1 Fall/Winter 2009 IESC to Spend $35 Million on Employee Medical Benefits in 2010 After three years of modest increases in medical expenses between 2006 and 2008, IESC experienced a 20% increase in medical costs in IESC spent approximately $7,000 per covered employee in 2009 compared to $5,814 per covered employee in And it is projected that IESC will spend approximately $7,400 per covered employee in That equates to an additional $2 million being spent by the company on health care in Because of the spike in health care costs in 2009, employee premiums will increase by a larger amount than in most years. However, it is important to note that IESC will still pay the same percentage of health care costs in 2010 as it did in 2008 about 76%. Medical claims have the biggest impact on health care costs and it is in all of our best interests to do everything we can to help control future claims to keep future premium increases as low as possible. Some of the ways you can help: Preventable, unhealthy lifestyle habits are a major contributor to high medical costs. Make the commitment to get or stay as healthy as possible. Be sure to take advantage of the many wellness resources that are available on the Blue Cross and Blue Shield website. Have an annual physical and take a completed health risk assessment with you to discuss with your doctor. Take the medications prescribed by your doctor for chronic conditions. Talk with the Blue Care Advisor nurses if they call you to discuss your health. Ask your doctor if there is a generic drug available when he or she writes you a prescription. Use primary care physicians when possible and avoid unnecessary trips to the emergency room. Ask your doctor how different test results might change your treatment plan. Doctors often order tests as defensive medicine when the actual results will not change how your are treated. Shop for the most cost competitive providers, especially for diagnostic tests. Dedicated nurses provide customized support On October 1, IESC, in cooperation with Blue Cross and Blue Shield of Texas (BCBSTX), launched a new, customized program for IESC, Rexel, and Gexpro employees and their covered dependents diagnosed with chronic health conditions. Blue Cross and Blue Shield has enhanced its Blue Care Connection program two experienced nurses now work exclusively with our company s employees, providing personal, confidential support to help manage chronic condition(s) and improve quality of life. These dedicated nurses can help you set achievable goals for managing your condition(s), develop an individualized plan of care, and help with coordinating care between your doctor, pharmacist, and other health care providers as needed. Nurses are also available to answer questions and provide general support as you work toward achieving your healthy best. If you are contacted by a BCBS Blue Care Connection nurse, please take advantage of this opportunity to receive free, customized, expert case management. Blue Care Connection services are available to those who have been diagnosed with a chronic condition such as: Asthma Coronary artery disease (CAD) Cancer Diabetes High blood pressure Or other serious, chronic conditions. Employees enroll for benefits online. The benefits enrollment website, Employee Central, is located at Note: In the first half of 2010, IESC will move to a new benefits enrollment site. Open enrollment period: November 16 - December 1, During the open enrollment period you may make changes to your elections as often as you like. Only your last saved elections at the close of open enrollment will be implemented. The company employs a passive benefits enrollment process. If you do not make any changes, your 2009 benefit elections will roll over in 2010 with the following exceptions: HSA: If you are enrolled in the Blue Edge HSA medical plan, employee deferrals to your health savings account (HSA) will reset to zero. FSA: Participation in flexible spending accounts (FSA) and deferrals will be canceled. You must actively enroll to elect either or both of these options. All employees are strongly encouraged to go online to review and verify elections even if you intend to keep the same elections and do not wish to enroll for HSA deferral or an FSA. It is especially important that those who wish to change or cancel coverage, and those who wish to enroll for an FSA, log in and make changes. After the open enrollment period, most benefit elections cannot be changed or canceled. To review, change, or elect benefits: Go to Employee Central at home.eease.com/login between 11/16/09 and 12/01/09. Enter your login, company identifier, and password in all lower case letters. Your Login is: rex plus your employee clock number. Your clock number is the last 5 digits of your employee ID number, found on your paystub. Your company identifier is: rexel