5 March is Peanut Month! N.E. March 8 Peanut butter is a convenient and inexpensive staple to keep in your kitchen. One tablespoon of peanut butter has about 95 calories; enjoy in moderation. The peanut is part of the legume family, which means they usually contain lots of B- vitamins that many senior adults are missing but needing in their diet. B-vitamins are especially important for memory and cognition. Peanuts (especially the skin) contain resveratrol, a bioactive nutrient, found to have anti-aging properties and protects cells from being damaged. And the fats found in peanuts are in just the right ratio to help ease joint pains caused by inflammation and the wear-and-tear over the years. A peanut butter sandwich tastes great and it is easy to eat. It is also packed, full of proteins to help senior adults retain their muscles, especially as they get older. Green fruits and vegetables to include in your daily diet are leafy greens, asparagus, green peppers, broccoli, green beans, cabbage, green onion, Brussels sprouts, okra zucchini, cabbage, green apples and grapes, honeydew melon, kiwi fruit and limes. Some Green Ideas: Vegetable Pizza with green peppers. Veggie Dip with Broccoli florets. Green Apple slices with fruit dip. Green Onions, chopped, in potato soup. Avocado slices to sandwiches and salads. Cabbage and corned beef. Kiwi, green grapes and honey dew melon in your fruit salads. 15 Medicare Minute Script February 2017 Special Enrollment Periods (SEPs) P.E. March 24 Medicare has Special Enrollment Periods called SEPs that allow you to leave your Medicare Advantage Plan or your Part D Prescription Drug Plan and enroll in a different plan, even if it is not the annual open enrollment period. Today we will talk about a few of these SEPs. Point 1: Know how to determine if you qualify for an SEP because your plan broke Medicare s rules. If you believe your plan did not follow Medicare rules, you may have the right to change plans. Here are some examples of rule violations: Your plan did not send you a notice telling you it wasn t going to cover one of your drugs in 2017 Your plan did not provide benefits in a timely manner Your plan did not provide benefits that meet Medicare s quality standards Your plan gave you misleading or inaccurate information that enticed you to enroll Your plan did not give you information about benefits in a timely manner If Medicare determines that your plan broke rules, you will qualify for an SEP. The SEP starts once Medicare decides that your plan broke rules. If you enroll in a new plan right away, coverage starts on the first day of the next month. Otherwise, you have 90 additional days to choose a new plan once you disenroll from your current plan. Your new coverage generally starts the first day of the month following the month that the plan receives your enrollment application. In the meantime, Original Medicare covers you. To see if you qualify for SEP, you can call MEDICARE and explain your situation. Your State Health Insurance Assistance Program called SHIP - or your Senior Medicare Patrol program called SMP -- can also assist you in understanding Medicare s enrollment periods and the rules that plans must follow. To qualify for this SEP, you must be able to demonstrate how the plan broke Medicare s rules, so you should keep plan notices. It is also useful to track the names of plan representatives you talk with and the dates you talked with them. Point 2: Know your right to an SEP if a federal employee makes a mistake with your drug plan enrollment. You may also have an SEP if a federal employee made a mistake when enrolling or disenrolling you from a drug plan. One example would be if you called MEDICARE to enroll in a new Part D plan for 2017 and you now find that you were never enrolled. This SEP starts the month that Medicare approves it and lasts for two more months. To use this SEP, you must be able to show that a federal employee made a mistake. It is useful to have the name of the federal employee who helped you, such as the name of the MEDICARE customer service representative you spoke with, the date and time of the call, and the outcome. Call MEDICARE to learn if you qualify for this SEP. You can also contact your SHIP program for more information about this SEP. Point 3: Know if you have an SEP because of a plan termination or sanction. You may have an SEP if your plan ends, which is called a termination. What if your plan decides to end coverage in the midst of a coverage year? 16 continued... months before the plan ends and up to one month after the month that plan ends. What if your plan announces it will not provide Medicare coverage in the coming year? You will have until the end of February of the coming year to select a new plan. What if Medicare ends your plan because the plan violated its contract with them? You will get an SEP to enroll in a new plan starting one month before the plan ends and up to one month after it ends. This can happen at any time during the year, so pay attention to all notices you receive from your plan and from Medicare, You may have an SEP if your plan is penalized by Medicare, which is called a sanction. This means that Medicare has found a problem with your plan. One example would be if the plan does not meet Medicare requirements for handling appeals. There are many other potential reasons. The start date and length of this SEP is decided on a case-by-case basis. If this applies to you, you will receive an announcement from Medicare. Read the Medicare announcement for more information, call MEDICARE, or contact your SHIP. Take Action: If you have a problem with your Medicare Advantage or Part D prescription coverage, consider whether you are eligible for an SEP. 1. Contact MEDICARE to explain your situation. 