4 Scope of Work Schedule Objectives This Request for Proposal is issued by UHN for the purpose of soliciting Proposals from interested Proponents for the provision of a General Contractor for the Cardiac Upgrades Project at the Toronto General Hospital and the Toronto Western Hospital (the Project ). UHN is seeking to satisfy the following objectives in issuing the RFP: (a) (b) (c) (d) to identify a supplier for the supply and delivery of Deliverables as further detailed in this RFP; achieve the best solution for the needs of UHN; obtain quality Deliverables at best overall value; and enhance customer satisfaction and improvements in Deliverables. Delivery locations Toronto General Hospital (TGH), 200 Elizabeth Street, 2nd floor -2 Eaton Toronto Western Hospital (TWH), 399 Bathurst St 5th floor, MacLauglin and East Wing Scope of Deliverables, Implementation Plan and Project Timelines Please refer to the Scope of Deliverables Summary and Appendices Schedule Project Timeline The following timelines are based on the current information. 5 Response Schedule Stage I Mandatory Requirements Declaration TO: University Health Network Attention: RFP Administrator REFERENCE NO: for General Contractor for Cathlab Upgrades Project, Peter Munk Cardiac Centre, University Health Network, Toronto ( RFP ) I am duly authorized by the Proponent, including the persons, firms, corporations and advisors joining in the submission of this Proposal, to execute this Declaration. I declare and certify as follows: 1. Proponent Information (a) The full legal name of the Proponent is: (b) Any other registered business name under which the Proponent carries on business is: (c) The jurisdiction under which the Proponent is formed is: (d) The name, address, telephone, and address of the contact person for the Proponent: 2. Offer The Proponent has carefully examined the RFP documents and by examination of actual site conditions (if applicable) has a clear and comprehensive knowledge of what is required under the RFP. By submitting its Proposal, the Proponent agrees and consents to the terms, conditions and RFP SCHEDULES - GC for Cathlab Upgrades Project - 5 -
6 provisions of the RFP, and offers to provide the Deliverables in accordance with the rates set out in the Price Exhibit. 3. Pricing The Proponent has submitted its pricing in accordance with the instructions in the RFP and in the form set out in the Price Exhibit and proposes to execute the Agreement in consideration for the prices and rates set forth in the Price Exhibit of this Proposal. 4. Addenda The Proponent is deemed to have read and accepted all Addenda issued by UHN prior to the Closing Time. The onus remains on the Proponent to make any necessary amendment to its Proposal based upon the Addenda. 5. Proposal Irrevocable The Proponent agrees that its Proposal shall be irrevocable for 180 Days following the Closing Time. 6. Disclosure of Information The Proponent agrees that any information provided in this Proposal, even if it is identified as being supplied in confidence, may be disclosed where required by law or if required by order of a court or tribunal. The Proponent hereby consents to the disclosure, on a confidential basis, of its Proposal as provided in the RFP. 7. Proof of Insurance By signing this Declaration statement, the Proponent agrees, if selected, that it has verified its capability to do so and will provide proof of insurance coverage as set out in the Form of Agreement Schedule. 8. Consortium and Subcontractors Where a consortium is responding to this RFP, the Proponent shall be the sole Proponent and the Proponent shall assume full responsibility and liability for the work and actions of all consortium members with respect to the obligations to be assumed pursuant to this RFP, provided that UHN shall be entitled to reject a proposed subcontractor. Proponents are to provide information for all areas of the table below. Execution of Agreement If its Proposal is selected by UHN, the Proponent agrees to finalize and execute the Agreement based on the form set out in the Form of Agreement Schedule in accordance with the terms of the RFP. 10. Unfair Advantage and Conflict of Interest Prior to completing this Declaration statement the Proponent is advised to review the definitions of Unfair Advantage and Conflict of Interest set out in Article 6 (Definitions and Interpretation) of the RFP. In the event that the boxes below are left blank, the Proponent shall be deemed to declare that (a) it has had no Unfair Advantage in preparing its Proposal and (b) there is no foreseeable actual or potential Conflict of Interest in performing the contractual obligations contemplated in the RFP. If either or both of the statements