Entry Period: The FTD Celebrating 100 Years Sweepstakes begins on November 1, 2009 at 12:01 a.m. Eastern Time and ends on November 30, 2009 at 11:59 p.m. Eastern Time (the Entry Period ). Automatic Entry: For each order a Qualified FTD Florist sends through the FTD Mercury Network during the Entry Period, the Qualified FTD Florist receives an automatic entry into the FTD Celebrating 100 Years Sweepstakes. Free Entry Permitted by Mail: During the Entry Period, a Qualified FTD Florist may send in a 3 x 5 card on which it has printed its name, owner s name, mailing address, telephone number, and e-mail address (if available) to: FTD Celebrating 100 Years Sweepstakes, c/o Florists Transworld Delivery, Inc. at 3113 Woodcreek Drive, Downers Grove, IL 60515. All mail-in entries must be postmarked during the Entry Period (and no later than November 30, 2009) and received by close of business on December 7, 2009. All entries become the property of Sponsor and will not be returned to the entrant. There is no limit on the number of entries by either method of entry. WINNER SELECTION: One First Place Prize winner, one Second Place Prize winner, and one Third Place Prize winner will be selected by a random drawing from all eligible entries received. Drawing is to take place at approximately 6:00 P.M. Eastern Time on or about December 7, 2009 by an independent judging organization, whose decisions will be final with respect to all aspects of the FTD Celebrating 100 Years Sweepstakes. NOTICE TO CANADIAN PARTICIPANTS: If a selected First Place Prize, Second Place Prize, or Third Place Prize winner is a Qualified FTD Florist that is a Canadian resident, such winner must also solve a mathematical skill test administered by telephone at a pre-arranged mutually convenient time in order to claim its prize. If such winner is unable to solve the test, an alternate winner will be selected. WINNER NOTIFICATION AND RELEASE: All prize winners will be notified via telephone and U.S. mail within seven days after drawing date and must execute an Affidavit of Eligibility and Liability Release, which must include the winner s tax identification number for tax reporting purposes, and, where permitted by law, a publicity release, within seven days following the first attempted notification by Sponsor. Non-compliance within this seven day time period may result in disqualification and selection of an alternate winner. LIMITATIONS ON TRIPS: For the First, Second and Third Place Prizes: each individual selected by a winner must agree to travel with his or her travel companion on the dates specified by Sponsor. The two must travel on the same itinerary (on the same dates and from the same city) and at the same time. If no such individual is available on trip dates, winner may be disqualified and the prize forfeited. All travel dates and arrangements are subject to change and availability. For all trips, each individual selected by a winner as his/her travel companion must travel on the same itinerary (on the same dates and from the same city) and at the same time. The winners and their travel companions may be required to provide appropriate government issued photo identification (such as a valid passport) at time of travel. The winners and their travel companions are solely responsible for determining and obtaining all such necessary travel documents and other travel requirements. Sponsor is not liable for any expenses incurred as a consequence of flight cancellation/delay. For the First or Second Place Prizes: If the BCS Championship Game or the Rose Parade/Rose Bowl Game, as applicable, is cancelled or postponed, then Sponsor reserves the right, in its sole discretion, either (i) to reschedule the First Place Prize trip or Second Place Prize trip, as applicable, to coincide with the new date or (ii) to maintain the travel arrangements already made and substitute cash for the value of event tickets. If Sponsor chooses to reschedule the trip and no individual designated by the winner is available for travel on the new date, the prize may be forfeited and, in such case, an alternate First Place Prize winner or Second Place Prize winner, as applicable, will, to the extent practicable, be selected from among the remaining eligible entries. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft of or destruction or unauthorized access to, or alteration of, entries.
