4 RADR TOWNSHIP SCHOOL DISTRICT The District Business Office acts as real estate tax collector for school tax collection. We currently use a lockbox service for this purpose. Does your institution provide lockbox services for collection and processing of tax payments? If lockbox service cannot use customer data base to process payments, please explain the system proposed to process payments and provide mechanism for electronic upload of all processed tax receipts into the customer data base Can lockbox payments be made at the local branch on a walk-in basis? Can lockbox payments be made at any branch operated by the financial institution? Are there limited hours when walk-in lockbox payments can be accepted at the local branch(s)? Name and mailing address of the reference along with a contact person and telephone number must be included with each reference provided. The Financial Institution shall also attach to the RFP the following information concerning its branches: The address of the bank's branch at which the School District would most regularly conduct business through the term of this agreement. The bank branch must be located within or adjacent to the borders of the Radnor Township School District Please specify the hours this branch is open to conduct business. Page 6 of 7

7 RADR TOWNSHIP SCHOOL DISTRICT The Financial Institution shall also include with this RFP the names and professional backgrounds of the manager of the branch, a cash management representative, a regional vice president, and the bank president. Please include copies of your last two (2) years audited financial statements. Conditions Financial Institution hereby agrees that all unit prices and all other conditions related to the banking services described herein shall be in effect for a forty-eight (48) month period. The only exceptions to this would be changes necessitated by Federal Reserve Bank directives or by any other applicable federal or state laws or regulations. The School District shall be permitted to void this agreement at any time, for any reason whatsoever as it deems necessary with thirty days written notice to the financial institution. I hereby certify that I have read and understand all of the above requirements contained herein; that all answers are true and correct; and that I am authorized to provide this proposal on behalf of the financial institution named herein.
