2 2 * Aetna Health Reimbursement Account (HRA) group health plan In 2013, more than half of the FPG associates covered on the Aetna FPG health plan had less than $500 in medical claims. That is good news! This year, FPG is offering a new health plan with a health reimbursement account with FPG commi ng up to $500 (or $1,000 for associates with covered dependents) in health claims reimbursement. What does this mean? With this new HRA plan, many Aetna FPG plan members will have no claims to pay this year except Rx copays. What if...i am not one of those who has less than $500 in claims? What if I have a large hospital bill? If I get a cri cal illness? What if I have an accident and get injured? This year we are offering a few other programs that can be used to fill the gap that so many health plans leave uncovered. * Trustmark Cancer/Cri cal Illness Plan This plan is Guaranteed Issue (no medical evalua on) and pays the owner of the policy up to $15,000 cash if diagnosed with a cri cal illness or cancer. This plan will pay a wellness benefit of $100 if you get the age appropriate cancer screenings, too. * APL Hospital Indemnity Plan This plan is also Guaranteed Issue and pays the owner of the policy $1,000 the first day of hospitaliza on to help offset out of pocket expenses. This plan will pay a wellness of $75, too. * Trustmark Accident Plan This plan is not new, but we want to remind our associates it pays the owner of the policy cash for injuries sustained in an accident to offset out of pocket expenses. This plan will also pay a $100 wellness benefit up to 2 mes per year. See Inside for More Details and Informa on on these new benefits

Thank you for your me today. Willis is honored to present your 2014 Benefit Op ons! The elec ons you make during open enrollment will become effec ve May 1, Family Physicians Group offers you and your eligible family members a comprehensive and valuable benefits program. We encourage you to take the me to educate yourself about your op ons and choose the best coverage for you and your family. Who is Eligible? Full me W-2 employees working at least 30 hours each week for at least 60 days. When can I Enroll? You may enroll for benefits when you first become eligible a er your wai ng period (1st day of the month following 60 days of full- me FPG employment) or each year during open enrollment in the month of April. The benefits you elect will be effec ve from May 1, 2014 through April 30, If you decline coverage or fail to make an elec on of a coverage during this open enrollment meframe, you may not be able to make a new or different elec on un l next year s open enrollment. How do I Enroll? Explain My Benefits will again be on-site this year, to guide each team member, individually, through the enrollment process. Just as last year, each team member will meet with a Benefit Counselor from Explain My Benefits individually on-site and make all elec ons/changes to your benefits on our online enrollment system. When you are scheduled to meet with a Benefits Counselor, if you are enrolling a spouse or dependent children, please don t forget to bring your dependent SS# and DOB.

4 Medical 4 Beginning May 1, 2014, Aetna will again be partnering with Family Physicians Group to provide our group medical plan. We want you to live a long, healthy and produc ve life! FPG is working with Aetna to ensure that all medical plan covered team members and covered dependents par cipate with our Wellness Program. For team members who complete the Healthy Steps, FPG is awarding reduced payroll deduc ons. Aetna and Trustmark also have cash incen ves built in when FPG associates follow the simple healthy behaviors and get their preven ve health exams (see page 12 for details). When performed by a physician in the Aetna health network, these exams are covered by our Aetna group medical plan at 100% with no copayment required and are important to the health of each one of us as well as the health of our medical plan. Look for these dollar signs throughout the benefit guide for various ways to save money! DEPENDENTS You may also elect coverage for your dependents in some circumstances. Eligible dependents may include the following: Your Legal Spouse Dependent Children: Dependent child who is supported primarily by you, or who is incapable of self-sustaining employment by reasons of mental or physical handicap (proof of their condi on and dependence must be submi ed) Medical - Dependent children up to age 26 regardless of financial dependency, residency, student status, employment or mar al status. Coverage terminates the end of the calendar year the dependent child turns 26. Individuals whose coverage ended, or who were denied coverage (or were not eligible for coverage), because the availability of dependent coverage of children ended before a ainment of age 26 are eligible to enroll in health insurance coverage. Individuals may request enrollment for such children within 30 days of receiving this handout. The coverage will be effec ve 1st of the month following the eligibility period. For more informa on contact FPG Benefits at Beyond the age of 26, the dependent must be unmarried and not have dependents of his or her own; be a resident of Florida or a Student, AND not have coverage as a named subscriber, insured, enrollee or covered person under any other group, blanket or franchise health insurance policy or individual health benefits plan, or is not en tled to benefits under Medicare. Coverage will be extended un l the 30th birthday.

