Total peace of mind A Bupa group member (covered for at least 6 months) is eligible to join our designated individual scheme upon his / her retirement or termination of employment Renewal of our individual scheme is guaranteed regardless of member's age or health status with no individual loading 2. Easy enrolment No health examination, no health declaration and no individual underwriting are required One subscription rate for members under the same plan All eligible children in the same family will be considered as one member for subscription calculation Simply complete and return the application form with member details and relevant subscription payment to us 10 working days before month end, the contract will take effect on the 1 st day of the following month 3. Superior services Direct claim reimbursement to employee s bank account Dedicated Customer Care helpdesk with 24-hour automated fax service via telephone Access to 24-hour online services via Bupa Active where a member can check claim status or membership details, search network doctors, and the scheme administrator can also submit members movements online Membership guide for each employee In an effort to best promote the wellbeing of our members, Bupa s commitment to members is incomparable

7 Major Exclusions Though you will be covered for most eventualities, there are certain things that Bupa will exclude. To enhance the transparency of our contract, some of the key things we cannot cover are listed below: 1. Illnesses or injuries that existed before the Coverage Commencement Date of Member; 2. Treatment or investigation which is not Medically Necessary; 3. Venereal disease, or AIDS or HIV-related conditions which originate during the first five years of membership; 4. Congenital, Developmental or Hereditary Conditions; 5. Senile Dementia (including Alzheimer s disease), Parkinson s disease, psychological or psychiatric condition(s) of any and all kinds, including but not limited to psychoses, neuroses, depression, anxiety, anorexia nervosa, schizophrenia, behavioural disorders, delirium, insomnia, neurasthenia; 6. Treatment related to pregnancy, infertility, sterilisation, sexual dysfunction; 7. Treatment arising from war or terrorism, or whilst serving in the armed forces, criminal acts or professional sports; 8. Cosmetic treatment, any treatment or investigation related to dental or gum conditions (unless Dental Benefit is available) except for Emergency treatment arising from Accidents or the extraction of impacted wisdom teeth during Hospital Confinement; 9. Rest cure, general check-ups, hearing or eye tests, or vaccinations, or alternative treatment (eg Chinese Medicines treatment, acupuncture, acupressure, Tui Nai, hypnotism, rolfing, massage therapy, aromatherapy, etc, unless Benefit is available); 10. Use of special braces or other equipment (eg wheel chairs, crutches, etc), blood or blood plasma; 11. Non-medical services (eg medical reports, telephone charges, etc); 12. Experimental or new medical technology (unless approved by Bupa). In the event of any discrepancy in respect of the meaning between the Chinese version and the English version, the English version shall prevail. All terms and conditions are subject to the Contract. 7
