Guaranteed Loyalty Additions: The Guaranteed Loyalty Additions is paid basis the Policy Term chosen by the customer. The Guaranteed Loyalty Additions is allocated as a percent of initial Annual Target Premium (ATP). Family Income Benefit We understand that after death of the breadwinner, the family needs regular support for certain unexpected expenses to enable them to meet these sudden expenses, post the death of the Policyholder. We will pay the nominee an amount equal to 10% of sum assured every year for 10 years or till original policy maturity date (whichever happens earlier), subject to a maximum of 100% of Sum Assured without any requirement to pay future Premiums. Take 3 easy eteps towards building a bright future for your family Step 1: Choose your premium Annual Target Premium or ATP means the level premium payable by you in a Policy Year by regular installments in the amounts and on the due dates. The Annual Target Premium (ATP) can be paid under any of the following options: a) Level Premium: Under this option, the ATP will remain unchanged, throughout the Policy Term. b) Increasing Premium: Under this option, the ATP will increase by an amount equal to 5% of the initial ATP on each Policy Anniversary after the effective date of the Policy. In this case, the sum assured shall also increase @ 5% per annum of the initial sum assured on each Policy Anniversary. The minimum premium you can choose is Rs. 24,000 per annum. There is no cap on the maximum premium payable. In addition to this, you may even want to invest a lump-sum as and when you have it in the form of Top-ups subject to the maximum limit. The Top-up payments do not increase the insurance coverage on the Policy. Note: Increase or decrease in premium amount is not allowed during the Policy Term and there is no changeover allowed between Level and Increasing Premium option once the Policy is in force.

Step 2: Choose your Policy Term You have the option to select the Policy Term as per your convenience. To enable you to match the maturity with your financial goals, you have the option to choose one of three policy terms, i.e. 10 years, 15 years or 20 years. Step 3: Choose your Sum Assured You can choose the amount of protection you want with a wide choice of Sum Assured multiple which can be a whole number between 5 to 30 times the annual Premium. However, at higher ages, a higher cover multiple may not be sustainable due to a high cost of insurance. Sum Assured = ATP X Cover Multiple Maturity Benefit: On maturity, the Policyholder will be eligible to an amount equal to Fund Value where the Fund Value will be calculated as: Fund Value = (Accumulated Units (including Top-ups) * prevailing NAV) At Maturity, along with your fund value you will get Guaranteed Loyalty Additions: Depending upon the Term chosen Subject to all due premiums have been paid by the policyholder and Life insured being alive If the policyholder opts for cover continuance option, then Guaranteed Loyalty Additions will not be paid Death Benefit: Incase of Death of the Life Insured before maturity of Policy, the following benefits shall apply: 1. Amount equal to higher of prevailing Sum Assured or prevailing Fund value 2. Payout s applicable under the Family Income Benefit (Note that the policy will only continue for the FIB payouts, no other benefits are available post the death of Life Insured and no Premiums will be payable or accepted by the Company after the death of Life Insured) Illustrative Example: Amount(in Rs.) The Fund Value shall include the Guaranteed Loyalty Additions, if applicable.

Balanced Fund: Provides balanced returns from investing in both fixed income securities (to target stability of returns) as well as in equities (to target growth in capital value of assets). Dynamic Opportunities Fund: Provides potentially higher returns to the Policyholder by dynamically investing in equities, debt or cash instruments to capitalise on changing market conditions. Fund Name and Objective Government Securities Corporate Bonds Money Market & Cash Instruments Equity & Equity Related Securities Potential Risk/ Reward Conservative Fund: Provides stable return by investing in assets of relatively low to moderate level of risk. The fund will invest primarily in fixed interest securities such as government securities, corporate bonds, etc. However, the fund may also invest in equities. 50%-80% 0%-50% 0%-40% 0%-15% Low Money Market Fund: Provides stable returns by investing in short term money market instruments like treasury bills and cash. The declared NAV of the fund is guaranteed to be not less than the declared NAV of the previous day. The fund will invest exclusively in fixed interest securities such as government securities, corporate bonds, etc. 50%-100% 0%-50% 0%-40% Nil Low Or opt for: Dynamic Fund Allocation This investment option enables you to plan your investments basis your life stage. Through this option, you don t have to opt for any fund manually or do any switch/redirection manually, which happens automatically. Dynamic Fund Allocation endeavours to safeguard your fund from any capital erosion, as the Policy progresses and nears maturity. It basically is a strategy where the fund is invested mainly in equity towards the start of the Policy and gradually shifts from equity to debt. This option can be taken at the stage of proposal after which the Company will effect automatic allocation/switch of Premiums received to Funds as per a pre-determined proportion given in the table below. There are no charges or processing fees if you opt for Dynamic Fund Allocation. You may opt out of the Dynamic Fund Allocation only once during the Policy Term and the change will be effective on the next Policy Anniversary. Once opted out, you cannot opt again for the Dynamic