No. 1 previously filed with the Indiana Utility Regulatory Commission and established concurrence with this Tariff I.U.R.C. No. T-6. (N) Effective December 1, 2012, CenturyTel of Odon, Inc. d/b/a CenturyLink withdrew its Tariff I.U.R.C. No. 2 previously filed with the Indiana Utility Regulatory Commission and established concurrence with this Tariff I.U.R.C. No. T-6. By this notice, CenturyTel of Central Indiana, Inc. d/b/a CenturyLink and CenturyTel of Odon, Inc. d/b/a CenturyLink hereby adopt and ratify all supplements or amendments to this I.U.R.C. No. T-6 which have heretofore been filed or adopted by, and which may be subsequently filed or adopted by United Telephone Company of Indiana, Inc. d/b/a CenturyLink. (N) Effective July 28, 2009, United Telephone Company of Indiana, Inc. registered the fictitious name CenturyLink. Effective October 19, 2009, United Telephone Company of Indiana, Inc. d/b/a Embarq, began operating under the name CenturyLink. 12 of Indiana, Inc. d/b/a CenturyLink Section 1 (T) First Revised Sheet 1 Cancels Original Sheet 1 EXPLANATION OF TERMS EFFECTIVE: December 1, 2012 ACCESS LINE This term denotes the line between the central office equipment and the customer's premises, up to and including protection equipment. ACCESSORIES The term denotes devices which are mechanically attached to, or used with, the communicating devices furnished by the Company and which are independent of, and not connected to the conductors in the communications part of the telecommunications system. ADVANCE PAYMENT An amount usually in lieu of a deposit that the customer is required to pay to the Company before service is established. (N) (N) AIRLINE MILEAGE The standard form of measurement for computing mileage - the direct or airline distance point-to-point. AUTHORIZED USER A person, (other than the customer) on whose premise a telephone, is located and who may communicate over such channels in accordance with the terms of this tariff. BASE RATE AREA That portion of the exchange area in which exchange service is furnished at scheduled rates for each class of service without extra zone or exchange line mileage charges. CENTRAL OFFICE A switching unit, in a system which provides telecommunications services to the general public, having the necessary equipment and operating arrangements for terminating and interconnecting customer lines and trunks or trunks only. There may be more than one central office in a building or exchange. CENTRAL OFFICE AREA A geographical sub-division prescribed by the Company within which service is provided from a specific central office or offices. CENTRAL OFFICE LINE See Individual Line CIRCUIT The term applied to a channel used for the transmission of electrical energy in furnishing telephone service. (N) (N) CLASS OF SERVICE A designation given exchange service dependent upon the nature of its use such as business or residence services. COMMUNICATING DEVICE A device consisting of a transmitter, receiver, network control signaling unit, and associated apparatus and so connected as to permit the sending and receiving of telecommunications messages through the exchange and long distance network.

13 of Indiana, Inc. d/b/a CenturyLink Section 1 (T) First Revised Sheet 2 Cancels Original Sheet 2 EXPLANATION OF TERMS EFFECTIVE: December 1, 2012 CONNECTING ARRANGEMENT Equipment provided by the Company to accomplish the direct electrical connection of customerprovided facilities with the facilities of the Company. (N) (N) CONNECTING COMPANY A firm operating one or more telephone central offices with which traffic is interchanged by this Company at any point. CONTINUOUS PROPERTY A continuous plot of ground occupied by the customer which is not separated by a public thoroughfare. CUSTOMER A person, firm, partnership, corporation, municipality, cooperative organization, governmental agency, etc., receiving service from the Company. CUSTOMER LINE The circuit or channel used to connect the customer termination with the central office equipment. CUSTOMER PREMISES WIRING Customer premises wiring is defined as that wire, including connectors, blocks and jacks, on a customer's premises that extends between the termination of the exchange access line and those standard jack locations on the customer's premises to which terminal equipment can be connected for access to the exchange access line. CUSTOMER-PROVIDED TERMINAL EQUIPMENT The term denotes devices, apparatus and their associated wiring, provided by a customer, which are used with the network control signaling unit, data set or other station equipment furnished by the Company and does not include customer-provided communications systems. DIRECT CURRENT SUPPLY Electrical energy for talking and signaling purposes, other than ringing. DIRECT ELECTRICAL CONNECTION A physical connection of the electrical conductors in the communications path.