Your default password is: The first 4 letters of your last name plus the last 4 digits of your social security number. If your last name has fewer than four letters, enter x s for the missing letters. Example: Don Ho; Login: rex98765; Company identifier: rexel; Password: hoxx1234 If you ve changed your password from the default at any time, enter that new password instead of the default. If you ve forgotten your changed password, click the forgot your password link on the login screen. If you need additional help, contact the Benefits Help Line at Click the Login button. If you know that you want to keep the same benefit elections you had in 2009 and do not wish to enroll for an employee HSA deferral or FSA participation, simply click the View/Print Benefits Statement and then the View my benefits on a specific date and enter 01/01/10. Print and review the summary carefully; save a copy for your records. If the statement is correct, your enrollment is complete. If you would to like to make changes or review all the election options, choose one of the three buttons at the bottom of the Benefits page: Walk me through this process, I know what changes I want to make, or Review my benefit coverage, and follow the on-screen instructions. When you finish making new elections or changes, you will see a Changes Submitted Successfully note. Click the View/Print Benefits Statement and then the View my benefits on a specific date and enter 01/01/10. Print and review the summary carefully; save a copy for your records. If the statement is correct, your enrollment is complete. Your printed summary is confirmation that your elections were properly entered and received.

3 Page 3 What s New for 2010? A snapshot of the benefit plan changes from 2009 Benefit Plan Sponsor The sponsor for all benefits plans (except 401k) is now IESC (instead of Rexel). Medical Blue Edge HSA: Higher HSA annual maximum contribution limits: $3,050 for employee only, $6,150 for employee + one, or $6,150 for employee + family. Lower HSA balance required for mutual fund investment option: $1,500. Deductible and out of pocket maximum increases for employee + one coverage (by federal law). Deductible: $2,400. Out-of-pocket maximum: $4,800. Employee Assistance Program (EAP) New administrator: ComPsych. Phone: Website: Company ID: GEN311. Life, AD&D, and Disability: New carrier: Prudential. Disability: Employees now files claims directly with Prudential. Optional life insurance: Maximum purchase allowance changed for employee, spouse, and child coverage. Higher annual increases allowed without evidence of insurability and carrier approval. ID Cards and Debit Cards Medical ID card: All employees who elect medical coverage will receive a new medical ID card in If you elect the same plan you had in 2009, your old ID card is also still valid. Vision ID card: Only new medical plan participants will receive a new vision plan ID card. A vision ID card is not required to access service. You may simply give the eye care provider your social security number. HSA debit card: Only new Blue Edge HSA participants will receive a new ACS Mellon HSA debit card. Your 2009 debit card is still valid. If needed, request new/additional cards from ACS Mellon. Dental ID card: You will receive a new dental plan ID card only if you are a new dental plan participant in An ID card is not necessary to receive benefits. Simply give the dentist your group number ( for IESC or Rexel, for Gexpro) and employee clock number or social security number. FSA debit card: You will receive a new debit card only if you are a new FSA participant in Your 2009 debit card is still valid. If needed, request new/additional cards from PayFlex. Travel assistance ID card: An AXA travel assistance brochure and ID card are included in your benefits packet. All medical plan participants are encouraged to complete an assessment. Understanding your health risks gives you the information you need to make changes that will help maintain or help improve your health status. The health risk assessment (HRA) is one of the many online health tools available at no cost or potential health issues. To enroll in Special Beginnings, call completing the HRA and participating in other healthy activities in the Personal Health Manager qualifies you for Blue Points which can be redeemed for health related products and services. After completing the registration process, log in. Access the Personal Health Manager from the My Health tab or click the Personal Health Manager icon on the home page. Extra benefit with Blue Edge HSA Employees who elect the Blue Edge HSA medical plan will receive an additional $200 per person ($400 maximum) lump sum deposit in their health savings account when eligible members over age 18 complete an HRA between January 1 and March 31, 2010 (or within 3 months of coverage start).

