Rates are based upon classes and types of service in each exchange. 3. Service is provided through facilities owned and maintained by the Telephone Company in accordance with the established standards. The territory served is shown on the maps filed with the Commission. The territory is referenced to as an exchange. Within the exchange is the base rate area and/or zones. 4. The rates specified for local exchange access service entitle customers to an unlimited number of messages to all access lines bearing the designation of central offices within the local service area. 5. The application of business or residence rates is determined by the actual or obvious use made of the service by the customer. Where only one local exchange access line is provided at a location which is both residence and business in character, the business rate will apply. GENERAL (Continued) The rate for local exchange access lines does not include a telephone instrument or inside wire. No more than one customer will be served by an access line in all of the Exchanges served by the Company. Subsequent to January 31, 1983, all inside wire installed by the Company in all exchanges shall be provided on a time and material basis as specified in Schedule 2 of this Tariff. All inside wire installed since January 1, 1983 is the property of the customers who are responsible for its maintenance and repair. Additional customer-provided instruments may be attached to exchange access lines. The Company reserves the right to limit the number of instruments connected to an access line if they cause interference with the normal operation of the line. One party business rates set fourth in B. following apply subject to the Commission s General Order 54 or as otherwise ordered by the Commission. Optional features for fraud protection are specified in Schedule 7 following. The method used to provide Call Forward With Hunt may vary from central office to central office due to the facilities available. Call Forward With Hunting is used by a customer with more that one line in order to route an incoming call to an available (non-busy) line. If a line is busy, this service will hunt for an idle line in order to complete the call. Call Forward With Hunt must be on each line arranged in a hunt group.

"Communication impaired" is defined as hearing impaired or speech impaired individuals as described by the Americans With Disabilities Act of 1990, Title IV, Section FUNDING This program shall be funded through a recurring monthly special fee on residence and business access lines not to exceed more than one hundred access lines per account. Except as may be specified elsewhere in this Tariff, Nonrecurring Service Charges, as specified in C. following, apply to customerinitiated requests for the establishment of service. 2. Unless otherwise noted, Nonrecurring Service Charges apply in addition to, and not in place of, special construction charges, mileage charges or installation charges applicable in connection with services provided elsewhere in this Tariff. 3. Nonrecurring Service Charges to install service are applied individually according to the components of work required. 4. RATES AND CONDITIONS 1. The Telephone Assistance Program (TAP) provides for a discount of the recurring monthly rate for residence customers who meet eligibility requirements established by Senate Enrolled Act No. 45, enacted by the Wyoming legislature in To be considered eligible, the applicant must receive Medicaid benefits under Title XIX of the Social Security Act. Subscription to expanded services will only be allowed at the principal residence of those recipient physically handicapped subscribers certified by the Wyoming Department of Health and Social Services. The "Telephone Assistance (T) Program" is a joint State and Federal program pursuant to 47 C.F.R. Subpart E, 54. (C) 2. The discount will be applied to the following local exchange service Offerings: Individual flat rate residential service. Individual measured rate residential service. Service station residential service. Access to emergency services Access to operator services Access to interexchange services, unless toll blocking is chosen Access to directory assistance Toll Blocking In no case will the discount exceed the rate charged for the grade of residential service subscribed to by each individual. 3. RATES AND CONDITIONS (Continued) The total cost of providing the Telephone Assistance Program shall be funded from a uniform monthly surcharge of $.01 to each residential access line and each business access line, not to exceed one hundred (100) lines per customer. Eligible residence subscribers will not be assessed the monthly $.01 surcharge. The TAP discount will begin with the date the Company receives a valid application from the customer or when new service is established for a qualifying customer. The discount will be prorated on the basis of a 30-day month from the customer s application. The service and equipment charges to change to or from this program due to eligibility status will be waived. The discount is applicable only to a single residence line at the principal residence of the eligible subscriber. TAP subscribers may subscribe to toll-blocking at no extra charge. Toll-blocking restricts access to the toll network and 900 service. Toll-blocked access lines are restricted from completing 0+, (J-,and 1+ calls. In addition, directory assistance calls may not be available in conjunction with toll-blocking. TAP customers who subscribe to toll-blocking will not be required to pay service deposits in order to initiate service. TAP subscribers will not be disconnected for non-payment of toll charges, regardless of whether toll blocking is activated on their service. Partial payments received from TAP subscribers will be first applied to local service and then to toll charges. TAP will not be furnished on a Foreign Exchange (FEX) basis. 11. Line extensions in excess of 2,500 feet will be made at the following charge: Non-Recurring Charpes None S&E Code Per each foot $ 1.00 XLES (The charge for a fraction of a foot will be rounded to the next higher foot,) 3. Real estate subdivisions: Extensions made to outside plant into tracts or sub divisions will be made on the basis of a special contract, based on actual costs, between the Company and the owners of the real estate or the subdivider thereof. CONDITIONS 1. Line extension charges are applicable in connection with all classes, types and grades of service when established by means of an extension to the Company s plant consisting of buried, or aerial construction. This includes extensions, by means of poles to be owned solely by the Company or jointly with others and by means of contacts or contact space on poles of others. 2. All line extensions will be owned and maintained by the Company. The applicant, however, if he so elects, may provide such material or perform such labor in accordance with construction standards of the Company, in lieu of the charges applicable, but in all instances the ownership of facilities shall be entirely vested in the Company. 