MnDOT Cntract N. 1026491 Prject Overview The gal f this prject is t investigate rehabilitatin feasibility and perfrm assciated design services fr six steel arch culverts with stne headwalls, fur f which are lcated in the histric district f Whitewater State Park. The bridge numbers and lcatins f the culverts are: - Bridges 5835 and 5836, TH 74 ver a stream, 2.4 miles suth f the junctin at Cunty State Aid Highway (CSAH) 26 - Bridges 8592, 8593, 8594, and 8595, TH 74 ver a stream, apprximately 5.5 miles nrth f the west junctin at TH 14. The prject is scheduled fr a January 2019 cnstructin letting thrugh State Prject N. 8508-38. Prject Backgrund Bridges 8592, 8593, 8594, and 8595 are lcated with the bundaries f the Whitewater State Park Histric District Bundaries; Bridges 5835 and 5836 are utside the bundaries. Since the histric district is listed n the Natinal Register f Histric Places, the prject is subject t review under Minnesta Statute 138. Subpart 138.665 requires State t seek t avid, minimize r mitigate any adverse effects t the histric district and its cntributing resurces. The General Bridge Management Plan fr histric bridges can be fund at: http://www.dt.state.mn.us/histricbridges/bridge/misc/mgmtplan.pdf Task 1 - Prject Administratin (Surce Cde 1010) 1.1 Cntract Administratin Cntractr will cnduct the administratin f the prject, which will include cmmunicatin with State. Als included will be invicing, supplemental agreements, cst and schedule updates, billing preparatin, and ther nn-technical wrk. State will prvide a Prject Manager t prvide directin t Cntractr s activities. It will be the respnsibility f the State s Prject Manager t receive the wrk prduced by Cntractr, review the wrk fr accuracy and cmpliance with standards, and cllabrate with State s Cultural Resurces Unit (CRU), State s Bridge Office, and stakehlders. N changes in Cntractr s prject management persnnel will be made withut a written ntice being prvided t State. State will ntify Cntractr in writing if there is any change in the assigned Prject Manager fr State. Prgress reprts will be submitted mnthly t State describing the tasks and/r deliverables cmpleted during that mnth, the hurs spent by task during that mnth, the cumulative hurs spent t date, and a cmparisn f hurs spent t budgeted hurs. 1.2 Kickff Meeting Cntractr will meet with State District 6 staff, State s Prject Manager, CRU, the Prject Histrian, and thers as needed, in a kickff meeting t discuss general scpe and timetable. Cntractr will present a prpsed prject timeline (with identified deliverables) fr the grup t discuss and refine as needed. Duratin f State reviews f Cntractr s deliverables will als be established as part f the prject timeline discussin. - 1 -

MnDOT Cntract N. 1026491 1.3 Cultural Resurce Crdinatin Cntractr will cllabrate with State s Prject Manager, CRU, and the Prject Histrian wh will be cntracted by CRU. The cllabratin will be dne frm the nset f the cntract t cmpletin f the final deliverables t determine the effrt needed t ensure that the design cmplies with the Secretary f the Interir Standards fr the Treatment f Histric Prperties (SOI Standards) thrugh either the rehabilitatin f the bridges, r the design f replacement bridges that meet the SOI Standards fr new cnstructin within a histric district. 1.4 Stakehlder Crdinatin Cntractr will meet with State s Prject Manager, alng with the apprpriate stakehlders, t discuss strategies and details at key times during the prject s life. The Minnesta Department f Natural Resurces (MnDNR), as the wner f the park, will be a key stakehlder in the prject. All crdinatin with the Minnesta Histric Preservatin Office (MnHPO) will be thrugh CRU, thugh Cntractr may be required t attend up t fur meetings with CRU, the Prject Histrian, and MnHPO. Scheduling meetings and taking meeting minutes are Cntractr s respnsibility. Task 2 - Rehabilitatin Feasibility Studies and Develpment and Evaluatin f Prject Alternatives (Surce Cde 1071) The gal f the rehabilitatin studies is t investigate rehabilitatin feasibility and explre any alternatives that wuld satisfy the Minnesta Histric Sites Act requirements t seek avidance and minimizatin alternatives. The primary need f these studies is t address structural deficiencies s that the traveling public cntinues t have sund bridge crssings at these lcatins. Cntractr s task is t perfrm feasibility studies f bridge rehabilitatin fr each bridge within the histric district that will ultimately lead t the selectin f preferred rehabilitatin alternatives in an apprved envirnmental dcument. State anticipates that the rehabilitatin study phase f the prject will be cnducted frm Nvember 2016 May 2017. 2.1 Data Cllectin and Site Visit Cntractr will review all available infrmatin and data relating t the prject, including rutine inspectin reprts, cnstructin files, histrical surveys, etc. Cntractr will investigate the prject site, identifying any deficiencies and cnstructability issues. The field review will dcument the issues with phtgraphs and a limited number f measurements. Cntractr will perfrm the site visit in cnjunctin with the Prject Histrian, and will crdinate the site visit with State s District 6 staff, State s Prject Manager, and CRU. 2.2 Design and Preservatin Guidelines Cntractr will use the current editins f the fllwing standards, manuals, and guidance dcuments t aid in the cmpletin f the tasks in this scpe f wrk (in n particular rder): MnDOT LRFD Bridge Design Manual MnDOT Bridge Imprvement and Replacement Guidelines AASHTO LRFD Bridge Design Specificatins AASHTO Manual fr Bridge Evaluatin MnDOT Histric Bridge Dcuments - 2 -