5 Medical 5 FAMILY PHYSICIAN GROUP OFFICE VISIT CO-PAY WAIVER PROGRAM If you enroll in an FPG Medical plan, you will be en tled to all medical services provided by FPG, up to two (2) visits per month, as follows: For each FPG team member who is enrolled in the Aetna HNO plan, your co-pay will be waived for FPG provided services. For each FPG team member who is enrolled in the new HRA plan, FPG provided services will be at no cost to you. You can use one or both of these visits for any covered dependents on the FPG medical plan. HEALTHCARE TERMS Copay: A specific dollar amount that you must pay for a specific service at the me when you receive the service. Deduc ble: A dollar amount you are responsible for before the plan will make any benefit payments. Each year, your deduc ble starts over (January 1st). You are only responsible for sa sfying your deduc ble one me per year. Coinsurance: A method of cost-sharing between the member and the insurance carrier for your benefit expenses. If you have 30% coinsurance, then you pay 30% of your eligible expenses and the carrier pays the remaining 70%. The coinsurance begins a er your deduc ble has been sa sfied. Out of Pocket Maximum: The maximum amount you will be required to pay for your benefits, a er which the plan will pay 100% of covered expenses. Your deduc ble, coinsurance and in some instances copays apply towards your Out of Pocket Maximum. Health Reimbursement Account (HRA): An account or agreement between the health plan sponsor (FPG) and covered plan members. The agreement is a promise to pay an approved health plan claim. For FPG employees, the account value is up to $500 per covered employee and up to $1,000 per covered employee with eligible dependents, per plan year. The HRA can be used to cover in network Aetna approved health plan expenses. The HRA does not cover out of network expenses, wellness (covered by Aetna at 100%) or prescrip on drugs. For prescrip on drugs, plan members s ll have the Aetna prescrip on card and copayments. (see Medical benefit summary on page 7)