Fund Allocation. To start with, 75% of you net investable Premium is invested in the Growth Super Fund and 25% in Secure Fund. As the Policy progresses, and the years to maturity remain as 15 years, the asset allocation is changed to 60% and 40% in Growth Super Fund and Secure Fund respectively. Similarly, as the Policy nears maturity, majority of the funds (80%) are invested in Secure Fund and 20% is invested in the Growth Super Fund. Flexibilities offered: Top-ups: You can invest extra money in your Policy through lump sum Top-ups at any time and any amount you want, subject to a maximum of 25% of total Premiums paid. However, Top-ups are subject to a lock-in period of 3 years except the Top-ups made in the last 3 Policy Years. Switching: You may switch Units between available Funds at any time during the term of the Policy, subject to a minimum switch amount of Rs. 5,000. Following receipt of your written notice, we will cancel Units in the Fund you wish to exit from and purchase Units in the Fund you wish to enter. We may at any time impose a complete or partial ban on switches for a time period. We shall determine (but not exceeding 30 days) if, in our sole and absolute discretion, we consider this to be in the interests of maintaining the stability of the Funds or necessary to protect the interest of Policyholder. Such a situation may arise under extraordinary circumstances such as non-availability of market prices, occurrence of any catastrophe, etc., where the declaration of unit price is not possible. No switiching is allowed in case of Dynamic Fund Allocation option. Premium Redirection: You may redirect your future premiums between available Funds at any time by giving us 7 days prior written notice. If you choose to redirect your future premiums to more than one Fund, the percentage of such premiums redirected to each Fund shall

be subject to a minimum percentage limit at the relevant time. Your notice must quote your Policy number and it must specify precisely the Fund in which you wish to redirect the premiums. Partial Withdrawal: An investment under insurance plans is viewed on a long-term horizon and the maximum benefits are also reaped if you stay invested for longer duration, therefore, for the first three policy years, the plan is not eligible for a partial surrender. Therefore, you can make lump-sum partial withdrawals from your funds at any time after the policy has completed three years provided the life insured has attained 18 years of age and within the policy term chosen. The maximum amount of partial withdrawal allowed in any particular year after first 3 policy years is 20% of the fund value subject to the fund value immediately after partial withdrawal being not less than 1.5 times of the the Annual Target Premium. The Sum Assured shall be reduced to the extent of partial withdrawals made during the two years period immediately preceding the death of Life Insured. However, on attainment of age 60 by the Life Insured, all partial withdrawals made within two years before attaining age 60 and all partial withdrawals made after attaining age 60 shall be set off against Sum Assured payable on death. Surrender: In case you decide to discontinue the plan due to any reason, you may do so by giving us a prior written request to surrender this policy at any time after the completion of first Policy Anniversary, provided an amount equal to two ATP s has been paid by you. Upon our receipt of your signed request for surrender of the Policy, this Policy will immediately terminate and the Unit Account shall be closed. However, the surrender value if any prevailing at the time of affecting the surrender shall be paid only after the completion of third Policy Anniversary. Charges under the Policy 1. Premium Allocation Charge Premium Allocation Charge (as a % of ATP/Top-up): Premium Band Year 1 & 2 First Rs. 50,000 30% Rs. 50,001 Rs. 1,00,000 20% Rs. 1,00,001 & above Nil Allocation Charge Year 3 and onwards: On the first Rs. 100,000, the allocation charge will be 5% and no allocation charge will be levied on premium amount above Rs. 100,000. Example: If your ATP is Rs. 200,000, the allocation charge is calculated as below: On first Rs. 50,000: 30%, which will be equal to Rs. 15,000 On next Rs. 50,000: 20%, which will be equal to Rs. 10,000 On the premium amount over and above Rs. 100,000: Nil Hence, the total allocation charge levied will be Rs. 25,000 on the entire Rs. 200,000 ATP. The allocation charge on Top-ups will be 2% of the Top-up Premiums received. 2. FUND MANAGEMENT CHARGES: In the long run what makes your investment returns look impressive is the way your funds are managed. Max New York Life s expertise in managing your funds is available to you at a nominal charge. This is a charge levied as a percentage of the value of assets and shall be appropriated, usually daily, by adjusting the Net Asset Value of the fund. The annual rate of fund management charge is as below. This charge may increase in future after clearance from the IRDA. Growth Fund: 1.25% Growth Super Fund: 1.25% Balanced Fund: 1.10% Dynamic Opportunities Fund: 1.25% Conservative Fund: 0.90% Money Market Fund: 1.25% Secure Fund: 0.90%

