3 BICYCLE FLOATER 1. PURPOSE: To provide broad form coverage on bicycles owned and operated by individuals. 2. COVERAGE: All risks of loss or damage to the described property except as excluded. 3. MAJOR EXCLUSIONS: A. Wear and tear, gradual deterioration, rust, inherent vice, mechanical breakdown, or malfunction; B. Loss or damage by any process or while the property is actually being worked upon and resulting therefrom, unless caused by fire or explosion. 4. TERRITORIAL LIMITS: The coverage is worldwide. 5. SPECIAL CONDITIONS: A. No loss or damage covered by this insurance is collectible hereunder unless such loss or damage exceeds $5.00; (Note: In reference to the above, this is not the usual deductible clause. Any claim for loss or damage exceeding $5.00 will be paid in full.) B. Any theft of property insured that may become a claim hereunder shall be reported as soon as possible to the police. 6. RATES: The annual premium per $ of insurance is $8.50. CAMERAS AND PROJECTION MACHINES 1. PURPOSE: To provide coverage for cameras, projection machines, films, and related articles of equipment. Also, coverage is provided for equipment pertaining to the recording, projection, reproduction, and operation of motion pictures, and property such as films, binoculars, telescopes and microscopes which may be used in conjunction with photographic equipment. Coverage is not provided for television cameras or equipment, video cameras or equipment, coin or token-operated devices, or property of dealers or manufacturers. 2. COVERAGE: All risks of loss or damage to the described property except as excluded. 3. MAJOR EXCLUSIONS: Loss or damage caused by wear and tear, gradual deterioration, insects, vermin or inherent vice. 4. TERRITORIAL LIMITS: The coverage is worldwide. 5. SPECIAL CONDITIONS: When non-commercial rates are being charged, coverage is void if any of the equipment covered is used for compensation of more than $ a year. PURPOSE: To provide coverage for camper units. 2. ELIGIBLE TYPES: Two-wheeled trailer with tent or metal assembly, detachable camper unit for use with a pickup truck, collapsible camper unit for use with a station wagon or car, and other camping trailers for which a license is not required by the Motor Vehicle Department, State of Wisconsin. 3. INELIGIBLE TYPES: Any trailer over 27 feet long; built-on camper (non-detachable) units or vantype camper units; and mobile homes (house trailers) used primarily as a permanent or seasonal dwelling. 4. COVERAGE* - PERILS INSURED AGAINST: A. Fire, lightning, explosion and smoke; B. Cyclone, tornado, windstorm and hail; C. Collision, derailment or overturning of transporting conveyance; D. Collapse of bridges; E. Riot or civil commotion; F. Vandalism and malicious mischief; G. Theft; H. 5 I. Collision, upset or overturn of the insured property. * COVERAGE: A $50.00 deductible applies to all losses. 5. MAJOR EXCLUSIONS: A. Loss or damage caused by wear and tear, gradual deterioration, insects, vermin or inherent vice; B. Infidelity of persons entrusted with the property; C. Loss caused by artificially generated electrical currents. 6. TERRITORIAL LIMITS: This insurance covers property within the limits of the Continental United States and Canada. 7. RATES: The annual premium per $ of insurance is $2.00. COMPUTER PROPERTY ENDORSEMENT 1. PURPOSE: To provide coverage for computers that are primarily used in the home for personal use, but are also occasionally used for business purposes. Coverage is provided for the basic unit and also for data and media which can be readily commercially purchased. 2. COVERAGE: All risk of physical loss or damage unless specifically excluded. 3. SCHEDULE REQUIREMENTS: We will provide blanket coverage for all equipment, data, and media up to $2,000. Coverage must be written on a scheduled basis if the total values exceed this amount. When written on a scheduled basis, we require a complete listing of hardware and software including make, model, serial number, and value. 4. WHERE COVERAGE APPLIES: Property is covered while at the insured location or insured premises as described in the policy. The property is also covered while temporarily away from this location or premise provided that the insured owns the property. 5. IMPORTANT: As the endorsement provides replacement cost coverage, the insured is required to maintain coverage equal to 80 percent (80%) of replacement cost. If the amount of coverage is inadequate at the time of loss, there will be a proportionate reduction on recovery. 6. PURPOSE: To provide coverage for paintings, etchings, pictures, tapestries, and other bonafide works of art (such as valuable statuary, marbles, bronzes, antique furniture, rare books, antique silver, manuscripts, porcelains, rare glass, and bric-a-brac) of rarity, historical value, or artistic merit. 