ST. LUKE S HOSPITAL - 2016 OPEN ENROLLMENT EMPLOYEE SELF SERVICE INSTRUCTIONS The following document will provide you instructions on how to access Lawson Employee Self Service (ESS) and enroll in 2016 benefits from within the hospital s intranet. LOGGING INTO ESS PULL UP THE INTRANET SITE First you will need to open a browser window and go to the hospital s intranet page: http://axpwas01.slh.stlukes.com/wps/portal - On most hospital computers simply opening the browser will start you off on this page.

LOG INTO THE INTRANET SITE Once the page has loaded, you will need to log in using your network username and password. If you do not know either, you will need to contact the IS Help Desk at extension 4357 (HELP). Otherwise, look for the User ID and Password fields in the upper right hand corner of the page and fill them in with the appropriate information, then click the Login button.

PULL UP AND LOG INTO EMPLOYEE SELF SERVICE The link to pull up Employee Self Service (ESS) will not appear until you have logged into the intranet site. Once you have done so. Scroll down the page to the Lawson Employee Self Service Banner located in the box labeled Lawson Employee Self Service and click the banner. Once you have done this, a window from ESS will pop up requesting that you log in. Use the same username and password you used to log into the intranet and click the Login button. If you have issues with your username or password, please contact the IS Help Desk at extension 4357 (HELP).

NAVIGATING TO THE OPEN ENROLLMENT LINK Once this is done you should be logged into Lawson and ready to navigate to the enrollment section of ESS. Inside the Employee Self-Service section of the left hand sidebar, click the Benefits link. This will open the Benefits sub-menu, from here click Open Enrollment to start the enrollment process. 1. 2.

BENEFITS ENROLLMENT You have now started the process of enrolling for 2016 benefits. Click the Continue button once you ve finished reading the page.

DEPENDENTS The next screen shown will depend on whether you have dependents already listed in the system. If you have no dependents listed, you will see the below screen. Otherwise you will be presented with your list of active dependents. On this page please review the information for any dependents you have, update any information that has changed or is missing, and if you need to add dependents to your coverage this year, add them. Please remember that if you are adding dependents this year, their enrollment process will not be complete until you drop by the Human Resources Department with the appropriate documentation (i.e. marriage or birth certificates). To review the information for an existing dependent, click their name. To add a dependent, click the Add button. Once you are finished reviewing and adding dependents as necessary, click the Continue button to go to the next step of enrollment.

DEPENDENTS UPDATING/REVIEWING When editing a dependent, either an existing one or a new one you are adding, the right hand side of the screen will provide you an area to enter their information. Depending on the size of your monitor and the window you have open, this area may have scroll bars on it that allow you to move the view up and down so that you may see the all the fields in this area. Please fill in all of the fields which have an red asterisk (*) next to them, as these are required fields for dependents. All other fields are optional. Once you have filled in all the necessary information, in order to save, click the Update button. As a reminder, if your window is too small to fit the entire area into the screen, you may need to use the scroll bars to scroll down and make the update button visible.

DEPENDENTS - SPOUSES Starting in 2016, if you are covering a spouse on your medical plan, you are required to indicate if they have access to medical insurance outside of St. Luke s benefit plans, including through another employer, Medicare, or any other plans. As such, if you have a spouse listed as an active dependent, you will not be able to continue until you update your record to indicate if they have access to this sort of coverage. The system will indicate this via the highlighted text on the screen as well as removing the Continue button. To continue, click the entry for your spouse in the list and update the field shown below. ENROLLMENT ORDER The next screen that appears will provide you with the order of the benefits you ll be enrolling in. This is purely informational; you are not required to do anything on this screen. Click the Continue button to move forward.

CURRENT BENEFITS This screen lists your current 2015 benefit enrollments if any. This is purely informational; you are not required to do anything on this screen. Click the Continue button to move forward.

