Participating states All small groups (2-50) have renewal options, based on state regulations. In some states, customers also may be able to benefit from our 2013 Small Group Premium Savings Program. Client renewal options Clients have the option to: 1 Cancel their current policy and add a new 12 month policy with a new renewal date of Sept., Oct., Nov. or Dec. 2013. In some states, customers may be able to retain current benefits and rate levels through September-December 2014. 2 Maintain their existing policy and renewal cycle ending in 2014 and move to plans and rates that comply with 2014 ACA regulations at their next schedule renewal. Note: State regulations may apply to options 5

Notification deadlines: Premium Savings Program Available to new and renewing business with July-December effective dates in participating states New or Current 2013 renewal July June 1 August July 1 September August 1 October September 1 November October 1 December November 1 Premium Savings Program Notice/Decision (no UW, flat rate) 30 days Note: If a July December customer misses the deadline for the Premium Savings Program, they can use the Standard process to cancel their current policy and initiate a new one with a new renewal date (see following slide). Standard cases will be re-underwritten and receive new rates. 6

Broker Dashboard & Customer List Customer analysis considerations We developed tools to help you and your customers navigate anticipated 2014 ACA changes. These tools are useful, but have their limitations, since the regulatory landscape continues to evolve. Estimates of 2014 pricing change are illustrative and do not bind Aetna Factors included in this analysis: rate up, demographics, industry and group size Factors not included in this analysis include product and rating area changes We anticipate final January 2014 pricing will be available in or around October 2013 Subscriber totals are based on the count from the last renewal period 16

The Customer Agreement has two parts Renewal Change Request Customer requests termination of current policy and issuance of new policy Acknowledges Aetna s right to adjust rates for any benefits changes Available now on Producer World New Plan Effective Date New group policy, effective date and new Plan Year Notice of consolidated renewal date will apply to ALL Aetna benefits (medical, dental, disability, life, etc.) Plan sponsor is responsible for all ERISA requirements, including SBC delivery Confirmation that Cafeteria Plan documents have been adapted, if needed Agreement that benefit accumulators will reset with the new Plan Year 17