below apply, check the appropriate box: The Proponent declares that there is an actual or potential Unfair Advantage relating to the preparation of its Proposal. The Proponent declares that there is an actual or potential Conflict of Interest in performing the contractual obligations contemplated in the RFP. In the event the Proponent declares an actual or potential Unfair Advantage and/or an actual or potential Conflict of Interest (by marking either of the boxes above), the Proponent shall provide all relevant detailed information below. The Proponent agrees to provide any additional information which may be requested by the RFP Administrator, in the form prescribed by the RFP Administrator. Where, in its sole discretion, UHN concludes that an Unfair Advantage and/or Conflict of Interest arises, it may, in addition to any other remedy available to it at law or in equity, disqualify the Proponent s Proposal, or terminate any Agreement awarded to the Proponent under the RFP. 9 Stage II General Requirements Proponents are free either to populate responses in the blank boxes provided below or use suitable alternate written formats with proper sequence and numbering as appropriate. Use designated response forms wherever instructed. A. Company Profile and Project Team Composition (14%) A.1 Company Profile: Description of the Company s overall experience and qualifications in supplying the Deliverables contemplated under this RFP. Include a brief company background, track record of services in the field relevant to the scope of the RFP and a mention of notable accomplishments (Max 1-2 pages) 2 % A.2 Assigned Project Personnel: Provide Resumes of the Project Lead and key individuals, including mechanical and electrical resources, proposed for the Project. Identify specific team leads that have significant experience in the general contracting function of Projects of similar size and scope. Demonstrate how their experience in the projects and functions similar to those envisaged in the RFP bring value to the Project. Evaluation will be based on education, professional qualifications, professional development, and direct experience in relevant / similar projects. 10 % A.3 Project Organization Chart: Provide a Project Organization Chart, including sub-trade, which clearly indicates assigned personnel s specific role and responsibilities in the Project. 2 % RFP SCHEDULES - GC for Cathlab Upgrades Project - 9 -

Related Experience (18%) B.1 Related Experience: Provide a list and description of minimum 3 (three) multiphased projects completed over the past 7 years, starting with the most recent, with a scope and magnitude which compares to or exceeds the scope of the RFP. Where possible, please provide projects that demonstrate work conducted in an operating hospital environment. Provide past examples, including specific mechanical and electrical details, which faced similar phasing challenges and what was done to successfully implement all aspects of the project, while maintaining working/public spaces. 2 % B.3 Understanding of Infection Prevention and Control (IPAC): How does your company support the IPAC requirements through current policies, practices and training programs? Please mention if the proposed Project Team, particularly the Project Manager and/or the Site Superintendent have completed formal IPAC training. 2 % Demonstrate your understanding of and compliance to the IPAC requirements specific to this project; Highlight some of the key IPAC considerations pertaining to this project and explain how you will address them RFP SCHEDULES - GC for Cathlab Upgrades Project
11 B.3 In the context of the Cathlab Upgrades Project and based on your team s experience and expertise presented in the RFP, highlight key factors that will make your firm the ideal candidate to deliver this project successfully. 2 % C. Methodology, Approach and Schedule Exhibit (18%) C.1 Methodology & Approach: Describe approach to initiation, planning and execution of the Project. 14 % Include steps involved in project progression, identified with owner s timeline requirements for project implementation. Include critical path, showing task dependencies, timeline requirements for Project progression and all major activities and milestones along with Gantt chart. This section shows attention to details, justifiable milestones and aggressiveness of schedule in line with the owner s suggested timelines. Include specific details of temporary requirements for sequencing and phasing of the works and specific mechanical and electrical tasks. 12 C.3 Risk Mitigation Plan: Based on the understanding of the Scope and Delivery Model, please identify potential risks or impediments which the Project may encounter.