2. Gather required information, including notices from your plan and information from government employees. 3. Contact your State Health Insurance Assistance Program (SHIP) if you have questions about if you qualify for an SEP, or if you want help accessing an SEP. 4. Contact your Senior Medicare Patrol (SMP) if you have questions about Medicare s plan marketing rules. 17 EXTRA Medicare Minute Teaching Materials February 2017 Special Enrollment Periods (SEPs) 1. What is a Special Enrollment Period (SEP)? A Special Enrollment Period, or SEP, allows you to change your Medicare Advantage Plan or your standalone Part D plan (PDP) that works with Original Medicare, outside of regular enrollment periods. You can usually only make changes to your coverage during Medicare s Open Enrollment, which runs October 15 to December 7 of each year, with coverage starting January 1 of the following year. If you qualify for an SEP, then you can change your coverage outside of Medicare s Open Enrollment. You have the right to an SEP in specific and limited circumstances. 2. Do I have an SEP if my plan violates its contract? If you believe your Medicare Advantage or Part D plan violated the terms of its contract with Medicare-- meaning the plan did not follow Medicare s rules for private health care plans you may have the right to an SEP that enables you to elect, or enroll in, another plan. Your SEP begins once Medicare determines that the violation occurred. If you do not choose a new plan right away, your SEP extends for 90 days from the time you disenroll from your current plan. In general, your new coverage starts the first of the month following the month that your new plan receives your enrollment application. Examples of a contract violation include: Your plan did not provide benefits that meet quality standards, such as if Medicare terminates your plan for failure to meet star ratings requirements. Your plan gave you misleading marketing information to get you to enroll, such as promising a zero dollar premium for a plan that does have a premium. Medicare sets marketing guidelines for Medicare Advantage and Part D plans, and there are certain things they cannot do or say when advertising their plans to you. Your plan did not give you information about its benefits in a timely manner, such as if the plan did not send you a notice telling you it wasn t going to cover one of your drugs in 2017, or your plan did not provide benefits in a timely manner. To access this SEP, call MEDICARE and explain your situation. You must be able to show that the plan did not follow its contract or marketing rules. Proof could be a plan marketing notice that does not follow Medicare guidelines, or the name of a plan agent who told you wrong or misleading information about the plan, and the date you spoke to them. 3. Do I have an SEP if a federal employee makes a mistake with my plan enrollment? You may have an SEP if a federal employee made a mistake when enrolling or disenrolling you from a prescription drug plan. For example, you could use this SEP if you called MEDICARE to enroll in a new Part D plan for 2017, but now find that you were never enrolled in that plan. Your SEP starts the month that Medicare approves the SEP and lasts for two more months, and when your new coverage starts depends on the situation. To use this SEP, you must be able to show that a federal employee made a mistake. Do I have an SEP if a plan representative mistakenly disenrolls me from my current plan? There is no SEP to change your coverage if a plan representative makes a mistake with your enrollment (unlike if a MEDICARE customer service representative makes a mistake, since they are considered federal employees and plan representatives are not). If a plan representative mistakenly disenrolls you from your plan, the plan must re-enroll you in that same plan. 5. Do I have an SEP if my plan is ending? There are a few SEPs you can use if your plan is ending, depending on the situation. If Medicare ends your plan because it violates its contract, you have an SEP to switch to another plan. The SEP starts one month before the plan ends and up to two months after the plan ends. Your plan must give you 30 days notice before the end date. You can choose to have your new plan begin up to three months after the month that your other coverage ended. Note: The managed care manual explains that if an individual does not elect an MA plan before the termination effective date, he/she will be defaulted to Original Medicare on the effective date of the termination. However, the SEP will still be in effect for one month after the effective date of the termination should the individual wish to subsequently elect an MA plan (for a current, not retroactive, effective date). When your new coverage begins depends on the situation. If your plan changes or ends its contract with Medicare in the middle of the year, you have an SEP to switch to another plan two months before the plan ends and up to one month after the plan ends. Your plan must give you 60 days notice before the date it will make the changes or will end. You can ask that your new plan coverage start the month after you get notice or up to two months after your old coverage ends. If your plan decides to no longer offer Medicare coverage for the next year, it must notify you by October 1 that it will not offer coverage next year. You have an SEP to enroll in a new plan from December 8 to the last day of February of the next year. Note that this SEP is in addition to Medicare s Open Enrollment Period, which runs October 15 through December 7 each year. During Medicare s Open Enrollment, you can make any changes to your Medicare coverage. 2

