Page 1 NOTRE DAME LAW SCHOOL LOAN REPAYMENT ASSISTANCE PROGRAM APPLICATION FORM Part II Applicant s Name: Primary Address: Date Part I was submitted if not submitted with this Part II: PDF Instructions Save this fillable PDF to your computer or flash drive. Name the file Last name, First name e.g., Kustner, Cathy. Open the saved PDF on your computer, and fill in the requested information. Save the completed PDF and send it as an attachment to ckustner@nd.edu. Alternatively, you may print the PDF and fill it in by hand, and either scan it to as a PDF, or mail it as a hard copy. In all cases, Part I (and Part II, if applicable) must be received at NDLS by November 1, 2017, not just postmarked by that date. Application Instructions Only your law school loan payments are eligible for LRAP funding; however, your non-law school educational loan payments will be taken into account, as they serve as a reduction to salary, as noted in Part I of your application. Thus, it is imperative to distinguish between loan payments to be made towards law school debt as opposed to the rest of your educational loan payments. To accomplish this allocation, we first ask you to list all of your original law school loans, followed by a list of all non-law school original loans (if any). If any original debt has been paid off in full (not just consolidated), please clearly note that fact where applicable. Next, we ask for a list of all your scheduled 2018 payments for both law school and non-law school loans. Given that there are a wide variety of loan repayment plans, many of which include consolidated loans involving law school and other educational loans (including federal income driven repayment plans), this application uses a numbering system (e.g., 1L-A, 2L-B, UG-A) that facilitates a way to connect original loans with the corresponding loan repayment amount. With consolidated loans, a single loan payment may be connected to multiple original loans. It is important to note EVERY original loan associated with each loan payment. If an original loan is NOT associated with a particular loan payment and it is not noted as paid-off in full, an explanation must be included. It is also important to remember that the LRAP resources you receive are directly tied to the loan payments you will make throughout the program year. If your payments change during the year, the amount of LRAP assistance will also be directly affected. As a result, this application also requests information about any dates when your loan repayment amount may change. That is particularly true for federal income-driven repayment plans. To aid your completion of the original loan data, federal loan information can be obtained from the National Student Loan Data Base System, at A copy of the report that you download from this site is required as an attachment to this LRAP application.
Page 2 Original Loan Information Law School Note 1: Types of loans include subsidized federal, unsubsidized, Grad Plus, Perkins, and private loans (CitiAssist, LawAccess, etc.). Include only loans obtained in connection with earning a J.D. degree. Transfer students may include loans incurred in connection with their first year J.D. studies at another law school. Bar loans are not eligible for LRAP and therefore may not be included. Note 2: As noted above, the numbering system below, i.e., 1L-A, 1L-B, will also be referenced later in the application. You will need to use these individual application loan numbers to match up original loans with the associated payments. Type of Loan Original Amount If loan has been (NOT (see Notes above) of Loan just consolidated), check the box First Year: Second Year: 1L-A: 1L-B: 1L-C: 1L-D: 2L-A: 2L-B: 2L-C: 2L-D: Third Year: 3L-A: 3L-B: 3L-C: 3L-D: TOTAL LAW SCHOOL ORIGINAL LOANS $

Include any undergraduate loans or loans incurred in connection with non- J.D. graduate study. UNDERGRADUATE LOANS: Type of Loan Original Amount If loan has been (NOT (see Note above) of Loan just consolidated), check the box UG-A: UG-B: UG-C: UG-D: UG-E: UG-F: UG-G: UG-H: POST-GRADUATE LOANS: PG-A: PG-B: PG-C: PG-D: TOTAL NON-LAW SCHOOL ORIGINAL LOANS $
Page 4 Income-Driven Federal Loan Repayment Plans (law school & non-law school) If your loans are in deferment, forbearance, or you do not currently know from your lender what your actual loan payments are or will be, please do not complete this section until you have solidified your repayment plans with your lender. Estimated loan payments from your lender are not sufficient. Your application will not be considered complete until you have solidified your repayment plans with your lender AND can attach a lender statement documenting this firm monthly repayment amount with actual monthly due dates. If your Part I application is received by the November 1 deadline, having incomplete LOAN REPAYMENT information will not disqualify you from the Program. If you are in a federal income-driven repayment plan, and already have documentation for your actual monthly loan payments, answer the following questions. Otherwise, skip to the next section (All NON-Income Driven Repayment Plans). What type of income-driven repayment plan are your federal loans in: Income based repayment plan (IBR) Income contingent repayment plan (ICR) Pay as you earn repayment plan (PAYE) Revised Pay as you earn repayment plan (REPAYE) Other. Explain: What is your monthly payment? If the above payment does NOT cover all your federal loans, law school and non-law school, please explain: What month did or will you start paying this amount? What month is your income-driven loan repayment subject to change? Please remember to report monthly loan changes when new loan payment amounts are known. Who is your federal loan servicer(s)? If this federal loan payment covers both law school and non-law school loans, is there a reason this payment amount should NOT be pro-rated using the above reported original debt for ALL YOUR FEDERAL LOANS?