5 Page 5 Generic Drugs: Safe, Effective, Less Expensive Q: Are generic drugs as safe as brand-name drugs? A: Generic drugs are evaluated by the Food and Drug Administration and undergo the same scrutiny as brand-name drugs. A generic-equivalent drug must have the same active ingredients and must be chemically equivalent to the brandname drug. Only generic drugs that have the same clinical effects and safety profile as the brand-name drugs require substitution under BCBSTX plans. Q: Are generic drugs available for all prescription drugs? A: No. However, almost 50% of all prescriptions are now filled with generic equivalent drugs, so there s probably a generic drug available for your prescription. Check with your doctor if you are interested in receiving generic equivalent drugs. Q: Who do I contact if I have questions about my prescription drugs? A: You should discuss questions and concerns about drugs that you are taking with your physician. He or she can discuss whether a BCBSTX listed medication is appropriate for you. All employees and their dependents (18 years and older) who enroll for medical coverage are encouraged to complete a health risk assessment at after January 1, Employees who choose the Blue Edge HSA plan will receive an additional HSA contribution of $200 per person ($400 maximum) for health risk assessments completed between 01/01/10 and 03/31/10 (or within 3 months of coverage start date for new employees). Q: How do I turn off the automatic payment of medical expenses from my HSA? A: To turn off the automatic payment option, just contact ACS Mellon at You can then use your HSA debit card or HSA checks to pay your share of costs directly. Q: Can I change my HSA employee deferral during the year? A: Yes. You can start, stop, or change your HSA deferral amount at any time. Just complete a HSA deferral change form. Q: Can I keep my ex-wife on our benefits plan? May I enroll my new girlfriend and her children for benefits? A: No. Ex-spouses or other individuals to whom you are not married under applicable state law, and children who are not your legal dependents, are not eligible for benefit coverage. Q: Is our medical plan valid if I get sick in another country? A: Yes, Blue Cross and Blue Shield members can access healthcare providers around the world. Always carry your current medical ID card with you when you travel. If you need to locate a doctor or hospital or need medical assistance services, call the provider locator number on the back of your ID card, They will help you find a doctor or hospital. Q: How do I know how much doctor s visits and treatments are going to cost? I want to make sure I contribute enough to my HSA. A: Doctor s office visit prices vary not only by the doctor s speciality but by the area you live in, your age, your gender, whether you are a new or existing patient, the complexity of the visit, etc. Medical procedures often have the same variables. Log on to the Blue Access for Members website at iesc. Go to My Health > Tools. Click the Treatment Cost Advisor button. There, you can get a range of prices for various conditions, tests, doctor visits, prescriptions, and procedures, based on your demographics. Q: What happens to my life insurance policy if I leave the company? A: Upon leaving employment with the company, you may convert your term life coverage to an individual whole life policy with Prudential. You also have options to convert or port your optional life insurance coverage. Applications must be received by the insurance carrier, Prudential, within 31 days of leaving the company. Q: Do we have a travel assistance program for medical emergencies? A: Yes. A travel assistance program is provided free to all full-time employees and their dependents. If you re traveling 100 or more miles away from home and have an unexpected medical emergency that requires medical transportation, call on AXA Assistance, 24/7. Services include emergency evacuation, critical care monitoring, medically supervised repatriation, dispatch of prescription medication, transportation to join a patient, care for minor children, return of mortal remains, emergency cash or bail assistance, and others. A travel assistance brochure with cut-out ID card is included in your benefits packet. Q: How long can I keep my adult child covered on my benefits plan? A: Eligibility ends at age 19 unless the dependent is attending high school or an accredited higher education institution full-time, or is approved as a disabled dependent. If your 19- to 25-year-old dependent is a full-time student, you must submit a certification form and required documentation each semester in order to maintain benefits coverage. Failure to return the form and required documentation by the deadline will result in cancellation of dependent coverage. Dependent coverage ends at age 25 regardless of