To reserve your date, contact Teresa McCarthy at 763-862-4363 or tmccarthy@epiphanymn.org. Once you have reserved space, contact Rhonda Dillon, Director of Catering at 763-862-4321 or rdillon@epiphanymn.org. One half of the Epiphany rate will be required in order to hold your date. The Church reserves the right to release a reserved date if the signed rental agreement and required deposits are not received within 30 days. This deposit is refundable up to ninety days prior to your event. It is non-refundable after that time. The balance is due two weeks before your event. Table linens and chair covers are available to rent for your event. Please contact Rhonda Dillon for prices. If the cafeteria has not been booked for a Friday or Saturday evening 60 days prior to the date, it will be made available for $250.

Alcohol Information If you choose to provide alcohol to your guests, please contact Rhonda Dillon at 763-862-4321 or rdillon@epiphanymn.org and she will put you in contact with Epiphany s provider. We will require you, the host, to carry an insurance policy for the day of your event, listing Epiphany Church as an additional insured. This policy needs to be in the amount of $1,000,000.00. You may obtain this through your homeowner insurance policy or we will arrange a policy from Catholic Mutual Insurance Company for a fee of approximately $150.00. We will need a photocopy of your policy. Any event that serves alcohol requires an additional $150 damage deposit. Deposit will be mailed within 3 business days of the rental, providing that there has been no alcohol-related damage. A police officer must be hired when alcohol is served, with costs ($85/hour) paid by the permit holder. The officer must stay until everyone has left the facility. If the police officer has to stay past the scheduled time, overtime will be charged to the permit holder at a cost of $85 per hour with a minimum of one hour. Catering Information Once you have reserved your date with Teresa McCarthy at 763-862-4363 or tmccarthy@epiphanymn.org, you should contact Rhonda Dillon, Director of Catering at 763-862-4321 or rdillon@epiphanymn.org. Epiphany has a well-equipped, state-approved kitchen and in-house, professional catering staff. All food to be served to your guests will be prepared by our staff. Outside caterers are not permitted. Pot luck is not permitted. Wedding cakes may be brought in from an outside source. Epiphany charges a $75.00 fee for cutting a cake if asked. You may appoint someone to cut your wedding or anniversary cake to eliminate this fee. Chocolate fountains are available for rental from Acapulco Restaurant. If you choose to provide alcohol to your guests, please let Rhonda Dillon know and she will put you in contact with Epiphany s provider. Full catering payment and final guest count is due one week prior to the event. Your invoice will be based on this guaranteed number, unless your guest count exceeds the confirmed number. If an adjustment needs to be made, the balance is due on the day/evening of your event. Payment may be made by cash, check, or Visa/MasterCard (credit card charges will carry a 3% up charge.)

Sound System Information A $200 fully-refundable deposit is required to use any Epiphany sound equipment. Deposit will be mailed within 3 business days of the rental, once it has been established by an Epiphany staff member that no damage has occurred. Facility Information The Church of the Epiphany provides two options for rentals - the cafeteria, which accommodates up to 325 guests, and the meeting rooms, which accommodate up to 80 guests. The Epiphany grounds contain a beautiful outdoor courtyard, a fountain and multiple, well-maintained gardens that make a variety of wonderful settings for family and wedding party photos. If your event involves more than 75 people, or the serving of alcoholic beverages, we will require you, the host, to carry an insurance policy for the day of your event, listing Epiphany Church as an additional insured. This policy needs to be in the amount of $1,000,000.00. You may obtain this through your homeowner insurance policy or we will arrange a policy from Catholic Mutual Insurance Company for a fee of approximately $150.00. We will need a photocopy of your policy. The User must identify in writing the nature and purpose of the intended Use of the Parish Facilities. No Use of the Parish Facilities will be allowed that is in conflict with, contravention of, in opposition to, or which ridicules the teachings, tenets or philosophy of the Roman Catholic Church or for any immoral or illegal purpose, as determined in the sole discretion of the Parish. It is the User s responsibility to provide sufficient information about the planned Use to allow the Parish to make a determination whether the planned Use of the Parish Facilities complies with this provision and to inquire whether the planned Use of the Parish Facilities complies with this provision. The Parish shall have the right to terminate this Agreement at any time if the Parish, in its sole discretion, determines the Use violates this provision or the User misrepresented the planned Use of the Parish Facilities. In that event, the User shall forfeit any deposit and shall be responsible for all damages incurred by the Parish. In accordance with Minnesota Statute 363A.26, the Parish Facilities may not be used to solemnize or celebrate a civil marriage, except a marriage between a man and a woman. The hallways and other common spaces are not included in your rental agreement and must remain clear at all times. No tables and/or equipment are allowed to be set up in this space. No loitering is allowed in this space. When renting the meeting rooms, the doors going out to the church foyer must be closed at all times and there will be no loitering in the foyer area. The Epiphany 24 hour chapel is located across from the meeting rooms and we need to be respectful of our adorers. Violating this policy of fire code/trespassing will result in a penalty of $500 and/or termination of the existing event and/or refusal to grant permission for future use of the facility.

Facility Information (cont d) The Church of the Epiphany is handicapped accessible and totally smoke free. The Church of the Epiphany is not responsible for lost or stolen articles. The Church of the Epiphany reserves the right to deny the use of its facilities to any individual, group or organization. We do not allow our facilities to be rented by outside groups. No form of fundraising is permitted. The renter will agree that every person connected with the event shall abide by, conform to, and comply with all applicable statutes, laws, rules and ordinances, and will not do anything in violation of any such statutes, rules, laws, or ordinances. The Church of the Epiphany reserves the right to refuse entry to or evict any objectionable person from the property. The renter waives any right and all claims for damages. The Church of the Epiphany assumes no responsibility whatsoever for any property, renter or event patrons. The Church is hereby expressly released and discharged from any and all loss, injury, or damage to persons or property that may be sustained. Signs and banners are welcome, but must be affixed with blue painter tape. Candles are allowed if the flame is enclosed. Renters will not injure, mar or deface or use nails, tacks or pins in any part of the facility. Please remove all personal items and place trash in the proper receptacles before leaving the facility.
