Fields with an asterisk* are mandatory and must be completed before new business can be quoted or submitted: *Agency/Branch-Select the agency code / location and or discounts for the policy to be issued under. *Policy Form Select the appropriate form: HO3, 6288, HO4 or HO6. *Effective Date Enter the policy effective as MM/DD/YYYY. The application will automatically populate the expiration date to one year from the effective date. Is there an auto insured with Mt. Washington or another Plymouth Rock Company? Eligibility-Pet Information Does the applicant or any tenant have any animals or exotic pets under their care, custody or control? If yes, select type. Roofing Information *What is the age of the roof? - required on HO3, 6288 and HO6 forms *Type of Roof required on HO3, 6288 and HO6 forms Select Next to advance to the Plumbing Information screen. Optional Coverages Coverage Categories: Select Section I-Property, Section II Liability, Company Specific or Scheduled Personal Property *Coverage Type: Depending on the category selected the appropriate list of endorsements will display. Select the endorsement you would like to add. For additional coverages select +Add Another Coverage at the bottom of the screen. Select Rate to obtain a quote. Page 12 of 33
Rating The Premium Summary page will give you a calculated premium. Select previous or any tab to change information on policy and re-rate. If you choose not to submit the policy you may select decline. Select Bind Quote to advance to Additional Interest screen. Page 13 of 33
Additional Interest To select Next to advance to the Billing screen. Billing *Bill Type: Direct Bill, EZ Paid, Mortgagee *Payment Plan: *Down Payment Method: WebPay, Insured Check (bill insured). To issue the policy select Submit. Page 14 of 33
Final Steps Webpay button will appear if Web payment was selected as Down Payment Method. Select WEBPAY to process down payment. New Hampshire Homeowner Acord Application is available to print for submitted policies. Page 15 of 33
*To Endorse a Policy: Under New Business-Home, select Endorsements. You will be brought to the Endorsement Search screen. Page 16 of 33
ENDORSEMENT SEARCH Enter the policy # or the insured s first and last name to retrieve the policy to be endorsed. Select Start Search to advance to the Search Results screen. SEARCH RESULTS Confirm the policy number and policy term in the search results. Select Endorse to begin entering the endorsement and advance to the Endorsement Details screen. Page 17 of 33
ENDORSEMENT DETAILS Enter the *Endorsement Effective Date and select Next to advance to the Applicant screen. Note: You will not be able to do out of sequence endorsements. Those will need to be sent in to Customer Service for processing. ENDORSEMENT TO CANCEL POLICY To cancel a policy, enter an *Endorsement Effective Date, Select Cancel Policy and select a *Cancellation Reason. Then select Cancel Policy Page 18 of 33

Add additional interest Select ADD ANOTHER ADDITIONAL INTEREST: o Type: Select type of additional interest, First Mortgagee, Second Mortgagee, Third Mortgagee, Additional Insured, or Additional Interest. o * Name, *House#, Street, Apt#, *City, State, Zip Code: Edit additional interest Select View/Edit for the appropriate interest. Delete additional interest Select Delete for the appropriate interest. If the policy is Mortgagee Billed, you will need to forward this request to Customer Service for processing. RATING The premium summary displays the old and new annualized premiums. Select Next to advance to the Billing screen. Page 24 of 33
SUBMIT/BILLING *Bill Type, and *Payment Plan- Pay plan changes are not available for processing at this time. Please forward requests for mid-term pay plan changes to Mt. Washington Customer Service for processing. Select Exit if the endorsement should not be submitted. Select Submit to process the endorsement and advance to the Confirmation screen. CONFIRMATION-Endorsements o If the endorsement was successfully processed, a message indicating such will be displayed. Page 25 of 33
To retrieve an existing homeowner quote select Search Quotes under New Business-Home. Search Quotes You can search for a quote by Last Name, First Name, or quote number. Page 26 of 33
Select Submit to advance to the Search Results screen. Search Results Edit Application-allows you to modify the current quote. Copy and Edit Quote-allows you to have different variations of a quote. Web Pay Search. To make a payment you can search by last name, first name, or Policy Number. Select Start Search to advance to the search results. Verify the policy Name and Address, policy number, and effective date of the policy. Select Webpay to make the payment. Page 29 of 33

Format MM/YYYY *Credit Card CVV#: Typically the 4 numbers on the back of the credit card. *Amount: Dollar amount of the web payment. Check off box to certify that you are authorized to make payment. Select Submit for Payment Confirmation Electronic Withdrawal Select from the dropdown menu *Account Type Checking, Savings, or Money Market. *Bank Routing Number - This is always 9 digits. Typically this is the first set of numbers on a personal check. *Bank Account Number - Typically the second set of numbers on a personal check. *Re-verify Bank Account #. *Amount - Dollar amount of Web payment. All payments made by 5:00 p.m. on a non-weekend day or holiday will be posted the same day. Most withdrawals from the designated bank account will occur on the following business day. A copy of this screen may be printed out for the insured s records. You can search for payments made on a policy by the Customer Information, Policy Number or Confirmation Number. You can cancel a payment on the same day it was submitted. You are unable to edit an existing payment, if the payment is incorrect please cancel and process another web payment. Page 33 of 33