3. Other Charges Charges Explanation Explanation Policy Administration Charge Rs. 60 per month increasing @ 5% compounded annually. On each Monthiversary, an appropriate number of units, including a part thereof, in the Unit Account will be cancelled at their Unit Price. Mortality Charge for providing risk cover to the Life Insured shall be recovered. Where the sum at risk will be a sum total of the following benefits: Policy Sum Assured (plus PAB rider, if opted) + sum at risk on account of Family Income Benefit. Mortality/ Morbidity Charge On each Monthiversary, an appropriate number of units, including a part thereof, in the Unit Account will be cancelled at their Unit Price to meet mortality for the life insurance and other benefits. Please refer to the below mortality rates per Rs. 1000 sum at risk for some sample ages: Age 25 30 35 40 Mortality charge (Rs.) 1.57 1.61 1.82 2.49 Morbidity charge will be levied only if you have opted for the Dread Disease Rider. On each Monthiversary, an appropriate number of units, including a part thereof, in the Unit Account will be cancelled at their Unit Price to meet Morbidity Charges. Switching Charge Redirection Charge Maximum 12 switches allowed in a Policy Year and these would be free of charge. The minimum switch amount will be Rs. 5,000/-, which can be altered by the company from time to time. Maximum 12 redirections allowed in a Policy Year and these would be free of charge Maximum of 12 Partial Withdrawals allowed in a Policy Year and these would be free of charge. Partial Withdrawal Charge The minimum amount for which request for partial withdrawal of units shall be entertained is Rs. 5,000. Maximum Partial Withdrawal is 20% of Fund Value subject to the Fund Value immediately after Partial Withdrawal being not less than 1.5 times ATP. Note: No Partial Withdrawals are allowed till the Policy completes 3 Policy Years. Please Note: Service Tax and Education Cess is payable at the applicable rates on all charges except Surrender Charge. All Charges are guaranteed and shall not increase during the term of the Policy subject to the limits specified and subject to clearance from the IRDA. Morbidity charges are subject to change after clearance from the IRDA. You may surrender this Policy at any time after the completion of first Policy Anniversary, provided an amount equal to two ATPs has been paid.

Premium Payment Discontinuation Discontinuation of Premiums after paying the Premiums of at least three consecutive years: a) If after payment of all due premiums of first three policy years, any further premium is not paid to the Company within the grace period (30 days), the policy and all applicable covers will continue till the end of the revival period (3 years) and all charges including Policy Administration Charge, Fund Management Charge, Mortality Charge (if any) and Rider Charge (if any) will continue to be levied during this period. However, at any time during the revival period, if the surrender value of the policy is equal to one initial annual target premium, then the policy will terminate and the applicable surrender value will be paid to the policyholder. b) If before the end of the revival period, the policyholder wants to pay premium, then all due premiums should be paid till date to revive the policy. Alternatively, the policyholder has the option to opt for Cover Continuance Option under which the policy will continue with all the due benefits. Once the Cover Continuance Option is chosen by the policyholder, the company will not accept any further premiums or Top-ups and the policyholder shall not be entitled for the Guaranteed Loyalty Additions. c) If the policyholder fails to pay all due premiums and also fails to choose the Cover Continuance Option before the end of the revival period, then the policy will terminate and the applicable surrender value will be paid to the policyholder. d) However, at any time during the policy lifetime if the surrender value of the policy is equal to one initial annual target premium, then the policy will terminate and the applicable surrender value will be paid to the policyholder. Discontinuance of Premiums within three years of inception of the Policy: a) Should a premium falling due before the third policy anniversary not be paid, then on expiry of the grace period (30 days) the Unit Account will be closed (this means that the units will be disinvested and the monetary value of such units will be held) and any life cover or rider cover will cease. b) The policyholder can revive the policy within the revival period (3 years) from the date of first unpaid premium by paying all due premiums till date. On revival, the Unit Account will be re-opened (this means once the customer revives the policy, the monetary value of the units closed together with the amount of premium paid at the revival date less any Premium Allocation Charge and according to the ratio in which the premium should be allocated in various Funds, as specified by the policyholder, will be used to purchase Units at the Unit Price as at the date of revival) and any life cover or rider cover will recommence. c) An amount equal to the Policy Administration Charges falling due between the date of lapse and the date of revival will be levied at the date of revival by cancelling Units in the Unit Account at the then applicable Unit Price. In case the policy is not revived during the revival period, the policy will terminate and the cash surrender value as at the date of lapse, if any, shall be paid at the end of the third policy anniversary or at the end of the revival period, whichever is later. Grace Period and Revival of Lapsed Policy 1. A grace period of 30 days from the due date of payment of Annual Target Premium shall be allowed for payment of unpaid Annual Target Premium. The Policy can be revived within 36 months of its date of lapse by the Company in its sole discretion, if the Policyholder: i) gives us a written request to revive the Policy; and ii) produces evidence of insurability acceptable to us as per our underwriting practices, and iii) pay us all overdue Annual Target Premiums and unpaid charges. 2. During the grace period, we will accept the lapsed notice amount without interest. The insurance coverage continues during the grace period and in case of death of Life Insured during the grace period, we will pay the Death Benefit.