MnDOT Cntract N. 1026491 Design Exceptins and Variances n Histric Bridges: Effective Applicatin and Utilizatin Prject Team Decisin Pints Evaluatin Criteria Rehab Study Management Plan fr Histric Bridges in Minnesta Guidelines fr Cmpleting a Histric Bridge Prject Rehabilitatin Study Reprt Frmat Guidance fr Develping Histric Bridge Rehabilitatin Alternatives 2.3 Evaluatin Criteria and Develp Range f Alternatives Cntractr will facilitate and lead a half-day meeting with State s Bridge Office staff, Prject Histrian, State s District 6 persnnel, CRU and thers t develp the criteria fr evaluating the rehabilitatin alternatives fr the bridges within the histric district. Evaluatin criteria will include purpse and need based criteria, regulatry requirements, and cst based criteria (including life cycle csts). After the develpment f evaluatin criteria, the grup will identify at least ne rehabilitatin alternative that meets the SOI Standards and avids r minimizes an adverse effect t the bridges and the histric district. The alternatives will be develped cllabratively with the Prject Histrian shuld address the bridge cnditin, fcusing n the knwn structural deficiencies and areas f cncern as indicated in the bridge inspectin reprts and ther dcuments, as well as thse nted during the site investigatin perfrmed by Cntractr. The level f detail f the alternatives will be the same fr all alternatives and will be at an apprpriate level f detail fr a prject in the early stages f scping. The range f alternatives being develped will fllw the minimum t high levels f actin as defined in MnDOT s Guidance fr Develping Histric Bridge Rehabilitatin Alternatives dcument. The range f alternatives will be develped t incrprate rehabilitatin measures that avid adverse effects, if pssible. Cntractr will supply all presentatin materials required fr the meeting (i.e. sketches, bards, ther visuals, etc.) and cmmunicatin with stakehlders. Cntractr will cllabrate with State s Prject Manager, State District 6 persnnel, CRU, and the Prject Histrian thrughut the develpment f the alternatives. 2.4 Evaluatin f Alternatives Upn cnclusin f the analysis f the rehabilitatin alternatives, Cntractr and Prject Histrian will facilitate and lead a meeting t present a draft rehabilitatin reprt. Meeting attendees include State s Bridge Office, Prject Histrian, State District 6 persnnel, and CRU. The gal f the meeting is t assess the feasibility f the alternatives and t present a recmmended alternative. If nne f the rehabilitatin alternatives are deemed feasible, State intends t amend this Cntract fr Cntractr t explre ther rehabilitatin alternatives that satisfy the requirements f the Minnesta Histric Sites Act. Cntractr, in cllabratin with the Prject Histrian, will prvide a finalize rehabilitatin reprt after all cmments frm this meeting have been reslved. The draft and the final reprt will use the frmat utlined in the MnDOT Rehabilitatin Study Reprt Dcument. - 3 -

Task 3 - Public Outreach Meeting (Surce Cde 0054) Cntractr will prvide graphic materials and attend tw public meetings. One Cntractr representative will attend public meetings t explain the prject t the public. Task 4 Final Bridge Design and Plan Preparatin (Surce Cde 1250) Based n the results f the rehabilitatin studies, State intends t amend this cntract t include needed design services fr either structural rehabilitatin r replacement. Cntractr will crdinate design activities directly with an assigned Final Design Unit f the State s Bridge Office and the Prject Histrian. The District 6 Prject Manager must be cpied n all significant crrespndence. State anticipates that the design phase f the prject will last frm July 2017 thrugh the summer f 2018. 4.1 Quality Management Plan (QMP) Cntractr will submit a prject specific QMP t State s Prject Manager within five days f Ntice t Prceed with cntracted services. The QMP must specify hw Cntractr will perfrm Quality Assurance and Quality Cntrl (QA/QC) activities thrughut the duratin f the prject t ensure delivery f a quality prduct in a timely manner that cnfrms t established cntract requirements. Cntractr will prepare and distribute the QMP t all prject team members, including subcntractrs. QA/QC must address: - 4 -