6 Medical Health Insurance Op ons for GET IN STEP with the FPG healthy steps, pay less and con nue along a path to improved health and wellness! There are new programs to help you this year. They are outlined in this booklet and in s you have been receiving. In addi on to the new plan booklets summarizing benefits for , meet with the Explain My Benefits enrollment counselors to help make an informed decision for Health insurance premiums keep going up and have increased for FPG has taken steps to offset this increase by offering some new products, addi onal choices, and highligh ng a new health and wellness plan from Aetna. The same health plan we had last year, Aetna HNO, is s ll available but at a higher premium. Our new, lower cost health plan is a consumer driven health plan from Aetna, called an HRA plan. What is the new HRA plan? FPG is commi ng a Health Reimbursement Account (HRA) of up to $500 for a single covered employee and up to $1,000 for an employee who is also covering eligible dependents to help cover medical expenses that fall under the deduc ble. If you choose the HRA plan, upon comple on of the required Wellness steps, FPG will deposit these funds into an account for you. You will be able to use these funds to pay for medical expenses such as office visits, specialists, etc. With the HRA plan, there is no copay except for prescrip on drugs (see benefit summary on page 7). All medical services like office visits, surgery, hospitaliza on, etc. will be subject to the deduc ble first. A er the deduc ble is met, the health plan will pay 70% of medical expenses un l out-of-pocket maximum is reached, then 100%. All preven ve services are covered at 100% on all health plan op ons. You MUST complete the 3 Healthy Steps by July 15, 2014 to receive the HRA deposit and the discounted premium. What are the Wellness steps? FPG encourages our associates as well as our pa ents to make be er choices and take be er care of their health. Taking personal steps to maintain or improve our health is not only the right thing to do, it is less expensive! Everyone has the right to decline to par cipate. If you do not complete these steps, you will not get the HRA money or the discounted premium. The 2014 Steps to Wellness are: 1. Biometric screening/fas ng lab work from Quest. For quick and eaxy service, use the Quest scheduling tool and make an early morning appointment. There are over 50 Quest loca ons between The Villages and Kissimmee. Your results are confiden al. Individual results are given only to you from Quest in the My 5 to Health report. 2. Complete the Aetna Online Risk Assessment. We did this last year; many FPG associates answered honestly and we learned a great deal about what health risks we have at FPG. Thank you! The results are confiden al (Aetna is not allowed to tell us your answers, only whether you did the survey or not). This is the same as last year and can be completed by logging into And, if you complete an online health challenge, Aetna offers a $50 gi card. 3. A end the Metabolic Health in Small Bytes coaching program. Details on how to enroll will follow. The goal for FPG employees, just like our FPG pa ents, is to raise awareness of Metabolic health and help everyone become healthier in 2014! Check out page 8 for more details. Please watch your very closely throughout April 2014, as we will be sending various communica ons regarding new programs and wellness features for plan year. 8 Medical Metabolic Health in Small Bytes A new and powerful interven on that targets metabolic syndrome risk factors Did you know? About 25% of US adults have metabolic syndrome (MetS). This means that one quarter of Americans are at risk for heart disease and diabetes. And the impact of metabolic syndrome on your health care costs is significant. In fact, it s a game changer! CRITICAL ILLNESS / CANCER PLAN Cri cal Illness/Cancer is a benefit that will pay you a lump sum of money if you are diagnosed with a cri cal illness, heart a ack, internal cancer or stroke. The cash benefit is provided upon the first diagnosis of a covered condi on to help you with associated costs and beyond. Special Underwri ng at Ini al Offering Guaranteed Issue: $15,000 employee / $7,500 spouse / $1,500 children Regardless of other coverage in force, the benefit is paid out in a full lump sum. The Hospital Indemnity Plan can help offset your out-of-pocket expenses including deduc ble, co-insurance and co-pays, and services not covered in your group health coverage for Hospitaliza on and Outpa ent surgery. On and off-the-job coverage (24/7) Money is paid directly to you for (please see brochure for a complete list of benefits): Ini al Doctor s Office Visit: $200 Fractures: up to $15,000 Hospitaliza on: $3,200 admission, $500 per day Disloca ons: up to $12,000 Wellness Benefit Included: A wellness benefit is paid for all rou ne physicals, vaccines, and health screening tests for each covered person. There is a 60-day wai ng period, a er ini al enrollment, for this benefit. This benefit pays $100 per test per person, twice each year (maximum of $200 annually per insured). 13 Voluntary Benefits Trustmark & APL 13 UNIVERSAL LIFE INSURANCE & LONG TERM CARE Universal Life with Long Term Care includes both a death benefit and a living benefit. Trustmark Universal Life with Long Term Care is a permanent life insurance policy that is designed to match your needs throughout your life me. It pays a higher death benefit during your working years when expenses are high and you need maximum protec on. The Universal Life with Long Term Care policy is priced to remain the same cost to you un l age 100. The death benefit reduces at age 70 when the need for life insurance typically decreases. The Living Benefit, Long Term Care, never reduces and is 4% of the original death benefit per month for up to 25 months. If you use the Long Term Care benefit, your death benefit amount does not reduce due to the Benefit Restora on feature included. Coverage available for spouse and children as well. Underwri ng: Guaranteed Issue offering for Newly Benefit Eligible Employees ONLY. No health ques ons for $3 - $12/week up to $200,000 If you waived this benefit previously, you must answer a few health ques ons and be approved for coverage. Life with Long Term Care example: $100,000 Death Benefit Long Term Care Benefit (LTC): Pays a monthly benefit equal to 4% of your death benefit for up to 25 months. Benefit Restora on: Restores the death benefit that is reduced to pay for LTC. 14 Term Life Insurance 14 As an employee, you may apply for Life Insurance for you and your eligible dependents. If you did not previously elect this benefit, Evidence of Insurability and approval by Mutual of Omaha will be required for any amount of coverage. Employee You may elect life insurance in increments of $10,000 not to exceed 5 mes your annual salary or a maximum of $500,000, whichever is less. If you elect Voluntary Life Insurance for yourself, you also have the opportunity to elect coverage for your spouse and/or children. Spouse You may elect life insurance in increments of $5,000 not to exceed the employee s elected amount up to a maximum of $250,000. Child(ren) Coverage in the amount of $10,000 can be elected for all children ages 14 days old through the age of 21 (25 if a full- me student and unmarried). The $10,000 benefit is a Guaranteed Issue for all children. Coverage terminates for dependents when the FPG Team Member reaches age 70.

16 Short Term Disability 16 As an employee of Family Physicians Group, you are able to enroll in Short Term Disability (STD) coverage. STD coverage supplements your lost wages should you be unable to work due to an illness, injury or pregnancy. STD coverage begins a er missing the specified elimina on period below due to a medically cer fied reason. Benefits are payable up to the specified benefit dura on period below. If you did not previously elect this benefit, Evidence of Insurability and approval by Mutual of Omaha will be required for any amount of coverage. Elimina on Period for sickness, accident or pregnancy: 14 days Maximum Benefit Period: 11 weeks for total or par al disability Weekly Benefit: Up to 60% of your weekly earnings to a maximum of $2,000 benefit Pre-Exis ng Condi on: Anything you received medical treatment, advice or consulta on, care or services including diagnos c measures, or had drugs or medicine prescribed or taken in the 3 months prior to your insurance effec ve date will not be covered for the first 6 months of the policy. BI-WEEKLY COST CALCULATION Example: A 36 year old female, Sally, wants to purchase short term disability and has an annual salary of $25,000. Weekly benefit: $288 (60% of weekly salary) $25,000 x = $7.99 Annual Salary Premium Factor Bi-Weekly Cost ($173,333 max)

17 Long Term Disability 17 As an employee of Family Physicians Group, your Employer pays for a Long Term Disability Benefit that is 40% of your monthly income.