Page 5 All NON-Income Driven Repayment Plans (law school & non-law school); include federal and private loans On page 6, please list the amount of each of your monthly loan payments that are not in any federal income driven repayment plan, scheduled to be made during 2018, including law school and non-law school loans. Each scheduled loan payment MUST BE matched up to an original loan(s) using the loan numbering system explained on page 1, i.e., 1L-A, 1L-B, UG-A, etc. If any of your loans have been combined or consolidated, it s very important to clearly identify which loans are covered by each combined or consolidated monthly payment. Identifying the loans associated with each payment will determine which payment will be pro-rated between law school and non-law school loans. For example, let s say you have (1) three subsidized loans that were received for each of your 1L, 2L, and 3L years; (2) three unsubsidized loans for each year; and (3) one Grad Plus loan for your 1L year only. Additionally, you have undergraduate loans. If your lender (loan servicer) has combined the payments for your subsidized 1L, 2L, and 3L loans into one monthly payment, and the payments for your unsubsidized 1L, 2L, and 3L loans into a second monthly payment, you would list each monthly payment amount only once in the first column, along with the name of your lender in the second column, and all loan numbers corresponding to the combined loans in the third column. Continuing with this example, if your payments for the subsidized and unsubsidized loans were not due until March, you would list that month in the fourth column. Finally, indicate the number of scheduled payments you expect to make in 2018; i.e., January through December would be 12 payments. This payment reporting process also applies to consolidated loans. For example, if you had consolidated your 1L Grad Plus loan with your undergraduate debt, you would list the monthly payment for that consolidated debt along with the corresponding original loan numbers (for example, IL-C, UG-A, etc.). The examples given above would be reported on page 6 as follows: Month payment Number of Monthly Lender or Loan Numbers begins if not in scheduled Payment Loan Servicer (e.g., IL-A) January payments for 2018 $ Fedloan 1L-A, 2L-A, 3L-A March 10 $ Fedloan 1L-B, 2L-B, 3L-B March 10 $ Sallie Mae 1L-C, UG-A through UG-F February 11 All payments must be supported by lender documentation. Lender documentation can be submitted in the form of a PDF or screen shot of your electronic monthly statement, but needs to reflect your name, the amount(s) due, and the due date(s). Please also clearly mark all attached lender documentation according to the numbering system explained on page 1 (e.g., 1L-A, 2L-B, UG-A). As mentioned above, all payments must be traceable to an original loan, and all original loans must be tied to a payment unless the loan has been paid off in full. Remember, LRAP covers the law school loan payments that you will be making in 2018, so it is important to indicate how many months you expect to have scheduled payments. If you will not be making 12 monthly payments, please identify the month these payments are scheduled to begin.
Page 6 All NON-Income Driven Repayment Plans (law school and non-law school); include federal and private loans Number of Month payment payments Monthly Lender or Loan Numbers begins if not in scheduled for Payment Loan Servicer (e.g., IL-A) January 2018 Total Monthly Payments
Page 7 Are any of your loan payments subject to change during the year (other than variable rate loans)? No Yes. If yes, please explain: CERTIFICATION AND AGREEMENT I certify that all information provided above is true and accurate as of this date. I understand that a new application must be submitted each year and that the deadlines to apply for the Loan Repayment Assistance Program at the Notre Dame Law School (the AProgram@) in future years will be November 1. I further agree to provide any additional or clarifying information or documentation that the Program Administrator or Program Coordinator may request and to notify the Program Coordinator within two weeks of any significant changes in my circumstances, such as a change in employment status, a change in loan repayment terms or amounts, a change in annual income, the receipt of loan repayment assistance from another program, a change of residential address or any other personal contact information, or any other change that would affect either my eligibility or the amount of my benefits under the Program. If selected to receive loan repayment assistance, I also agree to cooperate with publicity efforts. Notre Dame Law School may use my name, photograph, and information about my public service work to publicize the Loan Repayment Assistance Program, both within the university community and to outside audiences in print, electronic, or any other format.