19 EXTRA 6. Do I have an SEP if my plan is sanctioned? A Medicare sanction means that Medicare has found a problem with your plan, such as if the plan does not meet Medicare requirements for handling appeals. You can use an SEP to choose a new plan if the sanction directly affects you. The start date and length of your SEP is decided on a case-by-case basis. You can learn more about the SEP from the Medicare announcement that describes the sanction, on the plan s website, or by calling MEDICARE. 7. Do I have an SEP if I move? You will have an SEP if you permanently move out of your plan s service area, or if you move to an area covered by your plan but with new plans available in your new coverage area. If you notify your plan in advance, you have an SEP to enroll in a new plan beginning as early as the month before you move and up to two months after you move. If you notify your plan after you move, you have an SEP to enroll in a new plan beginning the month that you notify your plan and up to two months after. If you do not notify your Part D plan that you move, and your Part D plan learns from Medicare or the post office that you moved over twelve months ago, the Part D plan should disenroll you starting with the twelfth month after your move. Your SEP to enroll in a new Part D plan begins at the beginning of the twelfth month and continues through the fourteenth month after your move. If you do not choose a new plan during this time, you will be enrolled in Original Medicare without prescription drug coverage. If you do not notify your Medicare Advantage Plan of your move, and the plan learns from Medicare or the post office that you moved over six months ago, the plan should disenroll you starting with the twelfth month after your move. Your SEP to switch to another Medicare Advantage Plan begins at the beginning of the sixth month and continues through the end of the eighth month after you move. 8. Do I have an SEP if I move into or out of an institutional facility (such as a nursing home)? You are entitled to an SEP if you move into, reside in, or move out of a qualified institutional facility, such as a skilled nursing facility, nursing home, psychiatric hospital or unit, Intermediate Care Facility for Individuals with Intellectual Disabilities, rehabilitation hospital or unit, long-term care hospital, or swing bed hospital. Once you move to or reside in a qualified institution, you can enroll in, disenroll from, or change plans once a month. If you are in a Medicare Advantage Plan you may change to another Medicare Advantage Plan or change to Original Medicare. After you move out of the facility, you have two months to enroll in or disenroll from a plan, or to switch to another plan (including Original Medicare if you are in a Medicare Advantage Plan). Your coverage begins the first day of the month after you submit a completed application, but not before you become institutionalized. 9. What SEPs are available based on my plan s star rating? Medicare uses star ratings to measure plan performance in different categories. Ratings range from one to five stars, with one being the lowest and five being the highest rating. 3

20 EXTRA If you have been enrolled in a plan that has received an overall Medicare star rating of fewer than three stars for three years in a row, then you have an SEP to enroll in a higher rated plan throughout the year. You should have received notice in October of last year telling you about your plan s low star rating. You have until the end of that year and the end of the next year to switch to a plan with three or more stars. To use this SEP you must call MEDICARE directly. You can also enroll in a five-star plan if you live in the plan s service area. Your SEP to join a five-star plan starts December 8 of the year before the plan is considered a five-star plan and ends November 30 of the year the plan is considered a five-star plan. You can use this SEP once per year by calling MEDICARE. 10. Do I have an SEP if I lose creditable drug coverage? Creditable drug coverage is drug coverage with an overall value that is as good as or better than Medicare Part D coverage. If you lose creditable drug coverage through no fault of your own, or the coverage is reduced so that it is no longer creditable, you will have an SEP to join a stand-alone Part D plan. The SEP begins the month that you are told your coverage will end and lasts for two months after you lose your coverage or two months after you receive notice, whichever is later. Your coverage begins the first day of the month following the month after application, or up to two months after your SEP ends, if you request it. 11. Do I have an SEP if my prescription drug plan did not tell me that my drug coverage was no longer creditable? If Medicare determines that your previous drug coverage failed to notify you that your drug coverage was no longer creditable, you have an SEP to enroll in or disenroll from a Medicare Advantage Plan with prescription drug coverage or a stand-alone Part D plan. The SEP begins the month that Medicare approves the SEP and lasts two additional months. To access this SEP, you will have to make the case that the plan did not provide the notice or that the notice was not accurate. 