3 PLEASANT VIEW PUBLIC SERVICE DISTRICT (Sewer) P.S.C. W. Va. No, 9 Original Sheet No. 2 STEP 1 RATES (Effective for 24 months) APPLICABILITY Applicable within the entire territory served. AVAILABILITY Available for sanitary sewer service. RATE (Based on the metered amount of water used) All Customers Flat Rate of $6.52 per 1,000 gallons NON-METERED WATER SYSTEM SUBSCRIBERS In the event a building or premises discharging sewage or other liquid waste into the sewage systems uses water fiom a public or private water system that is metered, the owner or occupant may be required to install a water meter or other metering device. Pending such installation, the charges for service shall be based on the average for similar premises and occupancy, but, in no event shall the rate be less than the minimum bill. MINIMUM CHARGE No bill will be rendered for less than the following: Domestic and Commercial Customers Schools $19.56 per month $74.00 per month DELAYED PAYMENT PENALTY The above schedule is net. On all accounts not paid in full when due, ten percent (10%) will be added to the net current amount unpaid. This delayed payment penalty is not interest and is to be collected only once for each bill where it is appropriate. (I) TAP FEE A tap fee of $ will be charged to all customers applying for service outside of a certificate before the Commission for each new tap to the system. No. 9 Original Sheet No. 3 STEP 1 RATES (Continued) LEAK ADJUSTMENT $1.16 per 1,000 gallons is to be used when a bill reflects unusual water consumption which can be attributed to eligible leakage on the customer s side of the meter. This rate shall be applied to all such consumption above the customer s historical average usage. RETURNED CHECK CHARGE A service charge equal to the actual bank fee assessed to the District, not to exceed $25.00, will be imposed upon any customer whose check for payment of charges is returned by his or her bank due to insufficient funds. SECURITY DEPOSIT Not to exceed two-twelfths (2/12) of the average annual usage of the applicant s specific customer class or fifty dollars, whichever is greater.