2. COVERAGE: All risks of loss or damage to the described property except as excluded. 3. MAJOR EXCLUSIONS: A. Loss or damage occasioned by wear and tear, gradual deterioration, insects, vermin, inherent vice or damage sustained due to and resulting from any repairing, restoration or retouching process; B. Breakage of art glass windows, statuary, marbles, glassware, bric-a-brac, porcelains, and similar fragile articles, unless caused by fire, lightning, aircraft, theft and/or attempted theft, cyclone, tornado, windstorm, earthquake, flood, explosion, malicious damage or collision, derailment, overturn, or conveyance, unless endorsed hereon. 4. TERRITORIAL LIMITS AND STATEMENT OF LOCATION: This insurance covers the property within the limits of the continental United States, Hawaii, and Canada. We do not insure loss, from any cause, to property on exhibition at fair grounds or premises of national or international expositions, unless those premises are specifically covered by the policy. 5. ELIGIBLE PROPERTY: Private collections owned by individuals. Refer to Company if private collection is owned by any firm, corporation, association, university college or school. 6. INELIGIBLE PROPERTY: Property owned by dealers, commercial risks, museums, art galleries or art institutions. 7. COVERAGE: All risks of loss or damage to the described property except as excluded. 3. MAJOR EXCLUSIONS: A. Loss or damage resulting from the use of the property for its intended purposes; B. Loss or damage caused by dampness or extremes of temperature; any refinishing, renovating or repairing process, mechanical breakdown, rust, fouling, explosion, marring, scratching, tearing or denting; C. Any dishonest act by an insured, insured s employee(s), or persons entrusted with the property. 4. Rate and minimum premiums depend upon the coverage afforded, type of property, hazards, loss experience, etc. Examples of items eligible for coverage include: A. RATES: The annual premium per $ of insurance is as follows: Full Coverage $1.10 With $ Deductible $1.00 CLASS - PROFESSIONAL: Dance bands, dance orchestras, etc. COVERAGE: Limited Form - $50.00 deductible, perils of fire, lightning, cyclone, tornado, flood, theft or accident to a conveying vehicle. COVERAGE: All risks of loss or damage to the described property except as excluded. 3. MAJOR EXCLUSIONS: A. Loss or damage resulting from the use of the property for its intended purpose; B. Loss or damage caused by dampness or extremes of temperature, any refinishing, renovating or repairing process, mechanical breakdown, rust, fouling, explosion, marring, scratching,tearing, or denting; C. Any dishonest act by an insured, insured s employee(s) or persons entrusted with the property. 4. PURPOSE: To provide coverage for individuals who have a valuable collection of stamps or rare and current coins for hobby or private collection purposes. 2. COVERAGE: All risks of loss or damage to the described property except as excluded. 3. MAJOR EXCLUSIONS: Refer to form. 4. SCHEDULE REQUIREMENTS: We will provide $1, insurance on stamps and $1, on coins on a blanket basis (as per categories below). If value of either category exceeds $1,000.00, schedule must be furnished. A. On postage stamps including due, envelope, official revenue, match and medicine stamps, covers, locals, reprints, essays, proofs and other philatelic property owned by or in custody or control of the insured, including the books, pages and mountings thereof; B. On rare and current coins, medals, paper money bank notes, tokens of money and other numismatic property owned by or in custody or control of the insured, including coin albums, containers, frames, cards, and display cabinets in use with such collection. 5. TERRITORIAL LIMITS: The coverage is worldwide. 6. SPECIAL CONDITIONS: When a coin collection is covered on a blanket basis, we pay the cash market value at time of loss, but not more than $1,000.00, for the entire collection or $ for any single item in the collection. 7. RATES: The annual premium per $ of insurance is $1.25 for full coverage. 8. INFORMATION NECESSARY FOR WRITING: B. Location where property is kept; C. AMOUNT OF INSURANCE: As the policy provides coverage for a blanket amount of insurance, the policy contains a 100% coinsurance clause requiring that the insurance amount equals the full value of the property covered. It is not necessary to furnish a schedule of the items to be insured. 6. RATES: The annual premium per $ of insurance is as follows: Full Coverage $2.50 With $ Deductible $2.25 With $ Deductible $ UNDERWRITING AND SELLING: This form was designed for a limited amount of personally owned tools (approximately $2,500.00).