TERMS AND CONDITIONS We urge you to please read this brochure and benefit illustration and understand the plan details, how it works and the inherent risks involved before your decide to purchase. Unit Prices: Unit price of a fund will be determined by dividing the net asset value of the fund by the outstanding number of units on the fund valuation date. The value of a Fund will be determined and based on the market value/fair value at which assets referenced to such Fund can be respectively purchased or sold, plus the respective cost of purchasing or minus the cost of selling the assets, plus the value of current assets, plus any accrued income net of fund management charges, less the value of current liabilities, less provisions, if any. The value of Funds may increase, decrease or remain unchanged accordingly. The unit pricing shall be computed based on whether the Company is purchasing (appropriation price) or selling (expropriation price) the assets in order to meet the day to day transactions of unit allocations and unit redemptions, i.e. the Company shall be required to sell/purchase the assets if unit redemptions/allocations exceed unit allocations/redemptions at the valuation date. The Appropriation price shall apply in a situation when the Company is required to purchase the assets to allocate the units at the valuation date. This shall be the amount of money that the Company would put into the fund in respect of each unit it allocates in order to preserve the interests of the existing policyholders. The Expropriation price shall apply in a situation when the Company is required to sell assets to redeem the units at the valuation date. This shall be the amount of money that the Company will take out of the fund in respect of each unit it cancels in order to preserve the interests of the continuing policyholders. In respect of premiums received upto 3:00 p.m. under a local cheque or a demand draft payable at par or by way of cash, the closing NAV of the day on which the premium is received shall be applicable. In respect of premiums received after 3:00 p.m, the closing NAV of the next business day shall be applicable. In respect of premiums received under outstation cheques/demand drafts, the closing NAV of the day on which the cheques/demand draft is realised shall be applicable. All requests for switch, redirection or partial withdrawal received upto 3:00 p.m. will be processed at the closing NAV of the day on which the request is received. All such requests received after 3:00 p.m. will be processed at the closing NAV of the next business day. Exclusions: If the life insured dies by suicide, whether sane or insane, within one year from the date of acceptance of risk or the date of any revival of this policy, all risks under the policy shall come to an end simultaneously. In such an event, we will pay only the fund value, if any, of the policy. Free Look period: The policyholder has a period of 15 days from the date of receipt of the policy to review the terms and conditions of the policy and where the policyholder disagrees to any of those terms or conditions, he has the option to return the policy stating the reasons for his objections, upon which he shall be entitled to refund of an amount which shall be equal to the non-allocated premium plus charges levied by cancellation of units plus fund value as at the date of cancellation of policy less expenses incurred on medical examination and on account of stamp duty. Prohibition of Rebates: Section 41 of the insurance act, 1938 states: No person shall allow or offer to allow, either directly or indirectly, as an inducement to any person to take or renew or continue an insurance in respect of any kind of risk relating to lives or property, in India, any rebate of whole or part of the commission payable or any rebate of the premium shown on the policy, nor shall any person taking out or renewing or continuing a policy accept any rebate, except such rebate as may be allowed in accordance with the published prospectuses or tables of insurer. Full Disclosure & Incontestability: This policy has been issued on the representation of the policyholder that he has made full disclosures of all relevant facts and circumstances. Any concealment, non-disclosure, misrepresentation or fraud by the policyholder shall render the policy liable for cancellation and shall be grounds for the Company to avoid all or any liability. If it deems fit, the Company may also forfeit the Premium(s) received. Section 45 of the insurance Act, 1938 states that under the provisions of Section 45 of the insurance Act, 1938: No policy of life insurance effected after the expiry of two years from the date on which it was effected be called in question by an insurer on the ground that a statement made in the proposal for insurance or in any report of a medical officer or referee or friend of the insured or in any other document leading to the issue of the policy, was inaccurate or false, unless the insurer shows that such statement was on a material matter or suppressed facts which it was material to disclose and that it was fraudulently made by the policyholder and that the policyholder knew at the time of making it that the statement was false or that it suppressed facts which it was material to disclose. Provided that nothing in this section shall prevent the insurer from calling for proof of age at any time if he is entitled to do so and no policy shall be deemed to be called in question merely because the terms of the policy are adjusted on subsequent proof that the age of the life insured was incorrectly stated in the proposal.
