Overview Welcome to Coventry Health Care! As part of your employment with Coventry, you will be using Coventry s Automated Benefits System for New Employees ( CABS-NE ) to elect benefits. This user guide highlights important steps employees must take to complete the election process. To access CABS: 1. Connect to Coventry today home page and click on Employee Benefits in the horizontal navigation bar 2. On the Employee Benefits home page, click on Benefits Enrollment for New Employees button on the right hand side of the screen As a newly hired employee you are required to: Confirm Personal Identity and Job Information Review all Coventry Benefits provided Add Dependents and Beneficiaries Enroll in Coventry s benefits plans within 31 days of you date of hire Save and Submit your elections to Human Resources Provide documents to Human Resources required to verify all dependents
Initial Log-in Screen Prior to completing benefit elections employees are required to: Confirm identity If the information presented is correct click Continue If the information presented is incorrect click Close and contact your HR Representative
Welcome Screen This screen contains important information when using CABS- NE. CABS Menu Your Election Period End Date (31 days including date of hire) Navigating the system Saving Election entries Printing Elections Individual plan screens are informational only upon initial entry to the system. Place your cursor over Benefits Options and Costs link within the menu for instructions.
Personal Information Screen Employees will need the following information to add dependents and beneficiaries. Date of birth and social security numbers for all dependents The addition of at least One Beneficiary If you add, edit or delete any Dependent or Beneficiary information, you must click SAVE for the system to record your changes: You must select "Save - Calculate Eligibility and Costs for the Plans" to calculate the plans, appropriate coverage levels, and costs for the plans you are eligible to enroll in.
Personal Information Screen (cont.) Once you click the Add Dependent or Add Beneficiary button, the Add Dependent or Beneficiary screen will open. Complete all required fields Select dependent Relationship from the drop down menu Select gender Enter birth date (system will auto calculate age) Respond to pop up messages, e.g. overage dependents Insert record to add dependent System will add dependent record to Dependent and/or beneficiary tables on the Personal Information Screen
Benefits Options and Costs Screen Access to this screen is allowed only after successful completion of the Personal Information screen. Benefit Plan choices and biweekly premiums listed are based on the following: Employment status (full or part time, temporary or regular) Regularly scheduled hours Assigned Work location Contact Human Resources if you have any concerns regarding your eligibility for a specific plan.
Benefit Plan Enrollment Plan screens are active for data entry upon completion of the Personal Information screen. Please notice the following: Dependents and Beneficiaries are displayed Plans you are eligible for default to WAIVE, until your election is made Option and Coverage levels ready for election Auto enrollment at 6% in the 401K plan Based on your status, auto enrollment in plans fully paid by Coventry Plans you are not eligible for are disabled with a default option of Not Eligible
Dependent and Beneficiary Tables Uploaded from Personal Information Screen (PI) Dependent Table To add, edit or delete dependents return to PI screen. Remember to always Save-Calculate Eligibility and Costs for plans. Uploaded from Personal Information screen (PI) Beneficiary Table To add, edit or delete Beneficiaries return to PI screen. Remember to always Save- Calculate Eligibility and Costs for plans. To enroll your dependent(s) in the plan, simply check the box next to the dependent(s) name, or select the applicable coverage level from the drop down menu. In order to complete the enrollment of your dependents, you must provide documentation required as proof of dependent eligibility to Human Resources. To assign a beneficiary type (primary or contingent) or a percentage amount to each beneficiary, simply enter the percentage amount in the applicable column. Total percentage per beneficiary type (primary or contingent) cannot exceed 100%.
Remember to Save Elections Option 1 Always SAVE entries SAVE each individual screen when data entry is complete Utilize the SAVE button on CABS Menu Option 2 SAVE before you EXIT Utilize SAVE and SUBMIT Elections on the Elections Report screen. Note: Once you Submit your elections, you will need to contact HR to make any additional changes during your election period All entries must be accurate prior to submission. When all screens are correct, click OK to submit for processing
Election Report The Election Report provides a summary of : Dependents and Beneficiaries added but still pending verification All plans and coverage levels elected Bi-weekly payroll deductions for each plan Prior to Submitting elections: Review plans elected, including all dependent and beneficiaries enrolled To make any changes, simply return the applicable screens After completing your changes, click the Save button on the menu bar Return to the Elections Report to confirm that the system captured your changes
SUBMIT ELECTIONS At this point you have successfully completed Part One of the Election Process. You cannot make any changes in CABS after you Submit your Elections. If changes are needed prior to the Election End Date you must contact Human Resources. Part two of the Election Process. If you elected coverage for your dependents in a Coventry plan, you must provide required documentation as proof of dependent eligibility to Human Resources before your election period expires. Vendors cannot be notified of your plan election until your dependents are verified. Payroll deductions start on the next scheduled pay date after your enrollment is complete and all dependents have been verified. Deductions are retroactive to your date of hire, so any delays in the process will result in multiple deductions being taken from your pay.
SUMMARY The enrollment process must be completed before the Election Period End Date. Part One -Employee: Complete, Save and Submit plan elections and provide HR with applicable documents to verify dependent enrolled. Part Two HR: Verify eligibility for all dependents with documents provided by employee. On day 32, the day after Your Election Period End Date, if your enrollment process is not complete CABS-NE will: First: Default all plans to Waive, when you do not to submit your elections to HR with the exception of: plans fully paid by Coventry based on job status, and default enrollment for the 401(k) plan ( 6% deferral) Second: Default any Dependent to a Beneficiary that you have not provided valid documents to HR in order to verify the dependent s eligibility for coverage: coverage levels will default to the next highest level, and dependents who were not verified will be excluded from the plans

Important Note: Coverage is effective as of an eligible employee's first day of active employment if elected within 31 days of such date. IRS regulations require that your enrollment be completed before the Election Period End Date, and these elections remain in effect for the entire calendar year. You may only change elections during the year if you experience a qualified change in status, as defined by IRS regulations.
