action to recover Page 19

Special leave will be granted when you are getting married. 03 days leave, if you are entitled for annual leave 05 days leave, if you are not entitled for annual leave How to get your wedding leave? How to receive your gift? You inform your immediate supervisor/ghr of the good news GHR will present the gift to you You acknowledge the receipt of the gift Page 23

26 Benefit No. 13 WISHING YOU WELLNESS Have your outdoor medical expenses reimbursed! You are entitled for an OPD reimbursement scheme from the date you join. What you can claim Medical claims (including ambulance service, scans, tests, vaccinations, dental, spectacles) for self and/or immediate family member must be supported by prescription/s and original receipt/s from a registered medical practitioner. The bills should not be older than 30 days from the date of issue. However based on your date of joining, the entitlement would be prorated Definition of immediate family Married employees: self, spouse and child/children Unmarried employee: self and within limits for parents Page 24

27 Limits As per the policy on OPD, Staff Category OPD Entitlement Parents Entitlement per annum (LKR) per annum (Only for single employees) Staff (Joined before ) 80,000/- 20,000/- New Recruits (Joined after ) 50,000/- 15,000/- Dental 8,000/- 2,000/- per annum Spectacles 10,000/- 2,500/- per annum New Recruit s 1st year entitlement will be pro-rated as per below. Pro-rated % for OPD entitlements Joined between 01st January and 31st March(Q1) 100% Joined between 01st April and 30th June(Q2) 75% Joined between 01st July and 30th September(Q3) 50% Joined between 01st October and 31st December(Q4) 25% Page 25

28 How you can apply? You apply for the outpatient reimbursement via HCM and hand over the claim to GHR supported by the following documents: Printout of the HCM submission Hard copy of the prescription (original or certified copy mandatory) Orginal payment receipts Upon receiving the documents by GHR, you will be notified with a received alert Verification of reimbursement documentation by GHR if required You must promptly clarify / act on queries if any Once approved by GHR, reimbursable amount will be remitted to the employee s bank account. 29 Benefit No. 14 INSURANCE COVERS We have got you covered Insurance covers for Hospitalisation, General Personal Accidents (GPA) and Critical Illness & Life Cover Hospitalisation You will be provided with a Hospitalisation Insurance cover from the date of joining If you are married, yourself, spouse & children will be covered (unmarried, unemployed children below 18 years of age) If you are unmarried, you will be entitled to a personal cover GPA A 24 hours accidental cover will be provided to you from the date you join. Critical Illness Insurance cover for 24 critical illnesses stipulated by insurer, will be provided from the date of joining Life Cover This insurance cover will be provided from the date of joining. However this will be activated at the demise of an employee. Page 27

30 How to obtain your hospitalisation cover? Upon renewing the insurer every year, you will be informed with the name of the insurance company & hotline number In the event of hospitalisation (except for the sole purpose of medical testing) which includes general personal accident and critical illness, you need to inform the admissions desk of the hospital / nursing home and insurer via the insurance hotline Cashless route On admission, you should inform the insurer via the insurance hotline and obtain a reference number On being discharged from the hospital, you inform the insurer via hotline, in order to settle bills Costs incurred outside of the stipulated limits, must be borne by you Reimbursement route When being discharged from the hospital, you should inform the insurer via the insurance hotline On being discharged from the hospital, you must obtain a doctor s endorsement on the Hospitalisation Insurance Claim Form, pay the final bill and obtain the diagnosis card to be handed over to the Insurance Unit, along with the duly completed hospitalisation insurance claim form, for reimbursement purpose within 10 working days You will be informed to collect the reimbursement payment by cheque from Insurance Unit/GHR Page 28

31 How to apply for GPA? Promptly inform GHR of incident Upon treatment, you need to send the duly completed Insurance claim form along with the diagnosis card and the medical certificates How to obtain your critical & life cover? Promptly inform GHR & Insurance Unit of incident Page 29

32 Benefit No. 15 LEAVE & WORK HOURS Paid leave on medical grounds Take rest for a speedy recovery If you meet with an accident / get injured while at work or advised by medical consultants to take bed rest in the following instances: Post operation, complicated pregnancy /confinement, nursing for broken arm/leg/hand/back etc, critical illness covered under the Critical Illness Procedure How you can apply? You or your immediate supervisor informs GHR in writing of the reason along with relevant medical documents within 7 working days Leave setoff order 1. Medical Leave 2. Annual Leave 3. Two months special paid leave 4. One month 50% special paid leave 5. No pay leave upto a maximum of 06 months including annual, and special paid leave granted GHR informs you/supervisor Page 30

33 Benefit No. 15 LEAVE & WORK HOURS Long term no pay leave Take some time off 1 year Fulltime studies relevant to the present job Max 3 months Travel overseas on personal grounds What you need to fulfill 03 years continuous service Minimum of 04 or 05 ranks at Annual Performance Review for the past two years No history of no pay leave (not more than 30 days) for the past 02 years No disciplinary records for the past two years No objection from the supervisor and divisional head How you can apply? You inform HRP GHR checks eligibility You will be informed on the GHR Approval/Rejection Page 31

34 Benefit No. 15 FLEX HOURS OF WORK What you need to fulfill Authorised to report to work with 2 hours flexibility commencing at 7.00am and ending at 9.00am (minimum expected hours should be completed) Who can apply If your hours of work are not based on a shift or a roster* (*Employees whose nature of work requires the business hours of the company/division/arcade/branch/service centre etc) How you can apply? You send the request through the HRP with the divisional manager s approval HRP to validate and inform GHR System is updated and you will be informed Page 32

35 Benefit No. 16 CARING FOR YOUR CHILD S EDUCATION Supporting your child s education by providing Stationery Vouchers! A voucher worth Rs. 2,500/- for stationery expenses will be given annually to your child. What you need to fulfill You should serve the company for 2 years without interruption You should be in the category of Assistant Manager or below Children should be in the age group of 3-18 years How will you receive it? GHR will facilitate this benefit to all eligible employees Page 33

36 Benefit No. 17 ASSISTANCE AT A TIME OF LOSS We are with you and are ready to support you, in time of grief... A funeral endowment of Rs. 15,000/-, 02 days paid leave and a vehicle for one day will be facilitated on the demise of an immediate family member. How we will support? Unit Head, Divisional Secretary, Supervisor, HR Partner or you promptly informs in writing of the funeral to the HRP/GHR GHR / Divisional representative would handover cash within two working days, to you or nominated family member Employee acknowledges receipt of cash at their convenience Definition of immediate family Married employees: mother, father, spouse, children, mother-in-law and father-in-law Unmarried employees: mother, father, sister (unmarried) and brother (unmarried) Page 34