3. Distances mentioned in this schedule are route specific distances. The routing of line extensions will be determined by the Company and the easement rights the Company is able to obtain. Construction charges are for construction other than normal in order to provide telephone service. These non-recurring charges apply under certain conditions, as hereinafter set forth, to cover all or a portion of the costs involved in the establishment of service and are in addition to the rate for the class of service furnished any any service connection charges, mileage charges or other charges that may apply in accordance with this Tariff. The application of construction charges rests solely with the Telephone Company. 2. The Telephone Company will furnish, install and maintain all outside plant necessary to serve its customers, except as otherwise provided in this Tariff. 3. Except as otherwise provided herein, the rules and regulations in this Schedule contemplate usual construction, i.e., the type of construction the Telephone Company would provide for the area and for the quantity and class of service involved if the decision rested solely with it, or where required by law. 4. When a charge is applicable for construction, the customer may undertake, where in the opinion of the Telephone Company it is practicable for him to do so, such construction in whole or in part, in lieu of the construction charges which apply to that portion. In all cases of construction by the customer, the material furnished and the method of construction are subject to the approval of the Telephone Company. If underground conduit construction is used to the property line of the subscriber by requirement of law or at subscriber s request, the subscriber will be required to furnish, install and maintain the conduit on his property in accordance with the Telephone Company s specification. If direct burial construction is used to the property line of the subscriber by requirement of law or at subscriber s request, the subscriber will be required to excavate and backfill the trench on his property, or be charged the actual cost for such work done by the Telephone Company. The subscriber shall replace the 1awn, shrubbery, pavement, sidewalks or other items damaged in the process of construction or maintenance of his property. 6. The Telephone Company is not liable for any defacement of or damage to the subscriber s premises resulting from furnishing of facilities, or from the installation or removal thereof when such defacement or damage is not the result of the negligence of the Telephone Company or its agents, 7. When it is necessary to relocate buried wire or cable or underground conduit and cable at the subscriber s request, the subscriber will be charged the actual labor and material cost. Where underground construction will not be within a utility strip or other designated right-of-way and where the Telephone Company requires adequate rights for the construction, operation and maintenance of such construction, the subscriber, or tract owner or developer in the case of real estate subdivision, will provide the Telephone Company with easements, deed restrictions or other appropriate covenants for these rights. 9. Except as otherwise provided herein, any outside plant facility provided at the expense of the customer is the property of the Telephone Company, is maintained and replaced by the Telephone Company, and shall not be used by the customer for any purpose other than service furnished by the Telephone Company. 10. Deposits or advance payments covering construction charges may be required at the time the application for service is made and are based on the estimated cost of the construction required. For construction other than normal, the customer will pay the actual cost above normal cost in accordance with the conditions above. Normal construction includes all distribution facilities, aerial and underground, placed at the option and initiative of the Company. 2. For changing from one type of construction to another on the customer s property, when done at the customer s request or when required by law, the customer will pay the costs of constructing the new and removing the old construction. 3. For moving existing or when required by construction, when done at the law, the customer will pay the customer s request actual cost, 4. General Provisions a. Customer-provided terminal equipment and communications systems may be connected with the facilities furnished by the Telephone Company for use with individual line service in compliance with Part 68 of the F.C.C. Rules. b. Where telephone service is used in connection with customerprovided terminal equipment or communications systems, the operating characteristics of such equipment or systems shall be such as not to interfere with any of the service offerings by the Telephone Company. Satisfactory performance of the telephone network requires continuing functional compatibility of the network control signals and the switching equipment involved. c. Such use is subject to the further provisions that the customerprovided equipment or systems do not endanger the safety of Telephone Company employees or the public; damage, require change in or alteration of, the equipment or other facilities of the company; interfere with the proper functioning of such equipment or facilities, impair the operation of the telephone network or otherwise injure the public in its use of the Telephone Company s services. d. Upon notice from the Telephone Company that the customer-provided equipment or system is causing or is likely to cause such hazard or interference, the customer shall make such change as shall be necessary to remove or prevent such hazard or interference. Customers shall be notified in advance wherever possible of their responsibility for the payment of all Telephone Company charges for visits by the Telephone Company to the customer s premises where a service difficulty or trouble report results from customerprovided equipment or facilities. The customer will be charged for visits by TeJephone Company employees to the customer's premises where a service difficulty or trouble ( report results from customer-provided equipment. Customer premises inside wiring (CPIW) is telephone wiring located on the customer s premises beginning at the point of connection of premises wiring to the Telephone Company s facilitates and terminating at the point of connection of terminal equipment. CPIW may be installed by either the Telephone Company or the customer. When inside wire is installed by the Telephone Company, it will be provided on a detariffed time and material basis. b. The connection to Telephone Company facilities will be at a network interface provided by the Telephone Company. Terminal equipment will be connected to the inside wire at each location of use via a customer-provided modular jack or similar device. FCC registered/approved equipment must be used. c. Inside wire initially installed by the Telephone Company may be used by the customer after contact with, and notification by, the Telephone Company determines that the Telephone Company itself has no other planned use for the wiring. Use of unapproved or altered equipment can result in discontinuance of service and a service charge for any and all costs for harm to the network. d. Maintenance of customer-provided inside wire is the responsibility of the customer unless other arrangements are made. The Telephone Company may offer either a monthly maintenance plan or offer to do work requested by the customer on a time and material basis. e.
