Introduction Open Enrollment in isolved allows you to elect your benefits through Employee Self Service. To access Open Enrollment, log in to isolved at https://hkp-usa.com by entering your username and password. If you have not received your username or password or have a problem logging in to isolved, contact HR or your administrator. Once logged in to isolved, choose Open Enrollment from the Employee Self Service menu. Welcome Enrollment begins on the Welcome page. The welcome page displays the enrollment period and any documents and links that have been attached to the welcome message. Move through the wizard by clicking Next or Previous or by selecting a page from the Open Enrollment Menu. As you move through Open Enrollment, a green checkmark icon will appear next to completed pages in the menu. Pages requiring more information will have an orange exclamation point icon next to the title. isolved 1 May 2015
Beneficiaries and Dependents Add contacts on the Beneficiaries and Dependents page. Dependents are required for you to select coverage options that include dependents. Beneficiaries may be required for disability and life insurance plans. Click Add New to enter a new contact or click Edit to update an existing one. Enter the contact s name and select his or her Relationship to you. If the contact will be included as a dependent on any of your benefits, select the Dependent checkbox. When the Dependent checkbox is selected, the contact will appear on plan pages that require dependents. Birth Date and SSN are required for contacts listed as dependents. If the dependent does not share your address, enter the dependent s Address information. If the contact will be included as a beneficiary on any of your benefits, select the Beneficiary checkbox. When the Beneficiaries checkbox is selected, the contact will appear on plan pages that require beneficiaries. isolved 2 May 2015
Tobacco User Option If you use tobacco, select the Tobacco Use checkbox. Cost Analysis The Cost Analysis displays all your available plan and coverage options, along with the corresponding deduction schedule, deduction amount, and annual deduction total. Scroll to the bottom of the page and click Print to open a printable copy of the page. Amounts that vary based on the coverage amount selected will show a single asterisk in the Deduction Amount column. Amounts that vary based on the employee contribution will show two asterisks in the Deduction Amount column. isolved 3 May 2015
Current Elections The Current Elections page shows any plans in which you are currently enrolled. Select the Keep checkbox to re-elect your existing coverage for the coming benefit period. Benefit Plan Types Pages for each benefit type included in the enrollment period appear in Open Enrollment. When multiple plans are included under the same benefit type, the page will display a list of all plans. Select the plan in which you would like to enroll. If you do not wish to enroll in any plan for the benefit type, select the waive option. isolved 4 May 2015
401(k) 401(k) plans include amount and percent fields. To enroll in a 401(k) plan, enter an amount, percent, or combination. Beneficiaries can also be included on 401(k) plans. isolved 5 May 2015
Medical, Dental and Vision Medical, dental and vision plans use the same basic set up. Select the Coverage. If the coverage selected requires dependents, a message will appear prompting you to select the required number of dependents. If you do not have enough dependents, you will not be able to enroll in the plan. You can return to the Beneficiaries and Dependents page to add dependents or select a coverage option that you qualify for. Select the Include checkbox to add dependents to the plan. If a plan requires that a primary care physician be listed, the Primary Care Physician field will appear on the plan page. If the Primary Care Physician field appears on the page, enter your and your dependents primary care physician. isolved 6 May 2015
Health Savings and Flexible Spending Accounts Health savings and flexible spending accounts require that you enter an Annual Target Amount. The Per Pay Amount will populate automatically based on your Annual Target Amount. The Par Pay Amount will be your regular deduction amount. If you enter an amount outside the required minimum and maximum values for plan, a message will appear prompting you to enter a value within the limits. HSAs may require that you also be enrolled in a high-deductible health plan. If you wish to enroll in an HSA and see a message similar to the one below appear on the HSA page, return to the Medical page and select a high-deductible health plan. FSAs may require that you not also be enrolled in an HSA plan. isolved 7 May 2015
Disability and Life Insurance Plans Several types of plans may fall under this category, including long- and short-term disability, group-term life, accidental death and dismemberment, and critical illness insurance. The example below uses an employee life insurance plan, but the fields on the page will be the same or similar for all disability and life insurance plans. Select your Requested Coverage Amount. Once you select your Requested Coverage Amount, the Actual Coverage Amount will default to the approved underwriting value. You must include at least one beneficiary. If you need to add beneficiaries, you can return to the Beneficiaries and Dependents page. Select the Include checkbox to add a beneficiary. Beneficiaries must have a Primary Percent entered. The primary percentages between beneficiaries must equal 100. A Contingent Percent can also be entered for beneficiaries. Contingent percentages between beneficiaries must also equal 100. isolved 8 May 2015
Benefit Summary The Benefit Summary displays your elected benefits. A printable copy of the Benefit Summary can be opened by clicking Printable Version. isolved 9 May 2015
Confirmation Statement The Confirmation Statement is the final page in Open Enrollment. It also lists a summary of the employee s elected benefits. If there are plan types that still require selection for the employee to complete enrollment, a message at the top of the page will list the plan types that still require action. (The required action may be selecting a waive option.) When you have met all the requirements, click Benefits Acknowledgement to enter your e-signature. isolved 10 May 2015
A message will appear asking you whether you are sure that you wish to sign your acknowledgement. Click OK to sign your acknowledgement or click Cancel to return to the page without signing. To complete your enrollment, click Submit My Benefit Elections. The enrollment status in the top-right corner of the page will change to complete. Your elections will be sent to the assigned reviewer for approval. You can still return to Open Enrollment at any time during the enrollment period to make updates to your elections. isolved 11 May 2015