student status unless the dependent is disabled. Q: How do I know which medical plan will be best for me? A: Since it s impossible to know what health expenses might arise in any given year, it would be smart to choose the plan that offers the lowest total costs for almost every employee, whether they are single or have a family, whether they have low, medium, or high medical expenses. Q: Is the Blue Edge HSA a good plan even if I have kids who go to the doctor a lot? A: The Blue Edge plan is a good plan, even if you have kids! Check out some of the examples on the next page. 7 Myths& Misconceptions Get the facts about your medical benefits Page 7 Don t Weight: The BCBS Weight Management Program can provide the information and motivation you need to achieve your weight loss goals. Would you like to slim down and stay younger looking? Do you want to reduce your risk of cardiovascular disease, cancer, stroke and arthritis? No matter what your age or fitness level, you can start a diet and exercise program to become healthier and more fit. The Weight Management Program (in the Personal Health Manager section) of the BCBS website is an online selfmanagement program designed with the individual in mind. The program approaches the difficult issue of weight management through engaging, personalized, and motivating secure messages that will be delivered to your Personal Health Manager "New Messages" inbox. You will receive education about weight management, healthy eating, being more active, setting goals, as well as targeted information in response to your self-reported assessments. The Weight Management program emphasizes: Helping you understand a healthy weight, healthy eating habits, and advice on being active Setting goals Taking action, and Providing ongoing motivation. Myth: High deductible health plans like the Blue Edge HSA plan are only for the young, healthy, or wealthy. Fact: While it is commonly reported in main-stream media that high deductible health plans are only for the young, healthy, or wealthy, the IESC high deductible plan has been intentionally designed to be the least expensive of the three medical plan options regardless of age or health condition. The media rarely takes into consideration factors like the difference in employee premiums or the amount companies contribute to an employee s health savings account. IESC supports the Blue Edge HSA plan with extremely low employee premiums and generous contributions to HSAs because we are convinced that the best way to control long term health care costs is for health care consumers to be actively engaged in their health care and to know how their health care dollars are being spent. Studies have shown that participants in high deductible health plans are more likely to practice preventive health, shop for the most cost effective treatment options, and question their doctors about the real need for expensive tests. Myth: The Blue Premium plan is the best plan for people with serious health problems. Fact: The difference in premiums between the Blue Edge HSA and Blue Premium plans deposited into your HSA, along with IESC s contributions, will completely fund your account for the year. That means you would have enough funds available in your HSA to not only pay your deductible, but all of your out-of-pocket expenses up to your out-of-pocket maximum. For less than the annual cost of employee premiums for the Blue Premium plan, you would have enough money in your account to pay ALL of your out-of-pocket medical expenses for the entire year. And, whatever money you don t spend from your HSA is yours to keep in a tax-free account, building year after year. On the other hand, you don t get any of your premiums back and there is no limit on doctor co-pays in the Blue Premium PPO plan. Myth: The Blue Standard PPO and Blue Premium PPO plans offer a larger selection and access to better doctors than the Blue Edge HSA plan. Fact: All three plans use the exact same Blue Cross and Blue Shield preferred provider organization (PPO) network. You have access to the same doctors and hospitals regardless of the plan you choose. Myth: The Blue Standard PPO plan is the safest choice because it offers a convenient co-pay plan with mediumpriced premiums. Fact: The Blue Standard PPO s small co-pays are convenient but they can quickly add up, and they don t count toward your deductible or out-ofpocket maximum. Plus the money you spend in higher premiums could be put to better use by funding your own Blue Edge HSA health savings account. Myth: Employees premiums and out-of-pocket expenses pay for all medical expenses. Fact: Employees premiums and outof-pocket expenses cover only about 24% of the total cost of the company s medical claims, while IESC pays about 76% of the cost. Last year, IESC spent more than $33 million for employee s medical benefits.