12. Do I have an SEP if I lose employer-based coverage? If you drop current or former employer/union drug coverage, regardless of whether it is creditable, you have an SEP to enroll in a Medicare Advantage Plan with prescription drug coverage or a stand-alone Part D plan for up to two months after the month that your employer or union coverage ends. Your new coverage begins up to three months after the month in which you submit a completed application. If your employer/union was late in sending the application, your coverage may be retroactive to when you submitted the application. 13. What SEPs are available if I have a limited income? If you have Extra Help, the federal program that helps pay for your Medicare prescription drug costs, then you have an SEP to change your prescription drug coverage up to once per month. Your new coverage starts on the first day of the month following enrollment. You can switch to a different PDP, disenroll from Original Medicare and enroll in an MA-PD, or disenroll from an MA-PD and enroll in Original Medicare with a PDP. If you have Medicaid, a Medicare Savings Program (MSP), or Supplemental Security Insurance (SSI), you get Extra Help automatically and can use the Extra Help SEP to change your coverage once per month. 4

21 EXTRA Many states offer a State Pharmaceutical Assistance Program (SPAP) that helps with your prescription drug costs. If you have an SPAP, you have an SEP to switch your prescription drug coverage once per year. Your new coverage starts on the first day of the month following enrollment. 14. What resources are available to help me understand SEPs and learn if I am eligible for one? MEDICARE: Contact Medicare if you are eligible for an SEP and want to change your coverage. You can also contact Medicare if you think you are eligible for an SEP and want more information. State Health Insurance Assistance Program (SHIP): Contact your local SHIP for personalized, one-onone counseling and assistance with understanding SEPs. SHIP counselors can explain the different SEPs and let you know if you may be eligible to use one to change your coverage. The national SHIP mission is to empower, educate, and assist Medicare-eligible individuals, their families, and caregivers through objective outreach, counseling, and training, and to make informed health insurance decisions that optimize access to care and benefits. Senior Medicare Patrol (SMP): Contact your local Senior Medicare Patrol (SMP) for education and assistance regarding suspected Medicare fraud, errors, or abuse. The national SMP mission is to empower and assist Medicare beneficiaries, their families, and caregivers to prevent, detect, and report health care fraud, errors, and abuse through outreach, counseling, and education. See the last page of the Teaching Materials for SMP program contact information. SHIP Case Study Duane recently visited the ophthalmologist, and they informed him that his plan no longer covers him. Lately, his eyesight has worsened. His wife used to take care of all of their paperwork, but since she passed away, it has really been piling up. After the news from the ophthalmologist s office, Duane went through his unopened mail and noticed a letter from his plan saying that they were ending coverage in his area. Duane is very confused and upset. He doesn t know what he should do. What should Duane do? The SHIP counselor can speak with Duane about his options, including whether or not he may qualify for an SEP. SMP Case Study Lisa became eligible for Medicare in June She signed up for a Medicare Advantage Plan that promised to cover one of her medications. Lisa did not find many Medicare Advantage Plans that covered her medication, so she was happy to enroll. She has tried to gain approval for this drug for several months, but has not received authorization from her plan. What should Lisa do? Is she eligible for an SEP? Lisa should contact her Senior Medicare Patrol (SMP) to discuss her situation. 5

26 5 Nutrition Tips to Promote Wound Healing Source: The Academy of Nutrition and Dietetics, Lynn Grieger, RDN, July 21, 2015 N. E. March 29 We've all had a wound: a cut, scratch or scrape that breaks the skin. Most wounds on healthy people heal quickly when kept clean and free of infection, while other types of wounds are more serious and often require medical intervention. People with diabetes also have a higher risk of developing foot ulcers that can take weeks or months to heal. Food choices and nutritional status influence wound healing since serious wounds increase the energy, vitamin, mineral and protein requirements necessary to promote healing. The Nutrition Tips 1. Eat sufficient calories from a balanced diet of nutritious foods. Plan healthy, balanced meals and snacks that include plenty of foods from all the MyPlate food groups protein, fruits, vegetables, dairy and grains. 2. Eat plenty of protein. Aim for 20 to 30 grams of protein at each meal and 10 to 15 grams of protein with each snack. A piece of cooked chicken, lean meat or fish the size of a deck of cards (about 3 ounces) contains 20 to 25 grams of protein. One egg, 1 tablespoon of peanut butter and 1 ounce of cheese each contain 6 to 7 grams of protein. One cup of low-fat milk or yogurt contains 8 grams of protein. 3. Stay well-hydrated with water and other unsweetened beverages such as tea, coffee, 100-percent fruit juice and milk. 4. Some wounds may require a higher intake of certain vitamins and minerals. Talk with a Registered Dietitian Nutritionist for an individualized eating plan with optimum amounts of calories, protein, fluids, vitamins and minerals for your specific needs. 5. For people with diabetes, controlling blood sugar levels is one of the best ways to prevent and treat a wound.
