2 REQUEST FOR BIDS Introduction: The Tift County School System invites qualified vendors (as later hereinafter defined) to submit a bid in accordance with the terms and conditions of this Request for Bids ( RFB ). Background Information: The Tift County School System is located in the City of Tifton, Tift County, Georgia, with its central office at 207 N. Ridge Avenue, Tifton, Georgia There are twelve (12) separate school sites within the Tift County School System. General Conditions: This RFB is not a contract offer. A bid submitted by a qualified vendor does not commit the Tift County School System to award a contract to any vendor. Project must not interfere with the basic operation of the school, unless otherwise outlined in this request. Vendor must notate in bid any variations or deviations from the specifications/basis of design. Bid must include all specifications, cut sheets, etc if submission is not the basis of design. Failure to meet qualifications and requirements may but will not necessarily subject a bid to disqualification. Delivery, setup, and training is required within 30 days of the Bid award or as approved by Craig Matthews, CTAE Director. Interpretation and Additional Information: Any interpretation, correction, or change of the RFB will be made by ADDENDUM. Interpretation, corrections, or changes to the RFB made in any other manner will not be binding, and the Contractor shall not rely upon such interpretations, corrections, or changes. Changes or corrections will be issued by TCSS. It is the responsibility of the Contractor to determine all addenda have been received. It is the responsibility of all respondents to contact TCSS prior to submitting a response to the RFB to ascertain if any addenda have been issued, and to obtain any and all addenda, execute them, and return addenda with the response to the RFB. (Any request for information must be submitted one week prior to the bid deadline.) Qualified Vendor: In order to be a vendor qualified to submit a bid in response to this RFB, a vendor must: (a) (b) Be an established provider (licensed/qualified to meet the needs of this RFB) Vendor must meet ALL codes that govern type of work requested.

5 (c) (d) (e) (f) (g) (h) Have and maintain, at minimum, general liability insurance in the amount of $200,000. A copy of your current Certificate of Liability must be submitted to TCS. You must notify TCS of a 30 day notice of cancellation and/or 10 days for non-payment of premium. You must list TCS as additionally insured on the policy. All contractors must have and maintain workers compensation insurance as required by law. In the event that the contractor is exempt from the worker s compensation laws, company official must sign here indicating so. SIGNATURE All contractors, sub-contractors and sub-sub-contractors must register and participate in the E-Verify program (if required) to be eligible for work with the school system. A notarized copy of the affidavit (attached) for all contractors must be provided with the bid/proposal to the school system prior to any contract of agreement being finalized. In the event that bidder will have employees on any school property, all employees must have a background check and properly vetted on file with the bidder and available upon request. Bidders must provide references, minimum of 3, of projects of the same size and scope. Additional business references may be requested. Any areas that have the potential to have suspected asbestos containing materials must be verified through the school systems AHERA asbestos management plan. Any areas that have the potential to contain lead based paint must be tested by the contractor. Any work that may disturb asbestos containing materials or lead based paint materials must be handled by a certified and licensed contractor. All laws, rules and regulations must be followed. A copy of the contractor/company license must be provided to the school system. (can be removed if necessary) Additional Information to be Included in Bid: In addition to the information to be provided in the bid, all bids submitted shall also include the following Information: (a) (b) (c) Name of vendor or firm submitting bid; Name of vendor s representative; Vendor s business address and telephone number. The Tift County School System reserves the right to reject any or all bids, to waive any informalities or technicalities or reject any bid from a bidder that is not responsible or bid that is not responsive. Any bid may be withdrawn prior to the scheduled time for the opening of bids. Any bid received after the time and date specified shall not be considered. Selection of Vendor : The lowest responsible responsive bid will be recommended to (as necessary) the Superintendent or to the BOE for approval at its next regular monthly meeting, or at such earlier special called meeting-the date, time and place of which will be duly posted and advertised prior thereto as required by Georgia law. Should the vendor recommended be approved, thereafter the BOE may require a written contract containing all necessary terms and conditions designed to effectuate the bid accepted, including but not limited to the provision of work and services (together with the timelines and quality thereof) to be provided by the vendor; payment for such work and services (and times thereof), by the BOE; and such other terms and conditions as required by the RFB or otherwise by Georgia and/or federal law. All required closeout documentation must be provided at the end of the project, prior to the final pay request. All cost must be included in the bid to include but not limited to all federal, state, and/or local fees, shipping and delivery costs, unloading, installation, assembly, cleanup and removal of debris, etc. The Tift County School System appreciates your time and interest in this RFB and in the preparation of your responsive bid, and for your interest in doing business with us.