14 of Indiana, Inc. d/b/a CenturyLink Section 1 (T) First Revised Sheet 3 Cancels Original Sheet 3 EXPLANATION OF TERMS EFFECTIVE: December 1, 2012 EXCHANGE A basic unit of Company investment for the administration of telecommunication service in a specified area which may embrace a city, town or village and its environs. It may consist of one (1) or more central offices together with the associated plant used in furnishing telecommunication services with that area. EXCHANGE AREA A territory included within the boundaries of an exchange. EXCHANGE SERVICE This term refers to any and all of the various classes of service connected to the Company's central office switching apparatus. (D) (D) EXTENDED AREA SERVICE (a.k.a. Local Service Area) A service arrangement whereby customers may place calls to other specified exchanges without incurring message toll charges. (T) FACILITIES The physical plant necessary in connection with providing service. FLAT RATE SERVICE Service furnished at a fixed monthly rate. INDIVIDUAL LINE SERVICE A classification of exchange service which provides that only one (1) access line may be served by a central office circuit (not a PBX trunk line). (T) (D) (D) INSTALLATION CHARGE A non-recurring charge made for the placing or furnishing of telephone equipment, in addition to Service Connection Charges and other applicable charges for service or equipment. (N) (N) JACK A fixed socket designed to permit the establishment of a connection between terminal equipment with cords ending in plugs and the local exchange facilities.

EFFECTIVE: December 1, 2012 (D) LOCAL SERVICE AREA-LOCAL CALLING AREA The area within which telephone service is furnished customers under a specific schedule of exchange rates and without toll charges. A local service area may include one (1) or more exchange areas or portions of exchange areas. MAIN STATION (a.k.a. MAIN STATION TERMINATION) Also referred to as "Primary Station", this is the first telephone or termination on an individual access line connected to the switching apparatus in one of the Company's central offices. (D) (T) MESSAGE A completed customer telephone call. MESSAGE TOLL SERVICE The completion of calls via the toll network of this Company and/or Connecting Companies. NETWORK CONTROL SIGNALING The term denotes the transmission of signals used in the telephone company's ex-change facilities which perform functions such as supervision (control, status, and charging signals), address signaling (dialing), calling and called number identification, audible tone signals (call progress signals indicating reorder or busy conditions, alerting, coin denominations, coin collect and coin return tones) to control the operation of switching machines in the telecommunications system. NETWORK CONTROL SIGNALING UNIT The term "Network Control Signaling Unit" denotes the terminal equipment furnished for the provision of network control signaling.

16 of Indiana, Inc. d/b/a CenturyLink Section 1 (T) First Revised Sheet 5 Cancels Original Sheet 5 EXPLANATION OF TERMS EFFECTIVE: December 1, 2012 NETWORK INTERFACE (DEMARCATION POINT) The point of interconnection between Company communications facilities and equipment, protective apparatus or wiring at a customer's premises. The network interface shall be located on the customer's side of the Company's protector or the equivalent thereof. NETWORK INTERFACE DEVICE A device which readily permits the disconnection of all customer premises wiring from the Company network and provides access to the Company network through an industry registered jack as required under Part 68 of the F.C.C. Rules and Regulations. PAYPHONE LINE SERVICE (a.k.a. Payphone Service) Includes lines to which coin, coinless, card reader or a combination of coin/car reader telephones may be attached. (N) (N) PLUG A contact member on the end of an electrical cord which terminates the cord conductors. It can be inserted into a fixed jack, connector, or receptacle to make temporary connections with the conductors they terminate. PREMISES The property occupied by the customer or authorized user, either under lease or ownership. PREMISES WIRING - SYSTEM Wiring associated with private branch exchange and key telephone equipment where three or more central office access lines are used in association with the interior wiring between associated equipment. These systems are deregulated as determined in Part 68 of the F.C.C. Rules and Regulations. The types of system premises wiring are: (1) Fully protected system premises wiring. (2) Protected system premises wiring requiring acceptance testing for imbalance. (3) Unprotected system premises wiring. PRIMARY STATION Refer to "Main Station Termination."

