Agency / Firms fulfilling the following criteria will be eligible for empanelment for Hiring of DG Sets of various capacities for Offices under Gwalior Division. 1. Income Tax return for last three years. Agency / Firm should submit the Income Tax clearance certificates along with bid. 2. Trade Licence certificate and Professional Licecence No. 3. Valid registration certificate from CPWD / PWD / MES / Railways / Govt. / Semi Govt. Organizations or any other private Organization of repute. 4. List of organizations where D.G.Sets are installed on hire basis in last three years with supporting documents such as work order / Letter of Intent. 5. Performance certificate of best two works of similar nature issued by the respective Officer of the Deptt. / Company along with Name, Designation and Deptt. to be submitted. 6. Existing agencies providing DG Set services in our Branches/Satellite offices need to apply for empanelment afresh. Non applying for empanelment would mean non selection of the agency. Supporting documents in regard to the above mentioned points should be attached alongwith Technical Bid. No Brokers / Intermediaries shall be entertained. INSTRUCTIONS TO Applicants 1. The application forms will be available from 08.02.2016 to 27.02.2016 between 11.00 AM and 3.00 PM on week days and between 11.00 AM and 1.00 PM on Saturdays ( excluding Holidays and Sundays ) on payment of Rs. 500/- ( Rupees Five hundred only) by Demand Draft / Pay order ( Non refundable) in favour of Life Insurance Corporation of India payable at Gwalior or can be deposited at our Cash counter during working days and CASH hours. 2. The last date for submission of filled in applications is 29.02.2016 upto 2.30 PM. The applications received after the last date and time mentioned above will not be considered. 3. The filled in technical bids for empanelment should be submitted to the address given below : The Chairman, Stores Committee, LIC of India, Divisional Office, Jeevan Prakash Building City Centre Gwalior (MP) 4. The applications will be opened by the Tender Opening Committee on the same day i.e. 29.02.2015 at 3.30 PM in the presence of bidders or their authorized representatives if they are willing to attend. 5. In case the tender form is downloaded from the corporation s web site, the non refundable tender fee of RS 500 (Rupees Five Hundred only) shall be remitted in the form of Demand draft / Pay order drawn in favour of Life Insurance Corporation of India payable at Gwalior. 6. The following documents should be enclosed along with the Application as per Annexure A: (a) Trade Licence Certificate. (b) Professional Licence No. (c) Service Tax Registration Certificate. (d) List of clients with D.G.Set supplied on hire basis with date and location of installation during last three years.(annexure B)

(h) Income Tax return for the last three years. (i) Certified copy of average annual turn over of the firm / agency. (j) Sales Tax / VAT Registration and Clearance certificate. (k) PAN Card. (l) Category under which empanelment is applied for (Annexure D) If any of the above documents are not enclosed alogwith the APPLICATION by the Firms / Agency such firms shall be declared as Non Bonafied / Non Eligible. 7. All the pages of the application are to be signed by the authorized signatory of the tenderer. Any over writing or use of white ink is to be duly initialed by the tenderer. The Corporation reserves the right to reject the incomplete bids and bids lacking in details and without signatures. 8. Applications received late, not in properly sealed cover, received telegraphically & Conditional or not conforming with the prescribed conditions or not complete in all respects will be rejected. 9.. If any of the dates mentioned above happens to be a holiday, the transaction will be made on the next working day. 10. Issue of tender document does not mean that the Firm / Agency is considered qualified. 11. The Tender Inviting Authority reserves the right to accept any application or to reject any or all applications at his sole discretion without assigning reasons thereof. 12.