Design calculatins and plan sheets must be independently checked and recnciled prir t submittal. Review cmments frm State n Cntractr s varius plan review submittals des nt relieve Cntractr f liability fr an inaccurate r incmplete bridge plan. Cmputer Prgrams All cmputer prgrams and/r spreadsheets utilized by Cntractr must be verified by Cntractr s in-huse Quality Assurance Prgram. Quality Assurance Verificatin Cntractr s Prject Manager r Quality Assurance Manager will review the entire plan design and prductin prcess t ensure the cmpleteness and adequacy f their wrk and cnfrmance with the Cntractr s Quality Assurance prcedures. 4.2 Draft Bridge Preservatin Recmmendatins State will prvide draft Bridge Preservatin Recmmendatins at the prject kick-ff meeting. Cntractr, in cllabratin with State, the Prject Histrian, and CRU, will review the draft recmmendatins and prvide cmments t State. Prpsed repairs must include what is structurally necessary while maintaining the histric characteristics f the structures and cmplying with SOI Standards. 4.3 Final Bridge Preservatin Recmmendatins State will cmplete the final Bridge Preservatin Recmmendatins in cllabratin with Cntractr, CRU, and the Prject Histrian. At the State s Prject Manager s discretin, a meeting may be held t reslve questins and finalize the recmmendatins. 4.4 Plan Preparatin Cntractr will perfrm all required engineering t determine the gemetric, material, and prcedural requirements fr the rehabilitatin wrk fr the structures. Cntractr will prepare Certified Final Bridge Plans and nn-standard Special Prvisins. The Final Bridge Plan sheets will be prepared in accrdance with the fllwing manuals, standards, and dcuments: MnDOT Lad and Resistance Factr Design (LRFD) Bridge Design Manual American Assciatin f State Highway and Transprtatin Officials (AASHTO) LRFD Bridge Design Specificatins MnDOT Bridge Details Manual (Parts I and II) MnDOT Standard Specificatins fr Highway Cnstructin MnDOT Cmputer Assisted Design & Drafting (CADD) Standards MnDOT Summary f Recmmended Drafting Standards MnDOT Bridge Preservatin, Imprvement and Replacement Guidelines Cntractr will als cmply with dcuments related t the treatment f histric prperties. Plan sheets will be prduced using the current versin f MicrStatin. 4.5 Plan Reviews and Cntent State s Prject Manager will attend prject meetings, respnd t Cntractr inquiries and review submittals. Cntractr, in cllabratin with the Prject Histrian, will prepare the fllwing submittals fr review and acceptance by the State Bridge Office: 4.5.1 30% Plan Review The 30% Plan must cntain, at a minimum: General Plan and Elevatin sheet, cmpleted t the extent necessary t shw the prpsed rehabilitatin wrk with general dimensins, elevatins, bridge crss sectin, and basic design data Miscellaneus deck and sidewalk repair sheet(s) Railing Details sheet(s), including cncrete psts Index and Quantities sheet(s) Draft Special Prvisins fr special surface finish The 30% Plan review is an early assessment f Cntractr s prject understanding and cnfirmatin f the prject. Anticipated due date: Octber 2017. Cntractr may prceed with further design during the review perid, and State will return 30% Plan review cmments within ten wrking days. CRU will require a 45 day review perid fr the 30% Plan. Cntractr may prceed with design during this review. 4.5.2 60% Plan Review The intent f the 60% Plan review is t verify Cntractr s prgress tward plan cmpletin and evaluate against prject and cntract timelines. The 60% Plan must include a full set f in-prgress plan sheets. Anticipated due date: January 2018. Cntractr may cntinue with design during the review perid, and State will return 60% Plan review cmments within ten wrking days. CRU will require a 45 day review perid fr the 60% Plan. Cntractr may prceed with design during this review. 4.5.3 95% Plan Review The intent f the 95% Plan review is fr State t verify that the plan is acceptable fr the State Bridge Engineer s Signature. The 95% Plan must be cmplete in all areas t the extent that in can be certified by Cntractr, althugh a certificatin signature is nt required until after this review has been cmpleted. State will return final review cmments within ten wrking days. Anticipated due date: June 2018. At this design stage, it is anticipated that CRU will issue a findings f effect n the design, and then MnHPO will require 30 days t review the 95% Plan. Cntractr must verify this with CRU and the Prject Histrian. - 6 -

MnDOT Cntract N. 1026491 4.5.4 Final Certified Bridge Plan Cntractr is respnsible fr the cmpleteness and accuracy f its wrk. Design calculatins, plan details, pay item quantities, and final cst estimate must be cmpletely checked prir t submittal. Cntractr will submit the Final Certified Bridge Plan t State. If State determines that majr plan revisins are necessary at any stage in the review prcess, Cntractr will submit revised review plans and cmputatins within five business days.