17 of Indiana, Inc. d/b/a Embarq Section 1 Private Right-of-Way EXPLANATION OF TERMS A facility route granted to the Company on or over private property. Registered Protective Circuitry Original Sheet 6 Date: 02/20/08 Separate, identifiable and discrete electrical circuitry designed to protect the telephone network from harm, which is registered in accordance with Part 68 of the F.C.C. Rules and Regulations. Registered Terminal Equipment Terminal equipment which is registered in accordance with Part 68 of the F.C.C. Rules and Regulations. Residence Service Exchange Service furnished to customers where the actual or obvious use is for domestic purposes. Rural Zone Service Area That area within the exchange service area which lies beyond the base rate area. Same Building A structure on one foundation or structures on separate foundations with abutting walls and a direct access between structures through such walls. Same Premises All buildings on continuous property. In the case of a multi-office building, that portion occupied by the same customer on the same or separate contiguous floors of the building are considered same premises. Service Point The term when used in connection with customer-provided communication channels denotes the point on the customer's premises where such channels or facilities are terminated in switching equipment used for communications with stations or customer-provided terminal equipment located on the premises. Serving Central Office The Central Office from which a customer or authorized user would normally be served for exchange telephone service. Standard Exchange Service This term refers to normal telephone service within an exchange without regard to class of service. Standard Jacks Data or voice jacks as required under Part 68 of the F.C.C. Rules and Regulations for all connections of registered terminal equipment to the telephone network.

18 of Indiana, Inc. d/b/a Embarq Section 1 Station EXPLANATION OF TERMS Original Sheet 7 Date: 02/20/08 A telephone instrument, consisting of a transmitter, receiver and associated apparatus, so connected as to permit sending and receiving telephone messages. It also denotes the network control signaling unit, data set or other equipment located at the customer's premises which enables the customer to establish the communications connections and to effect communications through such connections. A Company leased station is a communicating device owned and maintained by the Company and provided as a part of the telecommunications service function, and which is connected for exchange or toll service. A Company leased station may be a main, extension or private branch exchange station. Subscriber - See "Customer." Tariff The rates, charges, rules and regulations adopted and filed by the Company and approved by the Indiana Utility Regulatory Commission. Telecommunications Service The services offered by the Company accessing the exchange switching network, including, but not limited to those services covered by the General Tariff. Telephone Company A person, firm, partnership, cooperative organization, or corporation engaged in the business of furnishing telephone service to the public under the jurisdiction of the Indiana Utility Regulatory Commission. Toll Rate The initial period and over-time period charge for toll messages, based on the length of call and distance between the rate centers of the calling and called station. Toll Service See Message Toll Service. Touch-Tone Service A classification of exchange service furnished from central offices, whereby, calls are originated through the use of pushbuttons in lieu of a rotary dial. Zone Rate Area, Rural That portion of an exchange area located beyond the base rate area within which Individual Line Service may be offered at a flat rate without mileage charges.

19 of Indiana, Inc. d/b/a CenturyLink Section 2 (T) First Revised Sheet 1 Cancels Original Sheet 1 I. APPLICATION OF REGULATIONS GENERAL REGULATIONS EFFECTIVE: December 1, 2012 A. The following General Regulations are applicable in addition to other regulations, rates and charges specified in other sections of this Tariff as they may be revised, added to or supplemented by superseding sheets. B. The regulations set forth herein apply to service furnished within the State of Indiana by United Telephone Company of Indiana, Inc., hereinafter referred to as the Company, and its Concurring Carriers, subject to the jurisdiction of the Public Service Commission of Indiana, hereinafter referred to as the Commission. (T) (C) (C) C. Telephone companies are authorized to serve in well defined exchange areas as indicated on the territorial maps attached to the Certificates of Territorial Authority (CTA) for each exchange area as issued by the Commission. D. When services and facilities are provided in part by other companies, these regulations apply only to that portion the Company provides. E. The Company does not transmit messages, but offers the use of its facilities, where available, for communication between parties subject to the terms and conditions and at the rates specified throughout this Tariff. II. ESTABLISHMENT AND FURNISHING OF SERVICES A. Applications 1. Applications for initial or additional services may be made verbally or in writing. 2. The Company may refuse service to any applicant who is indebted to the Company for service previously rendered until satisfactory arrangements have been made for payment of such indebtedness. 3. Any change in rates or regulations prescribed by the Commission modifies the terms and conditions of contracts to the extent of such change. B. Advance Payments At the time of application for service, an applicant may be required to pay an amount equal to at least one month's rental plus all service and installation charges in addition to such special construction charges as are to be borne by the applicant. C. Deposits 1. The Company may require deposits in accordance with Rule 170 IAC Interest will be paid on each deposit held by the Company in accordance with Rule 170 IAC