ENROLLMENT PROCESS For each of the plans that you may enroll in, you will be shown what you are currently enrolled in for 2015. If you are eligible to enroll in the same plan for 2016, you will have the option to keep the same coverage. Otherwise you will be instructed to pick a new plan. While the screen shot below is for the medical plans, the same basic instructions apply for all of the plans. * NOTE Due to programming changes, there are special instructions for medical enrollment in 2016 which will be presented at the end of this section. If you are keeping your 2015 elections, click the dot next to: Keep the same coverage. If you are keeping the same plan, but changing the type of dependents being covered, click the dot next to: Change dependent coverage. If you are changing plans (i.e. moving from a basic to premium coverage or vise versa, or switching to/from a declination) click the dot next to: Select a different medical plan. Once you ve made your selection, click the Continue button to move to the next screen.

CHOOSING YOUR PLAN The next screen that will appear depends on what you selected in the previous one. If you opted to change your plan you will first be prompted to pick a new plan: Select the appropriate plan, then click Continue.

CHOOSING YOUR DEPENDENT COVERAGE Once you ve done this, or if you opted to keep your previous plan and just change your dependent coverage type, you will be prompted to pick a coverage type: As a reminder the system will only show you the options available to you based on the dependents you have already entered into the system. If you are missing an option, please go back and review your dependents. Select the appropriate plan, then click Continue. Once you have done this you will be prompted to select which of your dependents to cover under the plan, click in the box next to their name to enroll them and then click Continue.

VERIFYING ELECTIONS Once you have finished your selections you will be prompted to review and confirm the information you have provided. If you have made an error, click the Previous button to move back a step, and continue clicking the previous button until you ve reached the area you need to correct. Otherwise, click the Continue button to move on to the next plan.

2016 MEDICAL COVERAGE CHANGES Due to the program changes concerning the Medical plan in 2016, there are a few items to be aware of while enrolling for 2016. Previously the PREMIUM MEDICAL plan was only available to participants in the Passport To Wellness program. This has been opened to all benefit eligible employees this year. However, participants in the wellness program will receive a discount to their monthly premium. In addition, participation in the wellness program was extended to spouses. Please note to receive the discount for Passport to Wellness enrollment when covering a spouse, both the employee and spouse are required to have completed enrollment. To help clarify what factors are affecting the monthly premium for coverage, a message has been added to the screen displaying the available coverage options and the monthly rate. This replaces the WELLNESS tag that was added to the plan name for 2015 enrollment. For 2016 enrollment, the available plans will not list - WELLNESS at the ends of their names.

FINISHING ENROLLMENT Once you have finished indicating your enrollment choices in all the plans available to you, you will be taken to the summary screen where you may review all of your choices. Please review the screen and read the authorization statement at the bottom of the page. If you have no further edits that you wish to make to your benefits, click the Accept button. If you wish to make a change to what you ve selected, click the Make Changes button. If you wish to exit out without saving your information, click the Exit button. Once you click the Accept button, you will be offered the option to print off your benefit selections for your records. Click the Continue button to send your information to the printer. Congratulations, you ve finished your 2016 BENEFITS ENROLLMENT.

MAKING CHANGES If you elect to make changes to your selections, either now or later on, you will be prompted to select which plans to edit. Place checks next to the appropriate boxes for the plans you wish to edit then click the continue button. The system will walk you through the same basic steps for enrollment as it had before, for the plans you selected.

IMPORTANT INFORMATION The HEALTH SAVINGS ACCOUNT (HSA) plan is required by law to be tied to a High Deductable plan and an employee may not enroll in both the HSA and MSA plans. If you wish to enroll in the new HEALTH SAVINGS ACCOUNTS (HSA) Please remember that you must enroll in a HIGH DEDUCTABLE MEDICAL PLAN in order to be eligible for enrollment. You will not be given the option by the system if you enroll in anything else. If you wish to enroll in an MEDICAL SAVING ACCOUNT (MSA) Please remember that you may not enroll in a HIGH DEDUCTABLE MEDICAL PLAN if you wish to be eligible for enrollment. You will not be given the option by the system if you enroll in a HIGH DEDUCTABLE MEDICAL PLAN. MEDICAL SAVINGS ACCOUNT (MSA) If you wish to receive a debit card for your account, You need to select the debit card plan option to indicate your preference.
