1 SPECIFICATIONS AND BID FORM FOR TRASH REMOVAL HOWARD G. SACKETT TECHNICAL CENTER The Jefferson-Lewis-Hamilton-Herkimer-Oneida Board of Cooperative Educational Services is seeking quotations for trash removal at its Howard G. Sackett Technical Center, Route 12, Glenfield. All items disposed of in the dumpsters become the responsibility of the trash removal contractor, as specified under this specification. The Contractor must provide Dumpsters/Containers with a minimum capacity and service as specified: 1. One (1) front load, 6 cubic yards, hinged-top, non-compacting container. 2. Service: 2 times per week during the school year and 1 time per week during summer. 3. One (1) 10 cubic yards roll-off covered recycling container with pickup up to six times per year. 4. Service: BOCES will call when recycling container needs pickup not to exceed six times per fiscal year. 5. Containers must be placed at sites as specified by owner. 6. Weekly trash removal date must be mutually agreed to by owner and service provider. The containers must be rodent resistant. All containers must be rinsed and sanitized during each lift. Each container must be painted and in good condition. If the container becomes damaged, the contractor must repair or replace upon knowledge of such condition. 7. Quotation Period The period of time specified for this quotation is July 1 st through June 30 th. 8. Insurance Contractor must provide insurance related to any aspect of trash removal including Dumpster, Contractor Employees and related Equipment. The minimum coverage shall be $1,000,000 Combined Limit. Certificate of Insurance to be provided to the BOCES, Office of the Assistant Superintendent for Business, before award of the contract. 9. The BOCES will pay Contractor monthly, billed on the first of the following month after service. 10. Contracts must provide a quotation for each additional lift not covered under this contract. 11. The final disposal site for all solid waste generated by the BOCES shall be the Development Authority of the North Country Solid Waste Management Facility located in the Town of Rodman. 12. After the opening of the bids, no bidder shall be allowed to withdraw a bid unless the bid involved a mistake

Bohlen, Jr. Technical Center on Arsenal Street, Watertown. All items disposed of in the dumpsters become the responsibility of the trash removal contractor, as specified under this specification. The Contractor must provide Dumpsters/Containers with a minimum capacity and service as specified: 1. Five (5) front load, 6 cubic yards, hinged-top, non-compacting containers; or Ten (10), 3 cubic yard, hingedtop, non-compacting containers for the twelve month period. 2. Service: 1 time per week or per agreement with the Senior Custodian and Assistant Superintendent for Business. (If more than once per week, fewer dumpsters will be needed.) 3. One (1) 10 cubic yards roll-off covered recycling container for pickups as called up to twelve times per year or equivalent as agreed to by BOCES. 4. One (1) 30 cubic yards construction material container with pickup up to four times per year. 5. Service: BOCES will call when construction material container or recycling container needs pickup. 6. 4 REQUESTS FOR QUOTATIONS The Jefferson-Lewis BOCES, Watertown, New York, hereinafter known as the Owner will receive Three (3) SEPARATE quotations at the office of the Assistant Superintendent for Business, Michele Traynor, Jefferson- Lewis BOCES, State Route 3, Watertown, NY until 10:00 a.m. on Thursday, June 4, 2015 for: Trash Removal Charles H. Bohlen Jr. Technical Center Watertown and Trash Removal Howard G. Sackett Technical Center Glenfield and Trash Removal Boak Educational Center Lowville for the period of July 1, 2015 through June 30, 2016, at which time and place the quotations will be opened and read aloud. Any quotation may be withdrawn prior to the above scheduled time for the opening of quotations or authorized postponement thereof. Any quotation received after the time and date specified shall not be considered. All quotations shall continue firm and effective for sixty (60) days from opening. Quotations must be submitted on the prescribed form. All blank spaces must be filled in, in ink or by typewriter. AWARD The award will be made to the responsible bidder that can serve in the best interest of the BOCES. The Owner reserves the right to waive any informality in, or reject any or all quotes, or to accept any proposal of any removal company which in the opinion of the Owner, will be in the best interest of the BOCES.

General Bid Certification The bidder certifies that he will furnish, at the prices herein quoted, the materials, equipment, and/or services as proposed on this bid. II. Non-Collusive Bidding Certification By submission of this bid proposal, the bidder certifies that he is complying with Section 103-d of the General Municipal Law as follows: Statement of non-collusion in bids and proposals to political subdivision of the state.