All facilities necessary for the provision of a given service will be furnished and owned by the Company, except as may be provided elsewhere in this tariff, and, upon termination of service, all Company-owned facilities shall be returned to the Company in good condition, reasonable wear and tear expected. a) The customer may be required to provide suitable housing or other protective measures where facilities are to be installed in locations exposed to weather or other hazards. The Company is not required to install its facilities where in its judgment the facilities may be subject to damage or destruction. b) The Company reserves the right to install the size and type of facilities which in its judgment, are necessary for the customer's present and estimated requirements. 2. One Company-provided line testing device may be connected to the line at the discretion of the Company. 3. Customer-provided terminal equipment may be used and customer-provided communications systems may be connected with the facilities furnished by the Company for telecommunications services as provided in this section. 4. Except as otherwise provided in this Tariff, nothing herein shall be construed to permit the use of a recording device, or of a device to interconnect any line or channel of the Company with any other communication line or channel of the Company or of any other person. 5. Use of Company facilities or service in connection with automatic announcement service, automatic answering and recording service, recorder-coupler service or miscellaneous devices for recorded public announcements are subject to the following conditions: a) For purposes of identification, subscribers to telephone service who transmit recorded public announcements over facilities provided by the Company must include in the recorded message the name of the organization or individual responsible for the service and the address at which those responsible for the transmitted recorded announcement may be contacted. b) Subscribers transmitting factual public announcements such as Time, Stock Market quotations, Airline schedules and similar information are excluded from the preceding condition. c) Non-published telephone service will not be furnished for use with recorded public announcements. d) Failure to comply with the provisions of this Tariff shall be cause for termination of the service.

Provision of Facilities (Continued) 6. Terminal equipment or protective circuitry will be directly connected in accordance with Part 68 of the F.C.C. Rules and Regulations which requires the installation of a standard plug and jack arrangement. While standard jacks are required by Part 68 of the F.C.C. Rules and Regulations, non-standard jacks may be used as a means of connection for grandfathered equipment when standard jacks are not available. E. Customer Billing 1. The minimum contract period is one month from the date of service or additions to service and the minimum charge is the established rate for one month, except as follows: a) Special contractual arrangements as specified elsewhere in this tariff or developed for special equipment and special assemblies of equipment. b) In lieu of required contract periods, a customer may arrange to pay special installation charges or termination charges. 2. The customer is responsible for all charges in conjunction with the service furnished him, including collect toll messages which have been accepted at his telephone. 3. Bills are due when rendered unless otherwise specified on the bill and may be paid at any business office of the Company or any agency authorized to receive such payments. They are delinquent on the eighteenth (18th) day of the billing period. Failure to receive a bill does not relieve the customer of the responsibility for payment. If payment is received after the bill becomes delinquent, a collection charge shall apply in accordance with the Company s deregulated local terms of service. (T) (T) 4. The Company may temporarily disconnect service in accordance with Rule 170 IAC Such disconnection shall not be made until at least seven (7) days from the postmark date of the written notification to the customer of the intention to suspend service. If the account remains unpaid five (5) days after disconnection, the Company may remove the lines. 5. The Company reserves the right to remove the telephone service of a delinquent account without the benefit of temporary disconnection, but not without benefit of proper notice, when previous treatment history indicates recurring disconnections for non-payments.

The use of the service may be extended to joint users, but may not be resold or otherwise used for transmitting, delivering or collecting charges for any message where any toll or other consideration has been or is to be paid to any party other than the Company except as provided in this Tariff or otherwise authorized by the Company in writing. 2. The Company reserves the right to refuse to install customer's service where the location of the instrument is such that the public in general or patrons of the customer may make use of the service. At such locations, however, customer's service may be installed provided the instrument is so located that it is not accessible to the public. 3. All operating of telephone equipment at the customer's premises must be performed at the expense of the customer and must conform with the rules and regulations which the Company may adopt to maintain a proper standard of service. 4. The Company reserves the right to limit the length of the conversation or message and to discontinue the connection when an emergency exists or when in the Company's judgment such action is necessary to assure equitable use of the facilities.

