Commercial Lines Quotes Once a completed application is received by underwriting, it will be input into the web application, rated and a quote issued. Agent will receive an email diary from the underwriter indicating that the quote is available for review. Agent will access the website with their User Name and Password at: https://secure.calmutual.com/siw/agents/frmlogin.aspx (the link is also in the email.) Page 5 of 30
You may access the quotes several ways: o Current Status New Diary find the diary item to go to quote Submissions Pending find the insured and click on App/Form No o Submissions Tab at top of page Find the insured and click on App/Form No Page 6 of 30
You are able to view and revise the following: o Application Info tab Quick Quote Applicant data Additional named insureds Business Info Billing info Policy Premium Summary Page 7 of 30

Print Quote This proposal is to present to the insured, including: Property Locations, Coverage Parts, Premium Summary and Authorization to Bind Coverage. (You may also print the quote from the top of the header) See next page for an example Page 12 of 30

o Request Issuance of Policy Review quote thoroughly; confirm payment plan, make any necessary revisions; review policy summary and or quote proposal. Request to issue policy can be done several ways: Request Issue at top of header Bind/Print Evidence at top of header Respond to diary include total premium in diary notes Email the underwriter include total premium in body of email Page 21 of 30
Personal Lines Complete Application On-Line Select the Homeowner or Dwelling Fire icon under the Create New Submissions on Home page Complete all Yellow Fields for a Quick Quote Complete all additional Blue Fields for a Complete Quote prepared to Issue Property/Cvgs tab includes an icon of a blue house on left side of screen click to enter the ISO Home Value replacement cost estimator. The more complete the estimator the more accurate replacement cost value. Select Calculate and select Print if needed. Close estimator by x upper right corner of screen. After closing properly many of the values will populate into the remaining application. Selected coverages, limits, complete forms if required, rate, print quote for your insured. If quote is accepted, request issuance or Bind/Print Evidence and the policy will be processed. Refer the Commercial Quote section for further detail on the individual tabs. Refer to the Policy Issued section following for further information once the policy is issued. Policy Issued Once a policy is issued there is a wealth of information on the web application for your agency s use. o Submissions tab select policy from list clicking on the Policy No Summary basic policy information including term, policyholder, agent, risk, coverages, limits, deductible and premium. You may print a COI or Loss History from this tab. Page 23 of 30
Policy Docs PDF of Declarations, forms and general correspondence (Some agencies no longer request copies of the policy as they can be accessed, printed, emailed or saved to their local computer) Page 24 of 30
Accounts Pay plan, invoices and payment activity on policy. Invoices can be reprinted from this tab by selecting the PDF icons. Use the Print Page at the top of header to print all activity. Claims Claims activity including claim number, type, loss date, status and paid amounts Page 25 of 30

General Web Application We will be happy to provide on-line demonstrations of the web application system by contacting California Mutual. Please set aside approximately 30 minutes to participate in the general demo. Or if you have specific questions please don t hesitate to call. You must enable (allow) pop-ups from our website. If pop-ups are blocked you will not be able to generate PDF s etc. If you need further information on each tab if you select Help at the top right of the screen further information about each tab will display. On many screens you may see Refresh Print Page near the top of the header. If you change filters you must Refresh. Select Print Page to do just that. At the top of the page are several tabs: Home this is the opening page from where you can check diaries and submissions pending. Please do not create a new commercial submission from this page. Email or fax applications and our underwriters will complete. Personal lines can create submission from the home tab. Change your password, link to CA Mutual Website Page 27 of 30
Diary - Review all diaries from this tab Submissions Listing of all submissions and policies for you as an agency or you can filter for all agents within your agency. There are several additions you can filter further. App/Forms New submissions for personal lines only Page 28 of 30
