This booklet summarizes the employee benefit options the City of Memphis provides part-time employees with 12-months of service, normally scheduled 20 or more hours per week. These benefits and perks are available to all employees with a valid City of Memphis employee badge. This summary has been prepared by Human Resources professionals, and provides you with information regarding part-time City of Memphis employee benefits. If there is a difference between this booklet and documents any will plan control documents, and the plan supersede this summary. Letters from the Mayor & Chief HR Officer Dear colleague: Greetings City of Memphis part-time employees, I m proud to be able to send this offering of benefits to you today. It s the first time in City of Memphis history that we ve extended our benefits offerings to part-time employees, and the reason is simple: You re a big part of our mission, every single day. I am excited to announce the City s first step into providing select package of benefits to our eligible part-time employees, those normally scheduled 20 or more hours per week, who have been employed by the City a year, effective November 1, 2017. This will be the first time the City has offered benefits to part-time employees. This is a major step toward showing you that we mean it when we say that. Rest assured, this is just the first step. Our Division of Human Resources is tasked with seeking ways to enhance this in the coming years, so I encourage you to stay tuned. In the meantime, I hope this offering of benefits will be of assistance to you and your family. Employer Paid Benefits Employee Clinic If you are an eligible, active part-time employee, you do not have to pay a copayment or a deductible when visiting the clinic. Additionally, the clinic has a limited supply of medications in stock at no cost to you. This will go into effect November 1, 2017. **Effective Jan. 1, 2018, the clinic will be located at 1520 Union Ave. and is open Monday Thursday 8 AM - 7 PM, Friday 8 AM - 5 PM, and Saturday 8 AM - noon.** Note: Take comfort in knowing your medical information is protected by HIPAA privacy laws and is not shared with the City of Memphis. On-site Total Wellness Center A service that is free for ALL active City of Memphis employees! Become a member of the Total Wellness Center located at 125 North Main on 2B. The center is equipped with weights, power rowers, elliptical machines, treadmills, stair climbers, and much more! You may contact the Wellness Coordinator at 901-636-1375 for enrollment information. Death Benefit All eligible part-time employees receive a Death Benefit of $10,000. $5,000 is provided by Standard Insurance Company and $5,000 is paid for by the City of Memphis. This will go into effect November 1, 2017. 4
Supplemental Benefit-available January 1, 2018 Short Term Disability (STD) Short Term Disability insurance pays a weekly benefit in the event you cannot work due to a covered Illness or injury. An STD benefit replaces a portion of your weekly income, providing funds directly to you, to help pay your bills and living expenses. case of a discrepancy between the plan documents and this guide, the plan documents will prevail. Benefits are subject to change without notice. Eligibility: Full-time permanent, active employees of City of Memphis over the age of 18. Premium: You pay 100% for this coverage through payroll deduction. **Your Benefit begins after a 14 day waiting period and will pay a maximum of 166 days** 5
Voluntary Benefits Accident Insurance Accidents can happen anytime, anywhere. Accidents are usually followed by a series of bills. Even if you have good insurance, you may still have to cover out-of-pocket costs, such as: Doctor bills Ambulance fees Hospital expenses Accident Insurance from Colonial Life & Accident Insurance Company can help protect you, your spouse and your dependent children from the unexpected expenses of an accident. Features of Colonial Life s Group Accident Insurance: You are paid benefits to help you with the care and treatment of a covered accidental injury. Your benefits are paid directly to you, unless you specify otherwise. You are paid benefits regardless of any other insurance you may have with other insurance companies. You can take your coverage with you, even if you change jobs or retire. Talk with your benefits counselor to learn more about accident insurance and how it can help protect what you ve worked so hard to build. Voluntary Benefits Cancer Insurance Colonial Life s Cancer Insurance Plans can help offset the out-of-pocket medical and indirect, non-medical expenses related to cancer that most medical plans don t cover. Our coverage provides benefits you can use to help with: Loss of income Childcare Lodging and meals Deductibles and copays If you or a loved one needed treatment for cancer, how would you cover all of the expenses? Health insurance can certainly help, but what about all of the other expenses that your health plan may not cover? Cancer insurance can help you pay for necessities such as: Care at out-of-network specialty centers Home health care Experimental treatments Lost income Childcare Travel, lodging, and meals during treatment Deductibles and copays Coverage is subject to policy exclusions and limitations that may affect benefits payable. Voluntary Benefits Critical Illness Critical illness insurance compliments your major medical coverage by providing a lump-sum benefit if you re diagnosed with a covered critical illness, such as: Heart attack (myocardial infarction) End stage renal failure Coronary artery bypass graft surgery Stroke Major organ failure You can use this benefit to help cover everyday living expenses or other costs related to your critical illness, such as home modifications, travel expenses and rehabilitation charges. You may receive additional benefits if you re diagnosed with more than one critical illness. Features Coverage is portable you can continue coverage if you change jobs. Benefits are paid in addition to your medical insurance. Benefits may be used, however the covered person chooses. Voluntary Benefits Term Life Insurance Help protect the people who depend on you. If something happened to you, the last thing your family should have to worry about is financial burdens. Plan for the future with term life insurance from Colonial Life & Accident Insurance Company. The advantages of term life insurance Level death benefit. Lower cost option compared with cash value insurance. Coverage for specified periods of time, which can be during high-need years. Benefit for the beneficiary that is typically free from income tax. Benefits and features Guaranteed premiums do not increase during the term. Coverage is guaranteed renewable to age 95, as long as premiums are paid when due. You can convert it to cash value insurance. Portability allows you to take it with you if you change jobs or retire. An accelerated death benefit is included.