The Company may refuse to furnish or may terminate service and remove its facilities without request by the customer of the service and without prior notice under the following circumstances. a) Upon order made in writing by or on behalf of any court, the Commission or other authorized public authority, acting within its jurisdiction, on the grounds that such service is or is to be used for an illegal purpose. b) Upon the use of a service in such a manner that, in the opinion of the Company, constitutes abuse, fraud, unauthorized use of service or creates a condition that is dangerous or hazardous to life, physical safety or property. c) If the Company's equipment has been tampered with and the Company has reasonable grounds to believe that the affected customer is responsible for such tampering. d) If the Company's equipment is used in a manner disruptive to the service of other customers. e) In extraordinary circumstances where unlimited access to the toll network may result in substantial loss of revenue to the Company. In such cases, reasonable efforts should be undertaken to discuss such circum-stances with the customer prior to disconnection. 2. In all other instances, the Company may terminate service subject to the other provisions of this Tariff upon providing the customer with proper notice. Examples are: a) Upon failure to make suitable deposit as required by this Tariff. b) In the event the character of use is not in accordance with the class of service for which the customer originally ordered and he refuses to order or accept the proper service. c) Violation of Part 68 of the F.C.C. Rules and Regulations pertaining to connection of F.C.C. registered terminal equipment. Such violation may cause a temporary discontinuance of service.

The following conditions apply to termination of service not under specific contract, either in cases described in B.8.1 and B.8.2, above, or at the customer's request. a) The customer shall notify the Company at least three (3) days in advance of the day disconnection is desired. The customer shall remain responsible for all service used and the billing therefore until service is disconnected pursuant to such notice. b) Upon request by a customer to disconnect service, the Company shall disconnect the service within three (3) working days of the requested disconnection date. The customer shall not be liable for any service rendered to such address after the expiration of three (3) such days. c) The customer shall not charge service, or authorize the charging of service, to any account which has been disconnected. Paragraph B and B.8.4.2, above, to the contrary notwithstanding, a customer shall be responsible for any services he or she charges, or authorizes charged, to such account in violation of the prohibition in the foregoing sentence. I. Telephone Numbers 1. The customer has no property right to the telephone number nor any right to continuance of service through any particular central office. 2. The Company reserves the right to change the customer's telephone number or the central office associated with such number, or both, as may be required for the proper conduct of its business. 3. Where Automatic Announcement Services, Automatic Answering and Recording Services or Recorder Coupler Services are associated with local exchange telephone services, identities of customers and their addresses will be furnished to all persons requesting such information, provided that the inquiring parties specifically identify the telephone numbers or which customer's names and addresses are desired.

The Company's obligation to furnish service or to continue to furnish service is dependent on its ability to obtain, retain, and maintain without unreasonable expense, suitable facilities and rights, and to provide for the installation and testing of those facilities required incident to the furnishing and maintenance of that service. 2. The Company will determine the type of facilities to be provided for the furnishing of a service. 3. When facilities beyond those normally required are provided to satisfy customer requests, charges based on the additional costs incurred will apply. Examples include but are not limited to: a) When the Company has no other requirement for the facilities requested. b) When the Company provides service using a type of facility, or via a route, other than that which the Company would normally utilize in order to provide services for the customer. c) When the Company provides a greater quantity of facilities than that which the Company would otherwise provide. d) When it is requested that construction be expedited resulting in added cost to the Company. e) When the Company provides facilities which involve excessive or unreasonable costs (encountering natural or other barriers such as, but not limited to, lakes, rivers, rocky terrain, gas and oil fields, limited highways, bridges, dams or concrete/asphalt). (1) Generally, such charges are payable in advance. When at the option of the Company, the payment is deferred, a deposit may be required before construction of facilities begins. 4. When attachments are made to poles of other utility companies in place of new construction for which the customer(s) would be charged under the provisions of this Tariff, the rental charges to the Company of such attachments and the expense of obtaining the use of such poles may be charged to the customer(s) upon demand by the Company. The decision as to whether poles of other utility companies are suitable for the attachment of the Company's facilities rests with the Company.

