1 Central Purchasing 213 South Oliver Drive Aztec, New Mexico (505) Proposal No EMPLOYEE BENEFITS PROGRAM VOLUNTARY SUPPLEMENTAL BENEFITS Human Resources ADDENDUM #1 March 27, 2018 CLARIFICATIONS / ADDITIONAL INFORMATION TO THE PROPOSAL SPECIFICATIONS AS FOLLOWS: CLARIFICATION Question: We noticed that one of the requirements of the RFP was for us to remove "actively at work" language from our proposal. It reads as follows: G. All proposals submitted must address the following and must provide answers to the following questions: 1. We request that all Actively At Work Language be deleted. Can you clarify and make sure that this is correct? We've been working in this industry for over 25 years and it is a mandatory requirement for all insurance companies in the voluntary benefits market for all products. In fact, the permanent life insurance (longterm care) plan that we want to propose only has one underwriting requirement; the employee applicant must be actively at work in order to get the coverage because the insurance is guaranteed issue. If the "actively at work" language must be deleted from our proposal; then I doubt if any insurance carrier can even submit a proposal for their products. This is a very important issue and needs to be addressed before the County can get even one quote for any of the products that you're requesting in the RFP. Maybe we don't understand the requirement and are confused. Can you please clarify? Response: An actively at work provision is included in most group insurance contracts San Juan County has asked for the provision to be waived; however, I can see where it would be prudent in a long-term plan to have people AAW when policy is written. The AAW requirement stipulates that coverage will be effective for an employee provided he/she is not absent from work due to sickness, injury, or other reasons on the date that coverage begins; if an employee happens to not be AAW (if it remains as a benefit) then the benefits offered by any company will need to be offered to the employee upon the employee s return. Furthermore, AAW will not be permitted for continuous enrollment in the plan since most individuals will need to utilize the plan because they were not AAW. Also, members will need to be considered AAW if they were at work immediately preceding: a weekend; a holiday; paid vacations; non- Page 1
2 scheduled work days; excused leave of absence. No previous confinement or non- AAW status will be held against the employee if currently and AAW. Benefits requested in the RFP are supplemental. We make no guarantee to the number of enrollees nor will we require employees to enroll. Our whole purpose is to give our employees options to fill in gaps. Question: Please confirm the commission levels for all requested lines of coverage. Response: We do not have commission level information. Question: Please confirm the current enforce rates for all requested lines of coverage. Response: Please see attachment D for current Supplemental Term Life and Short Term Disability rates. Question: It appears all, or some of these coverages are voluntary, please request an enrolled census confirming the current enrollments, and rerolled amounts, as well as the following: o Date of birth, gender, salary, occupations, locations (zip codes), class Response: A revised census is attached as Attachment A. Question: The RFP stated this case has 683 lives. If this is the true lives count, I will need the following experience: o Basic Life, and Voluntary Life: 3-5 years on carrier letter head Paid claims by product broken out by month or year Paid premium and lives history by product, broken out by month or year Waiver claims including DOB, date of disability, gender, volume Pending claims and/or conversions o LTD: 3 years of paid claims 3 years of paid premium 3 years of rate history and plan history Open claims listing to include: DOB, gender, Date of disability, Gross or net monthly benefit, total amount paid Closed claim listing with date of disability, and total paid claims o STD: 3 years of paid claims 3 years of paid premium 3 years of rate history and plan history Response: 683 lives have the potential to enroll these are not current covered lives o We are only asking for voluntary life options Paid premiums history is listed on Attachment B There are currently no plans on WOP There are no current pending claims on voluntary life products Each member that leaves has the option to port their voluntary life product, but do not have numbers on available o We do not have anyone currently enrolled in a LTD plan. o Claims history for life and short-term disability is forthcoming in Addendum #2. 3 Question: We need current employee benefit handbooks for all benefits being offered by the County. Include additional voluntary/supplemental carriers summary plan descriptions and rate history. Response: o We currently do not have anyone enrolled or currently offer a LTD plan. o We have a few members enrolled in Aflac policies from an RFP several years ago but we just manage the plan increases and help process claims as necessary. o Our current County STD is included in medical or for $200/week after a 14 day elimination period and we offer a supplemental buy-up through our current vendor (rates attached as Attachment C age frozen). We currently have 57 employees enrolled. Claims history is forthcoming in Addendum #2. o We offer basic life and AD&D for members enrolled in $25,000 or can purchase at.78/pay and basic $5,000 for those enrolled in family medical plans. County pays 100% of cost for those enrolled in medical coverage and 80% of employee cost for those electing life only coverage. We also offer a supplemental 5 year term life policy (rates are attached as Attachment D, Supplemental Term Life and Short Term Disability Rates). o Attached as Attachment C is the summary of benefit pages for Medical, prescription, Life Insurance, and Short-term Disability. o Attached as Attachment E is the summary of benefits for Dental. o Attached as Attachment F is the summary of benefits for Vision. o Voluntary Life and STD rates have held firm since plan inception. o Attached as Attachment G is the current plan information for group basic & AD&D and voluntary term life and STD. We are not looking to replace current group life insurance policy. Currently we offer the following for new enrollments until 6/30/18: o 5 year term life with Companion Life Group (rates on Attachment D) o Supplemental vision with Superior Vision (see Attachment H) o Legal Shield coverage (see Attachment I). We have members on other products but they are not permitted to enroll new members so we don t have current rate information to share past options include Aflac, Allstate, ING, Vision Care Direct. Question: Current renewal, Summary of Benefits, renewal rates Response: We do not currently have supplemental dental. Question: 2 years claims experience Response: See Attachment J for 2 years dental claims experience Question: Full Census Response: Attachment K Census with DOB and Gender Question: Commissions requested Response: We do not have commission level information.
