1 REQUEST FOR QUALIFICATIONS Project No for OSHPD CERTIFIED INSPECTOR OF RECORD SERVICES April, 2009 I. INTRODUCTION University of California, San Francisco Medical Center (UCSFMC) is soliciting Statements of Qualifications from qualified firms to provide OSHPD certified Inspector of Record (IOR) services for The UCSF Medical Center at Mission Bay Hospital Project to be constructed in calendar years 2010 through The selected firm would provide OSHPD certified IORs with class A licensing to furnish inspection services throughout the period of construction. It is expected that the services will begin in June 2009 and continue through December 2015, with an option for an additional two years. IOR Services fees will be negotiated prior to contract award. The University may, but is not obligated, to issue separate work authorizations under a Professional Services Agreement, for each phase as required. It is the policy of The Regents of the University of California that race, religion, sex, age, color, ethnicity, and national origin will not be used as criteria in its business contracting practices. Every effort will be made to ensure that all persons regardless of race, religion, sex, age, color, ethnicity, and national origin will not be used as criteria in its business contracting practices. GENERAL DESCRIPTION OF THE WORK: The proposed hospital site is approximately 14.5 acres, located just south of the existing UCSF facilities at Mission Bay. The site will be developed in multiple phases. Phase I scope includes a 289-bed Women s, Children s and Cancer hospital, an ambulatory care center, a central utility plant and associated Sitework. Phase I scope is referred to hereafter as PROJECT. The scope of PROJECT includes a hospital with approximately 650,000 GSF; an ACC Building with approximately 140,000 GSF and a Central Utility Plant with approximately 40,000 GSF. II. PROPOSED SCOPE OF SERVICES The University is seeking a firm to provide OSHPD Class A Inspector of Record (IOR) services for The UCSF Medical Center at Mission Bay Hospital Project. The estimated project cost is anticipated to be $630,000,000. The scope of services to be provided shall be in accordance with the latest California Code of Regulations Title 24 Part 1 Chapter 7 Administrative Code OSHPD requirements, applicable State Laws and Regulations, JACHO Accreditation requirements, Department of Public Health licensure requirements, and University policies and procedures. In addition the successful firm will be expected

2 to work cooperatively with licensed IORs or IORs in-training who are now currently working at the University. Basic IOR Services: The IOR will perform inspections of the hospital construction to ascertain that the work is consistent with the Contract Documents, is compliant with applicable State Building Codes, and meets requirements for Accreditation with JACHO and licensure with the California Department of Public Health. Additionally, an OSHPD certified class A IOR will assist the University during the design/assist phase of the project to aid in the identification of efficient and cost effective solutions to code compliance issues. III. CONTENT OF SUBMITTAL A. Cover Letter (2 page limit) summarizing the key points of the submittal. B. Respond to each item listed under the Selection Criteria, Attachment A. Submittal should be in softbound cover tabbed in sections for each selection criterion. C. Complete Statement of Qualification form, Attachment B. IV. SUBMITTAL AND REVIEW PROCESS A. 3 E. After a review of the submittals from all interested applicant firms, all firms will be ranked. At least three (3) of the highest ranked may be contacted for an interview or engaged in a discussion regarding anticipated concepts, relative utility of alternative methods of approach for furnishing required services, and other issues as appropriate. If the selected firm and the University are unable to reach a fee agreement, the University reserves the right to discuss proposals with the next qualified firm, and so forth, responding to this Qualification process. F. Final selection and appointment is contingent upon funding and budget approval. V. ATTACHMENTS Attachment A, Selection Criteria Attachment B, Statement of Qualifications Form Attachment C, Business References ATTACHMENT A SELECTION CRITERIA Applicant firms shall respond to each item below and will be evaluated on ALL of the following: 1. Capabilities of proposed inspection team. General information about your firm and the capabilities of each of the proposed team members as evaluated in terms of the functional, technical, and economic features of comparable OSHPD inspection services provided. (See items below for specific requirements). 2. Required Experience. Firm s experience in OSHPD certified Inspector of Record services for complex renovation/construction projects with major structural and concrete construction components in the State of California. Include in each project experience description, the start and completion dates, contract value, and Owner s contact person and phone number. Also, include in each project submitted, experience with all phases of construction, including: electrical, mechanical, plumbing, and fire life safety elements. 3. Personnel Assignments. Submit summary of experience for each proposed IOR. Include the following elements: description of each proposed IOR s experience as it relates to all aspects of construction/renovation of new or existing hospitals; knowledge of hospital building codes and regulations; ability to interpret all applicable governing regulations of OSHPD. Additionally, provide each IOR s experience with reviewing construction plans and specifications; scheduling and coordinating visits by OSHPD; preparing daily and, monthly compliance reports, experience with OSHPD Testing Inspection and Observation reports, preparing closeout documents, and experience with studying test reports to ensure compliance with construction documents. Provide copies of each IOR s OSHPD

4 Class A License, Professional License or other certifications. Note that in the event of staff separation or any such emergency occurrences, any proposed personnel substitutes would have to be acceptable to the University and approved in writing. 4. Program Responsiveness. Previous experience that demonstrates success in providing inspection services that were consistent with program schedule, budget, and technical requirements, and was responsive to the specifics of the program. Firms must demonstrate ability to commit minimum of four (4) IORs, one (1) Executive IOR, and any other key personnel that might be needed for the duration of the Project. Include a matrix indicating availability of each individual proposed as an IOR to meet project requirements. Firms shall describe how they plan to provide inspections so to maintain the the progress of the construction. The IORs are expected to work cooperatively with other IORs or IOR s in training that are currently working at the University. The Executive IOR will be required to assist the University, the Contractor, and the Design Professionals during the design/assist phase prior to the start of construction to assist in the identification of cost-effective solutions to code compliance issues. Firms should be able to meet program requirement for inspection services in the evenings, weekends and holidays if necesary. Additionally, firms must provide resumes for all proposed subconsultants, if any. 5. Client relationships. Recognition of the need to work harmoniously and productively with University Representatives, contractors, construction managers, design professionals and other customers is required. 6. Production Capability. Provide evidence of ability to perform inspection services for construction projects under OSHPD jurisdiction and produce inspection reports of superior quality and to meet the completion schedule of projects. 7. Major OSHPD Projects. Provide a list of major OSHPD approved projects inspected by your firm over the last 10 years. 8. Equal Opportunity. Show evidence of firm s equal employment opportunity policy. 9. Acceptance of Professional Services Agreement (PSA) Contract. Firm must be willing to accept the contract language, including indemnification and insurance requirements, and will execute the Professional Services Agreement (PSA) as written, if awarded the contract. Once contract is awarded, the PSA will only be modified as it relates to the scope of work. A copy of this agreement may be obtained from the UCOP website at: The respondent to the RFQ must attest that they have read the document and agree to its terms and conditions without any reservations or exceptions. Except for addition of the project-specific information, we do not expect to make any other modifications to the PSA. Please include your concurrence with this requirement in the cover letter.

5 *Insurance requirements: General Liability coverage: $1M each occurrence; $1M Products completed, Operations Aggregate; $1M Personal and Advertising Injury; $2M General Aggregate. Business Automobile Liability Insurance for owned, scheduled, nonowned, or hired automobiles, with a combined single limit of no less than $1,000,000 per accident. Professional Liability Insurance: $1M per claim and $1M in the aggregate. Firms that cannot satisfy the above minimum criteria will not be considered.