Customer Premises Wiring a) General (1) Customer premises wire provided by the customer must be installed in accordance with the technical standards and installation guidelines furnished by the Company. (3) The "network interface or demarcation point," hereinafter referred to as a "network interface device," shall be defined as a device which readily permits the disconnection of all premises wiring from the Company network and provides access to the Company network through an industry registered jack of a type provided for in F.C.C. regulation Part 68, for testing purposes, and is provided as part of the exchange access line on all new installations and Company-initiated replacement programs. (a) (b) (c) All wiring on the customer's premises that is connected to the telephone network shall connect to the Company network through the network interface device or at any standard jack located elsewhere in the premises. Responsibility for the protector (and its associated grounding) on the network side of the demarcation point rests with the Company, though the customer is responsible for consequences resulting from erroneous wiring procedure conducted under his or her direction. The network interface device will be located on the customer's premises at a location determined by the Company which is readily accessible by the Company and the customer. The location of the network interface device shall be in close proximity to the Company point of entry. If the customer requests an alternative location from that determined by the Company, and there are additional costs to the Company to accommodate the customer's request, an appropriate charge will apply. (4) Applicable Service Charges apply where the customer desires the installation of a network interface device. (D)

The customer will save the Company harmless from any and all liability, claims, or damage suits arising out of the customer's wire maintenance activity. c) Responsibility of the Company (1) The Company will make technical standards and installation guidelines for customer provision of premises wiring available to customers at designated locations. (2) The Company will maintain the network interface device. d) Violation of Regulations (1) Where customer-owned premises wiring is in violation, the Company will promptly notify the customer of the violation and will take such immediate action as is necessary for the protection of the telecommunications network and Company employees. (2) The customer shall discontinue use of the customer-owned premises wiring or correct the violation and notify the Company in writing that the violation has been corrected within 10 days after receipt of such notice. (3) Failure of the customer to discontinue such use or to correct the violation will result in the suspension of the customer's service until such time as the customer complies with the provisions of this Tariff.

In case of damage, loss (including theft) or destruction of any of the Company's facilities not due either to ordinary wear and tear or to fire, storm or other like casualty, the customer shall be responsible for the value of the facilities lost or destroyed, or for the cost of restoring the facilities to their original condition, as the case may be. 2. Access to customer's premises will be given to representatives of the Company at any reasonable hour for the purpose of inspecting, repairing, testing or removing any part of the Company's facilities. C. Allowance for Interruptions In the event of an interruption to the service not due to the negligence or willful act of the customer, an allowance will be made for the interruption in accordance with Rule 170 IAC D. Liability The liability of the Company for damages arising out of mistakes, omissions, interruptions, delays or errors or defects in trans-mission occurring in the course of furnishing service and not caused by negligence of the customer shall in no event exceed an amount equivalent to the proportionate charge to the customer for the period of service during which such mistakes, omissions, interruptions, delays or errors or defects in transmission occurs.

The customer indemnifies and saves the Company harmless against the following: a) Acts or omissions of other companies when their facilities are used in conjunction with the Company s facilities to provide service. b) Any defacement or damage to the customers premises resulting from the existence of the Company s instruments, apparatus and associated wire on such premises, or from the installation or removal thereof, when such defacement or damage is not the result of the negligence of the Company or its employees. c) Any accident, injury, or death occasioned by its equipment or facilities, when such is not due to negligence of the Company. d) Claims for libel, slander, or infringement of copyright arising from the material transmitted or recorded over its facilities; claims for infringement of patents arising from combining with, or using in connection with, facilities of the Company, apparatus and systems of the customer; and against all other claims arising out of any act or omission of the customer in connection with facilities provided by the Company. e) Liability for failure to provide service. IV. UNIVERSAL SERVICE FEE A. Description Pursuant to the requirements of the Final Order in Cause No , all customer billings on and after October 2007 shall reflect an additional charge to fund the Indiana Universal Service Fund (IUSF). The purpose of the IUSF is to implement a competitively neutral funding mechanism that promotes universal telecommunication service to all Indiana residents by ensuring availability of basic telecommunications services at just, reasonable, and affordable rates that are reasonably comparable between urban and rural areas of Indiana.
