1 MOBILE SEALANT DELIVERY SYSTEM RENTAL PROCEDURE: 1. AC SUPPLY EMPLOYEE IS RESPONSIBLE FOR FILLING OUT RENTAL AGREEMENT 2. CUSTOMER MUST HAVE CHARGE ACCOUNT. NO COD 3. CUSTOMER MUST SIGN RENTAL AGREEMENT AND WAIVER 4. COPY DRIVERS LICENSE OF PERSON PICKING UP EQUIPMENT 5. MAKE COPY OF ALL FIVE PAGES FOR THE CUSTOMER 6. BILL DEPOSIT AND ONE-DAY RENTAL ON A SALES ORDER. SALES ORDER, RENTAL AGREEMENT, AND WAIVER GOES TO BOB FOR SAFE KEEPING UNTIL MACHINE IS RETURNED. RETURN PROCEDURE: 1. PLUG IN AND START THE MACHINE. 2. CHECK UNIT FOR CLEANLINESS. 3. USE THE CHECK LIST TO CHECK IN THE MACHINE AND ACCESSORIES. 4. PULL UP ORIGINAL SALES ORDER AND REMOVE THE DEPOSIT AND CHANGE THE RENTAL QUANTITY TO HOW MANY DAYS THEY HAD THE MACHINE. 5. ADD CHARGES FOR ANY MISSING ACCESSORIES OR CLEANING FEE

2 MOBILE DUCT SEALANT DELIVERY SYSTEM RENTAL AGREEMENT THIS AGREEMENT is made this () between AC Supply Co. INC, of 100 Page Ave Fort Worth, TX hereinafter called Owner, and (Company), of (Address), _(Phone), hereinafter called Renter. MOBILE SEALANT SYSTEM S/N BA05730 The Owner warrants that to the best of his/her knowledge and belief the aforesaid property is free of faults or deficiencies, which would affect its safe and dependable operation under normal and prudent usage. RENTAL PERIOD The Owner agrees to rent the above-described property to the Renter for a period of (Time Period), beginning () and ending (). USE OF PROPERTY SPRAY DUCT SEALANT HARD CAST # ONLY The Renter further agrees that the rented property shall [1] not be used beyond any rated capacity; [2] shall not be used for any illegal purpose; [3] shall not be used in any manner for which it was not designed, built, or designated by the manufacturer; [4] will not be used in a negligent manner; [5] will not be operated by any other person without the written permission of the Owner; and [6] will not be removed from the designated area of use or operation. INSURANCE The Renter hereby agrees that he/she shall fully indemnify the Owner for any and all damage to or loss of the rented property and any accessories or related equipment during the term of this Agreement whether caused by fire, theft, flood, vandalism, or any other cause, except that which shall be determined to have been caused by a fault or deficiency of the rented property, accessories, or equipment.

3 RENTAL RATE The Renter hereby agrees to pay the Owner at the rate of: $80 LARGE (32140) $50 JUNIOR (31770) per day for the use of said property and any accessories/equipment DEPOSIT The Renter further agrees to make a deposit of $ with the Owner. Said deposit to be used, in the event of loss of or damage to the rented property and any accessories/equipment during the term of this Agreement, to defray fully or partially the cost of necessary repairs or replacement. In the absence of any damage or loss, said deposit shall be credited toward payment of the rental fee and any excess shall be returned to the Renter. CLEANING FEE The Renter will be charged $ cleaning fee if the equipment is not returned in the same clean condition it was received. Cleaning instructions are provided in the instruction manual. RETURN OF PROPERTY TO OWNER The Renter hereby agrees to return the rented property and any accessories/equipment to the Owner at 100 PAGE AVE. FORT WORTH, TX (Location) no later than 11AM. TERMINATION OF AGREEMENT It is mutually agreed that the Renter shall have the right to terminate this Agreement at any time by payment of one full days rental for each 24- hour period or any part thereof, during which the Renter has retained possession of the property and any accessories/ equipment during the term of this Agreement. 6 WAIVER AND ASSUMPTION OF RISK I Customer, voluntarily sign this Waiver and Assumption of Risk in favor of the Owner, AC Supply Company, in consideration for the opportunity to use the Owner's MOBILE SEALANT DELIVERY SYSTEM and/or the opportunity to receive instruction from the owner or the owner s employees, on the use, safety, cleaning of the MOBILE SEALANT DEIVERY SYSTEM. I understand that there are certain risks and dangers associated with the activity and use of the MOBILE SEALANT DELIVERY SYSTEM and that these risks have been fully explained to me. I fully understand the danger involved. I fully assume the risks involved as acceptable to me and I agree to use my best judgment in undertaking these activities and to follow all safety instructions. I waive and release the Owner from any claim for personal injury, property damage, or death that may arise from my use of the MOBILE SEALANT DELIVERY SYSTEM, participation in the activities or instruction.
